Dissemination of exposure in the treatment of anxiety disorders and post-traumatic stress disorder among German cognitive behavioural therapists.

The efficacy of exposure is beyond doubt, which is reflected in guidelines recommending its application in the treatment of anxiety disorders and post-traumatic stress disorder (PTSD). Research suggests exposure to be underutilized in clinical practice in the United States and Europe. A systematic investigation of the dissemination of exposure in Germany is lacking. The present study examined the dissemination and application frequency of exposure among German cognitive behavioural therapists working in routine care. In an online-based survey, 331 psychotherapists provided information on treat

Predictors of treatment noninitiation, dropout, and response for cognitive behavioral therapy for trauma nightmares.

OBJECTIVE: This report aims to extend previous findings on predictors of treatment dropout and response within nightmare treatment. METHOD: Factors predicting treatment noninitiation, dropout, and response were examined in 70 trauma-exposed individuals treated in a randomized controlled trial of 2 conditions (exposure [EX] and nonexposure) of a therapy for trauma nightmares. RESULTS: Together, treatment noninitiation and dropout was predicted by younger age for the EX condition only and by fewer nights with nightmares at baseline for both conditions. Treatment noninitiation was predicted by

Virtual reality exposure therapy for social anxiety disorder: a randomized controlled trial.

OBJECTIVE: This is the first randomized trial comparing virtual reality exposure therapy to in vivo exposure for social anxiety disorder. METHOD: Participants with a principal diagnosis of social anxiety disorder who identified public speaking as their primary fear (N = 97) were recruited from the community, resulting in an ethnically diverse sample (M age = 39 years) of mostly women (62%). Participants were randomly assigned to and completed 8 sessions of manualized virtual reality exposure therapy, exposure group therapy, or wait list. Standardized self-report measures were collected at pre

Exposure therapy, D-cycloserine, and functional magnetic resonance imaging in patients with snake phobia: a randomized pilot study.

OBJECTIVE: d-Cycloserine may enhance fear extinction. The effects of d-cycloserine on human brain function are not well understood, with findings suggesting that d-cycloserine could augment exposure therapy via its effects on the neural substrates of emotional learning and extinction or by acting upon different neural pathways. The aim of this exploratory study was to investigate differences in neural response in patients receiving d-cycloserine or placebo in addition to exposure therapy. METHOD: Twenty adults with snake phobia (DSM-IV specific phobia) received 50 mg of d-cycloserine or place

Posttraumatic cognitions and PTSD symptom change in massed prolonged exposure: The impact of treatment delivery on mechanisms of change.

BACKGROUND: Prolonged Exposure (PE) is an effective treatment for PTSD and clarification of mechanisms of change can inform training and continued improvements. Posttraumatic cognitions have been shown to predict subsequent symptom change in PTSD treatment. The objective of the current study was to examine how cognitions related to symptom change in massed-delivered PE. METHODS: The current study used cross-lagged regression analyses to examine the relationship between change in posttraumatic cognitions and PTSD and depression severity in a two-week massed PE program for Veterans with PTSD.

Therapeutic alliance in intensive PTSD treatment with a therapist rotation model.

BACKGROUND: Therapeutic alliance is a robust predictor of treatment outcome. However, little is known about its development and relationship with symptom changes in intensive trauma-focused treatment for post-traumatic stress disorder (PTSD) using a therapist rotation model. OBJECTIVE: This study examined (1) how therapeutic alliance develops during intensive trauma-focused treatment with therapist rotation, (2) its bidirectional relationship with PTSD symptom change, and (3) exploratory associations between baseline patient characteristics, preferences, alliance ruptures, and perceived relat

Mirror exposure in adolescents with anorexia nervosa: a feasibility study on body image disturbance and attentional bias.

PURPOSE: Body image disturbances (BID) are central to anorexia nervosa (AN), influencing its development, maintenance and relapse. While mirror exposure (ME) has been shown to reduce BID in adults, research in adolescents is limited. This pilot randomized controlled trial examined the feasibility and effects of ME, added to treatment as usual (TAU), in adolescent inpatients with AN. In addition, we explored changes in body-related attentional bias as a potential mechanism of change. METHODS: Adolescent female inpatients with AN were randomized to receive either 12 ME sessions over 4 weeks (ME

Do laboratory fear extinction, avoidance and generalization predict exposure outcomes for fear of spiders?

This study assessed the predictive potential of laboratory conditioning processes (extinction, avoidance, and generalization) for exposure therapy outcomes in 112 spider-fearful women. Three aversive conditioning tasks were followed by a baseline assessment of spider fear, virtual reality exposure training, a post assessment after one week and a three-month follow-up assessment. Initial analyses indicated significant positive correlations between reduced extinction, increased avoidance of a conditioned threat stimulus, and overgeneralization, particularly evident in verbal measures. In subsequ

Graded exposure therapy for adults with persistent symptoms after mTBI: A historical comparison study.

Fear avoidance behaviour is associated with slow recovery from mild traumatic brain injury (mTBI). This study is a preliminary evaluation of graded exposure therapy (GET), which directly targets fear avoidance behaviour, for reducing post-concussion symptoms (PCS) and disability following mTBI. In a historical comparison design, we compared two groups from independent randomized trials. The GET + UC group (N = 34) received GET (delivered over 16 videoconference sessions) in addition to usual care (UC). The historical comparison group (N = 71) received UC only. PCS severity (Rivermead Post Conc

Interpersonal psychotherapy versus sertraline for women with posttraumatic stress disorder following recent sexual assault: a randomized clinical trial.

Background: Sexual assault often triggers posttraumatic stress disorder (PTSD), a potentially chronic severe mental disorder. Most guidelines recommend selective serotonin reuptake inhibitors (SSRIs) and trauma-focused psychotherapies as treatment options. Interpersonal Psychotherapy (IPT), adapted for PTSD (IPT-PTSD), focuses on interpersonal consequences of trauma rather than confronting the trauma itself. Studies have found IPT-PTSD efficaciously reduced PTSD symptoms with limited attrition. No efficacy trials have compared IPT-PTSD and SSRI. We hypothesized IPT would reduce PTSD, anxiety,

Change in Event Centrality and Posttraumatic Stress Disorder Symptoms During Intensive Treatment.

Event centrality, defined as the extent to which a traumatic event becomes a core component of a person's identity (Berntsen & Rubin, 2006), is both a correlate and predictor of posttraumatic stress disorder (PTSD) symptoms, over and above event severity. These findings suggest that decreasing the perceived centrality of a traumatic event to one's identity might result in decreases in PTSD symptom severity. To date, few studies have examined how centrality is affected by PTSD treatment. The present study tested the hypotheses that change in centrality would be associated with both change in PT

Cortical oxygenation during exposure therapy - in situ fNIRS measurements in arachnophobia.

Exposure therapy is a well-studied and highly efficacious treatment for phobic disorders. Although the neurobiological model of fear is well underpinned by various studies, the mechanisms of exposure therapy are still under discussion. Partly, this is due to the fact that most neurophysiological methods like fMRI are not able to be used in the natural therapeutic settings. The current study used in situ measurements of cortical blood oxygenation (O2Hb) during exposure therapy by means of functional near-infrared spectroscopy. 37 subjects (N = 30 completers) underwent exposure therapy during 5

Chinese Therapists' Beliefs About Exposure Therapy for Anxiety and Obsessive-Compulsive Disorders.

OBJECTIVES: The goal of this study was to assess Chinese therapists' beliefs about exposure therapy and to examine the psychometric properties of the Chinese version of the Therapist Beliefs about Exposure Scale (TBES). Modification of therapists' beliefs about exposure therapy was also assessed following attendance at an exposure and response prevention therapy (ERP) training workshop. METHODS: A total of 203 therapists participated in the study. The TBES and a measure of anxiety sensitivity were administered in Chinese. After a half-day ERP training workshop, the Chinese version of the TBES

A randomized controlled trial of the judicious use of safety behaviors during exposure therapy.

Safety behaviors-actions performed to prevent, escape from, or reduce the severity of perceived threat-are typically eliminated during exposure therapy for clinical anxiety. Yet some experts have called for the strategic and "judicious use" of safety behaviors during exposure to improve treatment acceptability/tolerability without diminishing its efficacy. Empirical findings regarding this debate are mixed and existing work is subject to several methodological limitations. The current randomized controlled trial incorporated longitudinal design and multimethod assessment to compare the efficac

Building physiological toughness: Some aversive events during extinction may attenuate return of fear.

BACKGROUND AND OBJECTIVES: Although exposure therapy is an effective treatment for anxiety disorders, fear sometimes returns following successful therapy. Recent literature in animal models indicates that incorporating some aversive events into extinction training may offset these return of fear effects. METHODS: The effect of occasional reinforced extinction trials was investigated in a sample of thirty-nine participants using a fear conditioning and extinction paradigm. Participants either underwent traditional extinction procedures during which the conditional stimulus which had been paire

Can a one-hour session of exposure treatment modulate startle response and reduce spider fears?

Preliminary evidence suggests that 3 hours of behavior therapy can reduce fear responses to phobic stimuli. Most of this research, however, has relied on self-reports and clinician assessments, and failed to include a comparison group. To extend this literature, with 32 adults with spider phobia, we investigated the effects of a single hour of in vivo exposure on subjective and electrophysiological aspects of fear; comparisons were made to a wait-list control group. Pre- and post-assessments included phobia-relevant questionnaires and startle reflex responses to spider, negative, neutral and p

Failure to replicate the deleterious effects of safety behaviors in exposure therapy.

The current study attempted to replicate the finding obtained by Powers, Smits, and Telch (2004; Journal of Consulting and Clinical Psychology, 72, 448-545) that both the availability and utilization of safety behaviors interfere with the efficacy of exposure therapy. An additional goal of the study was to evaluate which explanatory theories about the detrimental effects of safety behaviors best account for this phenomenon. Undergraduate students (N=58) with high claustrophobic fear were assigned to one of three treatment conditions: (a) exposure only, (b) exposure with safety behavior availab
