Branhamella catarrhalis infections.

Branhamella (Moraxella) catarrhalis is responsible for a significant number of bronchopulmonary infections in adults, as well as otitis media and sinusitis in children. This gram-negative diplococcus is indistinguishable from Neisseria gonorrhoeae on gram-stained smear. Many strains of the organism produce beta-lactamase and are resistant to the penicillins and other beta-lactam antibiotics. When B. catarrhalis is the probable pathogen, a beta-lactamase-resistant antibiotic is the initial drug of choice in both adults and children.

Atypical surgical infections.

The management of infections requiring surgical drainage or debridement is a significant component of surgical education and practice. Standard Gram's stain and culture techniques usually allow ready identification and adjunctive therapy of the offending organism; however, the surgical patient population is changing. More patients who require surgery are elderly, critically ill, have been exposed to broad-spectrum antibiotics, or are otherwise immunosuppressed. We identified 33 infections in this group of 32 patients. The majority of the etiologic agents were not identified using traditional m…

Ocular streptococcal infections.

Streptococcal infections are an important cause of corneal ulcers, endophthalmitis, conjunctivitis, and dacryocystitis. We retrospectively reviewed 53 ocular streptococcal infections diagnosed and followed at Wills Eye Hospital between January 1, 1984, and July 1, 1986. Corneal ulcers (23 of 53; 43%) and endophthalmitis (18 of 53; 34%) constituted a majority of the streptococcal infections in this series. Streptococcal infection was also a common cause of conjunctivitis, but clinical follow-up data were sufficient for inclusion in this series in only a small fraction of conjunctivitis patients…

Subclavian catheter infections.

We retrospectively studied subclavian catheter infection rates involving 481 patients from October 1983 to September 1985. Over this period of time 496 subclavian catheters including 252 single lumen catheters (SLC) and 244 triple lumen catheters (TLC) were inserted. These studies involved all catheters inserted during the 24-month period. The Methodist Hospital IV Team regularly cared for and recorded catheter-related problems. Charts were then retrospectively reviewed and data accumulated. The incidence of catheter tip infections in SLC was 4/248 (1.6%) compared to an incidence of 12/232 (4.…

[Systemic Candida infections].

Seven cases of neonatal systemic candidiasis are summarized. This means an incidence of 8.3% respect total amount of newborns admitted and 3.7% of babies admitted in the intensive care unit. Clinical presentation was not specific. Five blood cultures were positive and in two cases meningitis was recognized. Two cases died. Six cases were treated with amphotericin B and 5-fluorocytosine and in one case renal toxicity appear.

Intra-uterine infections.

The incidence of intrauterine infections during pregnancy is estimated to be about 14% when laboratory methods of detection are used. The commonest infections are cytomegalovirus infection, congenital rubella followed by herpes simplex. Hepatitis B virus infection occurs fairly frequently in this region of the world. Congenital syphilis, now highly treatable, is largely under control. Preventive measures are necessary since the lesions caused by some intrauterine infections are permanent and damaging. Immunisation against hepatitis and rubella is effective. Most of these infections are spread …

[Genital chlamydial infections].

Chlamydia trachomatis is today the most frequent cause of sexually transmitted diseases. In spite of their usually mild, sometimes even asymptomatic course, urogenital infections with Chlamydia trachomatis can lead to severe complications (infertility, adnexitis) if not treated properly. Clinical features, complications, diagnosis and therapy are discussed in detail.

Devastating scalp infections.

The treatment of scalp wounds is occasionally complicated by infection. Although infected scalp wounds are generally limited and readily treated, they can progress to devastating proportions if not promptly and aggressively managed. This report highlights three such cases of severe infection: extensive subgaleal abscess, fatal necrotizing fasciitis, and widespread carbunculosis. The authors emphasize the need for proper assessment of scalp wounds, meticulous cleansing and closure of all fresh wounds, definitive drainage of newly infected wounds, and adherence to sound surgical principles in ma

[Genital Chlamydia infections].

Chlamydia trachomatis represents an important infectious agent for sexually transmitted diseases. In the USA more than 3 million new cases are recorded per year. The clinical picture and late complications are similar to those of gonorrhea; however, infections from C. trachomatis have relatively mild symptoms. This group of diseases may therefore be overlooked, and complications and late sequelae due to C. trachomatis thus occur more frequently in comparison to N. gonorrhoeae. From the viewpoint of general health in a population, chlamydial infections seem to be more important than the classic…

[Genital mycoplasma infections].

Clinical and experimental investigations on the significance of Mycoplasma hominis and Ureaplasma urealyticum have revealed different and contradictory results. Both germs are frequently discovered in young, sexually active persons. Ureaplasma urealyticum might be the cause of some cases of non-gonococcal urethritis. M. hominis seems to be one causative agent of endometritis, salpingitis, parametritis and septicaemia after birth; we do not know yet, however, how often this may be the case. M. hominis may also infect the newborn, e.g., it may cause meningitis and encephalitis. The diagnosis of

Ocular viral infections.

The ocular manifestations of viral infection vary greatly. Involvement of the anterior segment is generally mild and self-limited, except in cases of congenital infection which are often associated with significant alteration of ocular structures or in cases of childhood infection with herpes simplex virus or varicella-zoster virus, in which prolonged inflammation may lead to corneal thinning or perforation, glaucoma and cataract formation. Involvement of the posterior structures is potentially sight-threatening. Retinal or optic nerve involvement should be suspected in any patient who complai
