Needle electromyography predicts outcome after lumbar epidural steroid injection.

INTRODUCTION: Needle electromyography (NEE) would be more valuable if it could predict outcomes after lumbar epidural steroid injections (LESIs) in lumbosacral radiculopathy (LSR). METHODS: We investigated the predictive value of NEE for outcome after LESI compared with other known predictive variables in 89 subjects with clinical LSR. Seventy patients completed the study, which included diagnostic lower extremity NEE and LESI. Outcome measures included changes in pain, physical function, and psychosocial function [assessed using the Pain Disability Questionnaire (PDQ)]. RESULTS: NEE was an

Systemic effects of epidural methylprednisolone injection on glucose tolerance in diabetic patients.

BACKGROUND: Several studies have shown that in diabetic patients, the glycemic profile was disturbed after intra-articular injection of corticosteroids. Little is known about the impact of epidural injection in such patients. The goal of this study was double, at first comparing the glycaemic profile in diabetic patients after a unique injection of 80 mg of acetate methylprednisolone either intra-articular or epidural and secondly to compare the amount of systemic diffusion of the drug after both procedures. METHODS: Seventeen patients were included. Glycemic changes were compared in 9 diabet

Percutaneous microdiscectomy versus epidural injection for management of chronic spinal pain.

In this study we present the efficacy of aspiration of disc material employing the Stryker Disc Dekompressor during percutaneous microdiscectomy for the treatment of chronic spinal and radicular pain due to contained lumber disc herniation and compare the short-term outcome in such patients with those who received lumber epidural injection. A total of 50 patients with chronic lumber discogenic pain and radiculopathy were enrolled in this study and were randomized into two groups. Group 1 (n=26) underwent first time, single-level lumber discectomy at either L3-4, L4-5, or L5-S1 using the Stryke

Interlaminar epidural corticosteroid injection in the treatment of lumbosciatic pain: a retrospective analysis.

Lumbosciatica is a common condition which is associated with significant pain and disability. The aim of the present study was to examine the efficacy of interlaminar epidural corticosteroid infiltration in the treatment of lumbosciatic pain. We evaluated retrospectively sixty patients with lumbosciatic pain that a sequential interlaminar epidural administration of 40 mg methylprednisolone in 7 mL bupivacaine 0.25% was administered. Each patient was interviewed and asked about the pain according to visual analogue scale (VAS) and the level of disability according to World Health Organization p

Intravenous propofol precipitates the hypotension induced by inadvertent epidural thiopental injection.

Administration of improper drugs into epidural space is occasionally present in anesthetic practice. In most instances it would not contribute to significant neurological complications. There had not been severe hypotension reported in the literature in consequence of inadvertent epidural thiopental administration. Here we describe our experience in a case of inadvertent epidural administration of thiopental coinciding with induction of anesthesia with propofol, as a consequence of which profound hypotension was induced, necessitating aggressive inotropic and vasopressive agents to maintain bl

[Transforaminal epidural steroid injection and paraplegia: case report and bibliographic review].

OBJECTIVES: We report a patient in whom paraplegia developed following transforaminal epidural injection of a corticosteroid and discuss the physiopathology of this complication, based on a literature survey. CASE REPORT: A 40-year-old man presented with low-back pain and symptoms of L4 radiculopathy due to degenerative disc herniation resistant to conventional medical treatment. Computed tomography revealed posterolateral L4-L5 disc protrusion. A dosage of 125 mg of hydrocortisone was given by epidural transforaminal L4-L5 left injection under radioscopy guidance. Within minutes following th

Flushing as a side effect following lumbar transforaminal epidural steroid injection.

BACKGROUND: Epidural steroid injections (ESI) are commonly used in managing radicular pain. The risk of complications with epidural steroids is small, with the majority of complications being non-specific. Flushing is a known side effect of corticosteroid administration. The occurrence of flushing after epidural steroids has not been studied prospectively. OBJECTIVE: To compare flushing as a side effect of Betamethasone acetate/Betamethasone sodium phosphate (Celestone) vs. Methylprednisolone (DepoMedrol) in fluoroscopically guided epidural steroid injections. STUDY DESIGN: Non-concurrent Pr

The effects of epidural injection of local anesthetics and corticosteroids on patients with lumbosciatic pain.

Although epidural cortisone injections are commonly used for treatment of lumbosciatic pain, insufficient critical analysis of the end result can be found in the literature. The present study is a retrospective critical analysis of 367 patients with leg pain who were engaged for a minimum of two weeks or an average of two months in multifaceted conservative management without relief of pain. Injections of 10 cm3 of 0.5% bupivacaine and 100 mg of methylprednisolone were given to inpatients treated by the same anesthesiologist. The average follow-up period was 21.4 months (range, 6-36 months). R

Candida Spondylodiscitis After Transforaminal Epidural Steroid Injection: A Case Report.

Transforaminal epidural steroid injection (TFESI) is a widely used intervention for lumbar radicular pain. Although rare, fungal spinal infections can follow such procedures and require prompt diagnosis and management. A 76‑year‑old male developed progressive back pain and bilateral leg weakness two weeks after bilateral L5 TFESI. He received empirical antibiotic therapy in an outside emergency department, but showed no progress. He subsequently presented to our outpatient clinic 11 weeks after TFESI. MRI showed pyogenic spondylitis and osteomyelitis at L1-L2 with psoas involvement. No pathoge

Management of Brachioradial Pruritus With Cervical Epidural Steroid Injection: A Case Report.

Brachioradial pruritus (BRP) is a neuropathic dysesthesia described as itching over the dorsolateral forearm. The etiology of BRP has not been fully identified but is hypothesized to be multifactorial, including sun exposure and cervical spine disease. Management of BRP is challenging, and conservative measures often fail to provide notable improvement. We present a case of a 71-year-old woman with BRP refractory to topical and oral treatment, with radiographic evidence of cervical spinal canal and neuroforaminal stenosis. Two rounds of cervical epidural steroid injections (CESI) were performe

Iatrogenic pneumocephalus following a cervical epidural steroid injection: A case report.

Pneumocephalus is a condition defined by air present in the intracranial space. There are several causes of pneumocephalus, however this is a case of a patient who developed pneumocephalus following a cervical epidural steroid injection. Uniquely, this patient's chief complaint was sudden onset of syncope, and respiratory arrest following the injection. The diagnosis was made by CT, and conservative treatment was used for resolution of the condition. Patients with pneumocephalus can present with a wide range of neurologic symptoms, and prompt recognition and treatment are key to preventing irr

Devastating Vertebral Osteomyelitis After Epidural Steroid Injection: A Case Report.

CASE: A 62-year-old man with no comorbidities presented with back and bilateral leg pain and progressive paraplegia that developed over a 1-week period. He had received 2 lumbar epidural steroid injections (LESIs) for lumbar stenosis 39 and 25 days before presentation. Workup revealed osteomyelitis of L4 and L5 with epidural abscesses. He ultimately underwent all-posterior L4 and L5 corpectomy with reconstruction and L1-pelvis arthrodesis, followed by 8 weeks of intravenous antibiotics. His weakness improved, but neurological deficits persisted. CONCLUSIONS: This case illustrates a catastroph

Lumbosacral Radiculopathy: Inciting Events and Their Association with Epidural Steroid Injection Outcomes.

OBJECTIVE: Low back pain is the leading cause of worldwide disability, with lumbosacral radiculopathy accounting for over one-third of these cases. There are limited data on the relationship between etiologies and lumbosacral radiculopathy, and it is unknown whether specific causes predict treatment outcomes. DESIGN, SETTING, AND SUBJECTS: This study explores patient-reported etiologies for lumbosacral radiculopathy in a chronic pain clinic between January 2007 and December 2015 and examines whether these causes affected epidural steroid injection outcomes. METHODS: We reviewed the medical r

The use of lumbar epidural injection of platelet lysate for treatment of radicular pain.

BackgroundEpidural steroid injections (ESI) are the most common pain management procedure performed in the US, however evidence of efficacy is limited. In addition, there is early evidence that the high dose of corticosteroids used can have systemic side effects. We describe the results of a case series evaluating the use of platelet lysate (PL) epidural injections for the treatment of lumbar radicular pain as an alternative to corticosteroids.MethodsRegistry data was obtained for patients (N = 470) treated with PL epidural injections presenting with symptoms of lumbar radicular pain and MRI f
