Racial Disparities in Pediatric Inflammatory Bowel Disease Care: Differences in Outcomes and Health Service Utilization Between Black and White Children.

OBJECTIVE: To describe racial inequities in pediatric inflammatory bowel disease care and explore potential drivers. METHODS: We undertook a single-center, comparative cohort study of newly diagnosed Black and non-Hispanic White patients with inflammatory bowel disease, aged <21 years, from January 2013 through 2020. Primary outcome was corticosteroid-free remission (CSFR) at 1 year. Other longitudinal outcomes included sustained CSFR, time to anti-tumor necrosis factor therapy, and evaluation of health service utilization. RESULTS: Among 519 children (89% White, 11% Black), 73% presented wi

The addition of internists to a breast health program.

With the increases in complexity of care for breast health concerns, there is a growing need for efficient and effective clinical evaluation, especially for vulnerable populations at risk for poor outcomes. The Breast Health Center at Boston Medical Center is a multidisciplinary program, with internists providing care alongside breast surgeons, radiologists, and patient navigators. Using a triage system previously shown to have high provider and patient satisfaction, and the ability to provide timely care, patients are assigned to either a breast surgeon or internist. From 2007 to 2009, intern

Management of Obesity: Review of Adjunctive Therapies and Clinical Management Pearls.

Obesity remains one of the most prevalent and costly diseases in the United States and across the world. New therapies are available to treat obesity, and more are in the pipeline. Although lifestyle therapy by itself achieves only modest weight loss, the new generation of anti-obesity medications, endoscopic bariatric and metabolic therapies, and bariatric surgeries are highly effective for the treatment of obesity. Despite these therapies, the rate of obesity in the United States has remained stable. Multiple factors likely contribute to inadequate obesity treatment in the United States, inc

[Complications of local and regional anesthesia. An analysis of closed files of insurance companies].

We report a review of closed insurance claims for local and regional anaesthesia mishaps in the main private professional insurance companies. Twenty eight cases with extensive written documentation were discovered, spanning a 6 years interval between 1983 and 1989, involving 21 epidural, 6 spinal and 1 caudal anaesthesias, half with surgical and half obstetrical indication, and excluding all other blocks. Twenty four out of the 28 patients were relatively healthy. ASA I or II, only 4 ASA III. They fitted 4 items of our taxonomy of known complications: 15 cardiac arrests, 10 neurological damag

Technical Refinements in Gender-Affirming Top Surgery.

Chest masculinization is the most common gender-affirming operation performed. With increased access to care and improved insurance coverage, there has been a marked increase in the number of procedures performed. A video is presented with narration focused on the surgical technique of a "double-incision" mastectomy for gender-affirming chest masculinization. The Fisher grading scale used in technique selection of double-incision versus periareolar mastectomy, based on patient-specific anatomical parameters, is reviewed. Technical pearls for refinement of the double-incision top surgery techni

Health disparities among children with cleft.

Oral health-related quality of life (OHRQoL) is a multidimensional construct that measures well-being associated with the teeth, mouth, and face. This cross-sectional study examined OHRQoL, demographic data, and clinical indicators in 839 treatment-seeking youths with cleft from 6 geographically diverse cleft treatment centers. Individuals without health insurance and representing ethnic minorities had lower OHRQoL scores on the Child Oral Health Impact Profile and a higher rate of surgical recommendations. These findings imply a risk factor for reduced OHRQoL and unmet needs among vulnerable

Exploring the cost implications of different treatment modalities for uterine fibroids under the DRG system.

This paper, based on data from inpatient medical records with uterine fibroids (ICD-10: D25) from the medical record homepages of secondary and higher-level hospitals in Sichuan Province between 2016 and 2024, investigated differences in medical resource consumption and costs between high-intensity focused ultrasound (HIFU) and traditional surgical treatments under the diagnosis-related group (DRG) system. Cases were classified using the MS-DRG grouper into groups with and without complications or comorbidities (CC/MCC). An XGBoost model was employed to reclassify data for HIFU patients, addre

Failed back surgery syndrome: problems, pitfalls and prevention.

Several basic and fundamental principles remain prime considerations in lumbar spine surgery, so a review of these is appropriate in view of the current headlong rush into the use of new and developing technology for lumbar spine surgery. Eighteen hundred and sixty-one laminectomy procedures are reviewed, the majority of these for simple lumbar disc protrusion and spinal canal stenosis but also 204 cases where patients required a surgical procedure for failed back surgery syndrome. Surgical technique and technology will fail when an inappropriate patient is selected for surgical intervention;

Surgical errors and complications following cesarean delivery in the United States.

BACKGROUND: Cesarean delivery is the most common inpatient surgery performed internationally. Although cesarean delivery is typically performed to prevent adverse maternal and fetal outcomes, there is still a risk of surgical errors and complications. This study examined maternal and hospital risk factors associated with errors and complications following cesarean delivery in the United States. OBJECTIVE: To determine the prevalence of, and associated individual- and hospital-level risk factors for, surgical errors and complications following cesarean delivery in the United States. Balancing cost and care: Analyzing commercial pricing for TAVR and SAVR.

In a nationwide examination of commercial prices for transcatheter aortic valve replacement (TAVR) and surgical aortic valve replacement (SAVR), we observed substantial markups and variability across hospitals, payers, and plan types-including differences exceeding $90,000 among insurers. Higher prices did not consistently correlate with better clinical outcomes, suggesting that factors beyond quality of care drive these pricing differences. These findings underscore the need for greater transparency and further investigation into how pricing structures and market forces affect both cost and q

The Decline of Lumbar Artificial Disc Replacement.

Lower back pain associated with degenerative disc disease is a common occurrence, with many surgical treatments targeting the degenerated disc to relieve symptoms. Current surgical options for degenerative disc disease primarily consist of a discectomy followed by either spinal fusion or artificial disc replacement, with the former being increasingly more common in the lumbar region despite the risk of adjacent segment disease. Though artificial disc replacement aims to provide an increase in range of motion and decreases risk of adjacent segment disease, surgeons are increasingly opting for s

Iatrogenic vascular injuries. A review of 125 cases (excluding angiographic injuries).

From the files of an insurance company and from a number of our own files as expert medical witness to the court we have been able to collect 125 cases of malpractice suits following a iatrogenic vascular injury. An analytic study has revealed the following: --12 vascular injuries occurred after stripping of varicose veins of the lower extremities; --4 accidents occurred during arterial surgery; --35 lesions followed orthopedic surgery; and --2 general surgery. Besides these 53 injuries following surgical procedures, 72 injuries are described following non-surgical diagnostic or therapeutic pr

Transsexual legal rights in the United States and United kingdom: employment, medical treatment, and civil status.

Whereas hormonal and surgical sex change have been increasingly refined and accepted medically during the past 40 years, legal protections have only recently received attention. This overview focuses on employment, medical treatment, and civil status as male or female in the United States and the United Kingdom. Employment protection in the UK is assured since a court decision in 1994, but in the U.S. is generally uncertain and inconsistent between states. Health care, including surgery, under the UK National Health Service, is assured since a court decision in 1996. In the U.S., the absence o

Disparities in AI-Based Prior Authorization for Head and Neck Reconstruction: A Large Language Model Analysis.

INTRODUCTION: Large language models (LLM) are being rapidly integrated into healthcare, particularly to streamline time- and labor-intensive administrative processes. However, the potential for artificial intelligence (AI) systems to demonstrate bias when employed for insurance authorization remains poorly understood. As insurers increasingly adopt AI to make coverage decisions, this study examined bias in LLM-driven prior authorization in otolaryngology, using oral cavity squamous cell carcinoma (SCC) as a case study. METHODS: Using OpenAI's generative transformer GPT-4o, this study assessed
