Autoimmune Hypothyroidism Presenting as Subacute Intestinal Obstruction: A Case Report and Literature Review.

Hypothyroidism is associated with multiple gastrointestinal manifestations, ranging from mild constipation to rare life-threatening complications such as paralytic ileus and intestinal pseudo-obstruction. We report a case of a 41-year-old male patient with severe autoimmune hypothyroidism presenting with persistent abdominal distension and subacute intestinal obstruction secondary to fecaloma and colonic ileus. The diagnosis was initially obscured by concomitant diabetic ketoacidosis (DKA) and electrolyte disturbances. Despite resolution of DKA and multiple bowel decompression procedures, gast…

[Light and electron microscopic changes in the small intestine mucosa in adynamic (paralytic) ileus].

Light and electron microscopic changes of intestinal wall sections obtained at enterostomy in patients with intestinal paralysis are reported. The observed structural changes offered certain information on the relationship between the pathophysiological and the morphological changes in intestinal obstruction.

A national survey of patients with intestinal motility disorders who are potential candidates for intestinal transplantation in Japan.

Colonic pseudo-obstruction.

Colonic pseudo-obstruction is often confused with mechanical intestinal obstruction. It occurs when there is an autonomic imbalance resulting in sympathetic over-activity affecting some part of the colon. The patient is often elderly with numerous comorbidities. Once mechanical obstruction is excluded by contrast enema, the patient should be treated conservatively with nasogastric and flatus tubes for at least 48 hours, and precipitating factors should be treated. When pseudo-obstruction does not settle with waitful watching, prokinetic agents and/or colonoscopic decompression can be tried. Wh

Neonatal Hypoganglionosis Mimicking Hirschsprung Disease in a Resource-Limited Setting: A Case Report.

BACKGROUND: Neonatal pediatric intestinal pseudo-obstruction (PIPO) is rare and may closely mimic Hirschsprung disease, particularly when caused by enteric neuropathies. CASE PRESENTATION: We report the case of an 18-day-old female neonate who presented with delayed passage of meconium, persistent bilious vomiting, progressive abdominal distension, and significant weight loss since birth. Initial imaging demonstrated dilated bowel loops without evidence of mechanical obstruction. A rectal biopsy suggested distal aganglionosis, and a colostomy was performed. Despite anatomically appropriate di…

Postoperative abdominal herpes zoster complicated by intestinal obstruction: A case report.

BACKGROUND: Intestinal obstruction is a common occurrence in clinical practice. However, the occurrence of herpes zoster complicated by intestinal obstruction after abdominal surgery is exceedingly rare. In the diagnostic and treatment process, clinicians consider it crucial to identify the primary causes of its occurrence to ensure effective treatment and avoiding misdiagnosis. CASE SUMMARY: Herein, we present the case of a 40-year-old female patient with intestinal obstruction who underwent laparoscopic appendectomy and developed herpes zoster after surgery. Combining the patient's clinical

Maternal paralytic ileus as a complication of magnesium sulfate tocolysis.

Beta-adrenergic agonists tocolysis is currently the most popular treatment modality in the United States. However, magnesium sulfate is receiving increasing attention as an alternating tocolytic agent in the presence of various clinical situations, such as the treatment of insulin-dependent diabetes. While there is an abundance of information about the maternal and fetal side effects associated with beta-adrenergic tocolysis, little information is available about maternal adverse side effects of magnesium sulfate treatment for preterm labor. Side effects such as pulmonary edema, respiratory de

Ogilvie's syndrome (acute colonic pseudo-obstruction): review of the literature (October 1948 to March 1980) and report of four additional cases.

Four additional cases of Ogilvie's syndrome (acute colonic pseudo-obstruction), representing the first cases described in Italy, are reported. The medical literature concerning the subject is also thoroughly reviewed. Ogilvie's syndrome is an acute massive dilatation of the large bowel without organic obstruction of the distal colon. Three hundred and fifty-one cases have been described in the literature to date. Eighty-eight per cent of the cases were associated with various extracolonic affections (metabolic and organ dysfunctions, postoperative and posttraumatic states, etc.). Twelve per ce

[Use of low-frequency currents in the treatment of severe intestinal paralysis].

Low-frequency impulse currents (Bernar current) are suggested for treatment of paralytic intestinal obstruction. The method was used in 108 patients, in whom the cause of paresis was as follows: acute purulent peritonitis, pancreatitis, operations on the abdominal aorta and its branches. In 102 patients the stimulation proved to be effective, in 6 cases of progressive purulent peritonitis no effect was gained. In a number of patients the procedure had to be repeated during 2--3 days. Generally, diadynamic currents were employed in a complex with other measures: syphon enema, paranephral blocka

[Use of guthimine for treatment of intestinal paresis in peritonitis under experimental and clinical conditions].

A new stimulator of intestinal peristalsis-guthymine (guanylthiourea) was used experimentally and clinically for treatment of the bowel paresis in peritonitis. Preliminary, in experiments on 53 rabbits with colibacillar peritonitis an efficacy of guthymine was compared with other stimulators of the gastrointestinal tract: proserine, a hypertonic solution of sodium chloride and sorbitol. Guthymine was found to be the most effective drug. Clinically, in 23 of 33 patients with intestinal paresis a drip administration of 15-50 mg/Kg of guthymine would provide for aborting the signs of paresis. In

Intestinal obstruction.

Despite improvements in knowledge of the pathologic physiology of intestinal obstruction, the introduction of gastrointestinal decompression, and more effective antibiotics, obstruction remains a serious disease with a high mortality rate. Although the diagnosis is often obscure, it can usually be made with a fair degree of accuracy by the history alone; pain is fairly constant and characteristically is of a cramping type simulated by very few other lesions. Distention is present in low lesions but absent in high lesions; on the contrary, vomiting is minimal in low lesions but prominent in hig

[A case of paralytic ileus associated with varicella zoster virus infection].

A 79-year-old woman with a history of pyothorax was admitted with a 4-day history of abdominal distension. Physical examination revealed marked abdominal distention, absent bowel sounds, and a vesicular rash over the left Th8-10 dermatome. Abdominal radiography showed gaseous distension of the colon and ileum. Colonoscopy excluded any obstructive process of the colon. Laboratory findings yielded positive results for serum IgM and IgG against the varicella zoster virus (VZV) . Paralytic ileus associated with the VZV was therefore diagnosed. The ileus improved after conservative treatment with i
