Infant Mortality Rates Among US-Born and Foreign-Born Latinx Women: The Effect of Black Race.

OBJECTIVE: To determine the extent to which Black race is associated with the infant mortality rate (< 365 day, IMR) of births to US-born and foreign-born Latinx women. METHODS: Stratified and multivariable binominal log-linear regression analyses were performed on the 2010-2013 National Center for Health Statistics linked birth-death certificate files of singleton infants. RESULTS: The IMR of births to US-born Black Latinx women (N = 54,542) exceeded that of births to US-born White Latinx women (N = 1,320,084): 5.7/1000 vs 4.2/1000, RR = 1.4 (1.2, 1.5). In contrast, the IMR of births to for…

Infant and Youth Mortality Trends by Race/Ethnicity and Cause of Death in the United States.

IMPORTANCE: The United States has higher infant and youth mortality rates than other high-income countries, with striking disparities by racial/ethnic group. Understanding changing trends by age and race/ethnicity for leading causes of death is imperative for focused intervention. OBJECTIVE: To estimate trends in US infant and youth mortality rates from 1999 to 2015 by age group and race/ethnicity, identify leading causes of death, and compare mortality rates with Canada and England/Wales. DESIGN, SETTING, AND PARTICIPANTS: This descriptive study analyzed death certificate data from the US N

A care bundle including antenatal corticosteroids reduces preterm infant mortality in Tanzania a low resource country.

BACKGROUND: Preterm neonatal mortality (NM) has remained high and unchanged for many years in Tanzania, a resource-limited country. Major causes of mortality include birth asphyxia, respiratory insufficiency and infections. Antenatal corticosteroids (ACS) have been shown to significantly reduce mortality in developed countries. There is inconsistent use of ACS in Tanzania. OBJECTIVE: To determine whether implementation of a care bundle that includes ACS, maternal antibiotics (MA), neonatal antibiotics (NA) and avoidance of moderate hypothermia (temperature < 36°C) targeting infants of estimat

Perinatal periods of risk: analytic preparation and phase 1 analytic methods for investigating feto-infant mortality.

The Perinatal Periods of Risk (PPOR) methods provide the necessary framework and tools for large urban communities to investigate feto-infant mortality problems. Adapted from the Periods of Risk model developed by Dr. Brian McCarthy, the six-stage PPOR approach includes epidemiologic methods to be used in conjunction with community planning processes. Stage 2 of the PPOR approach has three major analytic parts: Analytic Preparation, which involves acquiring, preparing, and assessing vital records files; Phase 1 Analysis, which identifies local opportunity gaps; and Phase 2 Analyses, which inve…

Morbidity and mortality after surgery for congenital cardiac disease in the infant born with low weight.

OBJECTIVE: Low weight at birth is a risk factor for increased mortality in infants undergoing surgery for congenitally malformed hearts. There has been a trend towards performing surgery in patients early, and for amenable lesions, in a single stage rather than following initial palliative procedures. Our goal was to report on the current incidences of morbidities and mortality in infants born with low weight and undergoing surgery for congenital cardiac disease. METHODS: We made a retrospective review of the data from patients meeting our criterions for entry from July, 2000, through July, 2

Outdoor air pollution and infant mortality: analysis of daily time-series data in 10 English cities.

BACKGROUND: There is growing concern that moderate levels of outdoor air pollution may be associated with infant mortality, representing substantial loss of life-years. To date, there has been no investigation of the effects of outdoor pollution on infant mortality in the UK. METHODS: Daily time-series data of air pollution and all infant deaths between 1990 and 2000 in 10 major cities of England: Birmingham, Bristol, Leeds, Liverpool, London, Manchester, Middlesbrough, Newcastle, Nottingham and Sheffield, were analysed. City-specific estimates were pooled across cities in a fixed-effects met…

Infant mortality by gestational age and birth weight in Canadian provinces and territories, 1990-1994 births.

We compared gestational age-specific and birth weight-specific infant mortality in the Canadian provinces (excluding Ontario) and territories using the linked birth and death records for 1990-1994 births. Compared with Quebec, early neonatal mortality rates were higher in Saskatchewan, Alberta and Newfoundland among extremely small and preterm infants and among infants with no information on gestational age and birth weight on their records. Post- neonatal mortality rates were higher in Prince Edward Island, Manitoba, Saskatchewan, Alberta, British Columbia and the Northwest Territories among …

Framing the debate: can prenatal care help to reduce the black-white disparity in infant mortality?

Prenatal care has been identified as necessary to reducing the disparity in infant mortality between black and white infants. The purpose of this paper is to review determinants of the disparity and describe the contribution that prenatal care can make to modifying those determinants, biologic or social. When examined by birthweight categories, 25% of the disparity is due to excess deaths among normal birthweight infants (> or = 2500 g); 13% is due to excess deaths among moderate low birthweight infants (1500-2499g); and 62% of the disparity is due to excess deaths among very low birthweight i…

Effect of postpartum maternal or neonatal vitamin A supplementation on infant mortality among infants born to HIV-negative mothers in Zimbabwe.

BACKGROUND: Young infants are at risk of vitamin A deficiency. Supplementation of breastfeeding mothers improves the vitamin A status of their infants, but there are no data regarding its effect on infant mortality, and data on the effect of directly supplementing infants during the first few weeks of life are conflicting. OBJECTIVE: The objective was to measure the effect on infant mortality of supplementing neonates and their HIV-negative mothers with single, large doses of vitamin A during the immediate postpartum period. DESIGN: A randomized, placebo-controlled, 2-by-2 factorial design t…

Implication of Vaginal and Cesarean Section Delivery Method in Black-White Differentials in Infant Mortality in the United States: Linked Birth/Infant Death Records, 2007-2016.

Racial/ethnic disparities in infant mortality (IM) continue to persist in the United States, with Black/African Americans (AA) being disproportionally affected with a three-fold increase in mortality compared to Whites. Epidemiological data have identified maternal characteristics in IM risk such as preeclampsia, eclampsia, maternal education, smoking, maternal weight, maternal socioeconomic status (SES), and family structure. Understanding the social gradient in health including implicit bias, as inherent in the method of labor and delivery and the racial heterogeneity, may facilitate interve…

Development of a prediction model for mortality in infants undergoing therapeutic hypothermia for neonatal encephalopathy.

OBJECTIVE: To develop and internally validate a model predicting neonatal mortality in infants with neonatal encephalopathy requiring therapeutic hypothermia (TH), using national data. STUDY DESIGN: Data from 385 infants treated with TH across 19 hospitals (2016-2021) were analysed. Multivariable logistic regression with backward stepwise selection was applied. Discrimination was assessed using the C-statistic, with internal validation by bootstrapping. The THERM (Therapeutic Hypothermia Early Risk Model for Mortality) tool was developed to calculate individualised mortality risk. RESULTS: F

Modeling Oxygen Saturation Targets, Mortality, and Retinopathy Among Extremely Preterm Infants.

IMPORTANCE: According to the Neonatal Oxygenation Prospective Meta-Analysis, a higher oxygen saturation as measured by pulse oximetry (Spo2) target range, compared with a lower Spo2 target, may reduce mortality but increase the risk of treated retinopathy of prematurity (ROP). Choosing a target range at an individual center may depend on the local baseline risks and the value placed on prioritizing survival over the risk of severe retinopathy. OBJECTIVE: To estimate trade-offs between mortality and treated ROP associated with Spo2 targeting and the association between variation in baseline ri

Pertussis infection in critically ill infants: meta-analysis and validation of a mortality score.

BACKGROUND: Despite widespread vaccination programs, pertussis continues circulating within populations and remains a life-threatening infection in infants. While several mortality risk factors have been described, a comprehensive synthesis is lacking. We conducted a meta-analysis of studies investigating mortality risk factors in Pertussis infections and validated those factors in a large cohort. METHODS: Observational studies published in English were systematically searched in PubMed, EMBASE, and LiSSa databases from 01/2000 to 06/2024. The search yielded 816 unique citations. The primary
