Cytomegalovirus (CMV) Co-infection in Treatment-Refractory Immune Checkpoint Inhibitor-Associated Colitis: A Case Report.

Immune checkpoint inhibitors (ICIs) have been increasingly used for the treatment of various malignancies, and ICI-associated colitis is one of the most common side effects. Here we report a 55-year-old man with metastatic melanoma who was initially treated with nivolumab-relatlimab. Due to side effects, therapy was transitioned to ipilimumab, followed by combination ipilimumab and nivolumab. Ten days after the third dose of ipilimumab and nivolumab, he presented to the emergency department with nausea, vomiting, bloody diarrhea, and fever. Stool pathogen panel and Clostridioides difficile pol

Host cell responses to Candida albicans biofilm-derived extracellular vesicles.

Candida albicans is a prevalent fungal pathogen responsible for infections in humans. As described recently, nanometer-sized extracellular vesicles (EVs) produced by C. albicans play a crucial role in the pathogenesis of infection by facilitating host inflammatory responses and intercellular communication. This study investigates the functional properties of EVs released by biofilms formed by two C. albicans strains-3147 (ATCC 10231) and SC5314-in eliciting host responses. We demonstrate the capability of C. albicans EVs to trigger reactions in human epithelial and immune cells. The involvemen

Pathogenesis of rabies in a pregnant HIV immune-compromised woman in Zambia: A case report.

Rabies is a fatal neglected tropical zoonotic disease caused by neurotropic viruses of the genus Lyssavirus in the family Rhabdoviridae. We report the disease progression in a 30-year-old woman, in her eighth pregnancy, living with human immunodeficiency virus (HIV) on antiretroviral therapy (ART), who presented with neurological symptoms including aggression, restlessness, fever and vomiting 20 days following rabies exposure through multiple dog bites on the face and upper limbs. She had received a 4-dose regimen of rabies post-exposure prophylaxis (PEP), starting 2 days after exposure, with

Immunotoxicity of bis(tri-n-butyltin)oxide in the rat: effects on thymus-dependent immunity and on nonspecific resistance following long-term exposure in young versus aged rats.

To investigate whether immune function suppression observed in an earlier study after short-term bis(tri-n-butyltin)oxide (TBTO) exposure also occurred after long-term treatment, function studies for specific and nonspecific resistance were performed after exposure of weaned male rats to diets containing 0, 0.5, 5, or 50 mg TBTO/kg for 4-6 and 15-17 months. Treatment for 4.5 months had no effect on body weight but reduced thymus weight at 50 mg/kg. Regarding the thymus-dependent immunity, delayed-type hypersensitivity reactions to ovalbumin and tuberculin were not depressed, in contrast to the

[Dermatitis herpetiformis: a review].

BACKGROUND: Dermatitis herpetiformis (DH) is a rare auto-immune bullous disease characterized by its almost constant association to gluten sensitivity. OBJECTIVE: Review of literature about epidemiology, physiopathology, clinical data and treatment of DH. METHODS: Research on Medline and Embase database without any time limit until April 2010. Because of the lack of randomized therapeutic trials in DH, retrospective series and case reports have been analyzed. RESULTS: DH is related to auto-antibodies against epidermal transglutaminase, which belongs to the same family as tissue transglutami

Pharmacological potential of the histone demethylase inhibitor GSK-J4 in mitigating NF-κB-mediated inflammatory cascades associated with autoimmune disorders.

Inflammasome-related sterile inflammation is a leading cause of several autoimmune and rheumatic disorders, including rheumatoid arthritis (RA), systemic lupus erythematosus (SLE), and inflammatory bowel disease (IBD). Although the exact causes of rheumatic diseases remain unclear, it is evident that ongoing inflammatory reactions are the main factors driving their development. Jumonji domain-containing protein-3 (JMJD3) is an epigenetic modifier that can strongly promote the inflammatory process in many diseases by impacting the Nuclear Factor-κB (NF-κB) signaling. Meanwhile, GSK-J4 has been

Bridging Resource Gaps in Toxic Epidermal Necrolysis Management: A Case Report Highlighting Innovative Care.

Toxic epidermal necrolysis (TEN) is a rare but potentially life-threatening dermatological condition. It is characterised by diffuse exfoliation of >30% of the skin and mucous membranes due to immune-mediated destruction of the epidermis. It is thought to be due to a hypersensitivity reaction to a variety of medications. The mainstay of treatment involves withdrawal of the potential offending agent, supportive care and use of non-adherent dressings. Pre-packaged dressings are expensive or not readily available in our locality; hence, the need for improvisation, to manage the condition and impr

Risks and Benefits of Nonsteroidal Anti-inflammatory Drugs in the Hemostatic System.

Chronic use of multiple drugs increases risk of adverse drug reactions, drug interactions and poor therapeutic adherence. In elderly or frail patients, some drugs may become ineffective or even harmful with advancing age and worsening clinical conditions. Nonsteroidal anti-inflammatory drugs (NSAIDs) have been extensively used for decades and, as a class, are both often inappropriately prescribed and involved in polypharmacotherapy untoward effects. In view of the dynamic and mutual relationship between inflammation and hemostasis in various clinical scenarios, use of anti-inflammatory agents,

VXX-401, a novel anti-PCSK9 vaccine, reduces LDL-C in cynomolgus monkeys.

Atherosclerotic cardiovascular disease (ASCVD) remains the leading cause of disease burden in the world and is highly correlated with chronic elevations of LDL-C. LDL-C-lowering drugs, such as statins or monoclonal antibodies against proprotein convertase subtilisin/kexin type 9 (PCSK9), are known to reduce the risk of cardiovascular diseases; however, statins are associated with limited efficacy and poor adherence to treatment, whereas PCSK9 inhibitors are only prescribed to a "high-risk" patient population or those who have failed other therapies. Based on the proven efficacy and safety prof

ICAM-1-mediated adhesion of peripheral blood monocytes to the maternal surface of placental syncytiotrophoblasts: implications for placental villitis.

Accumulation of maternal monocytes in the villous/intervillous space (villitis) is associated with increased risk of perinatal morbidity and mortality and may initiate in utero transmission of cell-associated infectious agents such cytomegalovirus and HIV-1. We have developed an in vitro model of trophoblast syncytialization and have investigated the adhesive interactions between this tissue and peripheral blood monocytes. We show that monocytes strongly adhere to cultured syncytiotrophoblasts (STs) and that treatment with the inflammatory cytokines interferon-gamma, tumor necrosis factor-alph

Immunologic injury in measles virus infection. III. Presence and characterization of human cytotoxic lymphocytes.

Human peripheral blood leukocytes (PBL) from patients with chronic measles virus infection (SSPE) or from immune, adult humans (convalescent from acute childhood measles virus infection) are cytotoxic for target cells infected with measles virus as measured by a 51Cr assay. Specific release of 51Cr by immune PBL occurred both with and without human antibodies added to measles virus-infected cultures. Maximal killing in the absence of added antibodies to measles virus was usually detected only after 15 to 18 hr of incubation and with a high PBL to target ratio (100:1). When antibody to measles

GelMA-GelDopa-Sr double-network hydrogel promotes skin regeneration by enhancing angiogenesis and macrophage polarization.

INTRODUCTION: Chronic skin defects typically exhibit persistent inflammation, damaged cell regeneration, and insufficient vascularization, all of which severely impede the healing process. Traditional skin dressings merely serve to cover the wound and do not actively promote wound healing. They lack anti-inflammatory capabilities and only minimally regulate immune cells, and their poor biocompatibility can lead to foreign body reactions. Some traditional dressings adhere to the defect area, and during dressing changes, they may cause secondary trauma, resulting in pain and tearing of new tissu

Complement-dependent B-cell activation by cobra venom factor and other mitogens?

It has been proposed that two distinct signals are required for the triggering of the precursors of antibody-forming bone marrow-derived cells (B cells): (a) the binding of antigen or of a mitogen to the corresponding receptor sites on B-cell membranes and (b) the interaction of activated C3 with the C3 receptor of B lymphocytes. There is growing evidence that B-cell mitogens and T (thymus-derived cell)-independent antigens are capable of activating the alternate pathway of the complement system (bypass). Therefore, the effect of another potent bypass inducer was investigated with regard to B-

Olmesartan-Induced Gastropathy: An Important Cause to Think about in Refractory Peptic Ulcer Disease.

BACKGROUND: Angiotensin receptor blockers are a pharmacological class widely used as antihypertensive therapy. Recently, a relationship between these agents and gastrointestinal disease has been described, namely, enteropathy, gastropathy, and microscopic colitis. The mechanism is unknown, but it is thought that a cell-mediated immune reaction is involved and does not appear to be a class effect. Treatment consists of stopping the drug and rechallenge can confirm the diagnosis. CASE PRESENTATION: An 85-year-old man with a history of hypertension treated with olmesartan/hydrochlorothiazide for

Inflammatory abdominal aortic aneurysm.

Inflammatory abdominal aortic aneurysm (AAA) accounts for 5% to 10% of all cases of AAA and differs from typical atherosclerotic AAA in many important ways. Although both inflammatory and atherosclerotic AAA most commonly affect the infrarenal portion of the abdominal aorta, patients with the inflammatory variant are younger and usually symptomatic, chiefly from back or abdominal pain. Unlike patients with atherosclerotic AAA, most with the inflammatory variant have an elevated erythrocyte sedimentation rate or abnormalities of other serum inflammatory markers. Computed tomography and magnetic
