The effect of isoflurane versus propofol-based total intravenous anesthesia on hemodynamic stability, intraoperative blood loss, surgical field quality, recovery characteristics, and complications in patients undergoing posterior lumbar fusion surgery: a randomized clinical trial.

INTRODUCTION: General anesthesia is commonly used for spinal surgeries, achieved through inhalational methods or total intravenous anesthesia (TIVA). This study compared propofol-based TIVA with isoflurane anesthesia in patients undergoing posterior lumbar fusion surgery. Primary outcomes - intraoperative blood loss, surgical field quality, and hemodynamic stability - were selected for their direct impact on patient safety and surgical outcomes. Secondary outcomes evaluated recovery characteristics (extubation time, eye-opening, response to commands, and post-anesthesia recovery quality) and p

Magnitude and associated factors of intraoperative cardiac complications among geriatric patients who undergo non-cardiac surgery at public hospitals in the southern region of Ethiopia: a multi-center cross-sectional study in 2022/2023.

BACKGROUND: Intraoperative cardiac complications are a common cause of morbidity and mortality in non-cardiac surgery. The risk of these complications increased with the average age increasing from 65. In a resource-limited setting, including our study area, the magnitude and associated factors of intraoperative cardiac complications have not been adequately investigated. The aim of this study was to assess the magnitude and associated factors of intraoperative cardiac complications among geriatric patients undergoing non-cardiac surgery. METHODS: An institutional-based multi-center cross-sec

Effects of intraoperative goal-directed fluid therapy and restrictive fluid therapy combined with enhanced recovery after surgery protocol on complications after thoracoscopic lobectomy in high-risk patients: study protocol for a prospective randomized controlled trial.

BACKGROUND: Acute kidney injury (AKI) is a common complication after thoracoscopic lobectomy in high-risk patients due to insufficient intraoperative infusion. Goal-directed fluid therapy (GDFT) is an individualized fluid infusion strategy; the fluid infusion strategy is adjusted according to the patient's fluid response. GDFT during operation can reduce the incidence of AKI after major surgery. Enhanced recovery after surgery (ERAS) protocol optimizes perioperative interventions to decrease the postoperative complications after surgery. In ERAS protocol of lobectomy, intraoperative restrictiv

Impact of intraoperatiVe moderAte positive end-expiratory pressure with reCruitment mAnoeuvres versus low positive end-expiRatory pressure on major postoperative pulMonary complications and death after on-pump cardiac surgery in high-risk patients: the VACARM randomised clinical trial-study protocol.

INTRODUCTION: Postoperative pulmonary complications (PPCs) are common after cardiac surgery and are associated with significant morbidity and mortality. Lung-protective ventilation strategies have been proposed to reduce PPCs, but the optimal level of positive end-expiratory pressure (PEEP) and the use of alveolar recruitment manoeuvres (RMs) remain controversial. METHODS/ANALYSIS: In this investigator-initiated, multicentre, open, randomised, parallel-group, superiority clinical trial, elective cardiac surgery patients at risk of PPCs will be assigned to one of two intraoperative ventilation

Predicting Intraoperative Hypotension: An Intraoperative Case Report.

Intraoperative hypotension (IOH) is a common issue associated with acute kidney injury, myocardial injury, stroke, and death. IOH may be avoided with the incorporation of newer advanced hemodynamic monitoring technologies. This case study examines the use of advanced hemodynamic monitoring with an early warning system for the intraoperative hemodynamic management of a patient presenting for pancreaticoduodenectomy. Incorporating the hypotension prediction index and other hemodynamic parameters to anticipate impending hypotension and treat potential causative factors is an emerging technologica

State of the art of neuromonitoring in thyroid surgery.

Thyroidectomy is a common procedure in endocrine surgery, frequently performed to treat benign and malignant thyroid conditions. Recurrent laryngeal nerve (RLN) injury, a major complication, underscores the necessity for meticulous nerve dissection during surgery. Intraoperative neuromonitoring (IONM) has emerged as a valuable adjunct to visual identification in RLN preservation. This review synthesizes current literature on IONM in thyroid surgery, emphasizing its role in enhancing RLN integrity assessment and reducing surgical complications such as vocal cord paralysis. IONM techniques inclu

Ethanol labeling: detection of early fluid absorption in endometrial resection.

A study is presented of ethanol labeling of irrigation fluid in endometrial resection. The introduction of ethanol labeling and intraoperative breath ethanol analysis provided an inexpensive and potentially useful means of detecting early fluid absorption during uterine surgery. The breath ethanol analyzer used was a hand-held meter; the irrigant solution was 5% dextrose with 1% ethanol. Simultaneous breath and venous samples were taken from women undergoing endometrial resection. An increase in breath ethanol was positively correlated with fluid absorption, blood ethanol, and serum glucose. T

Higher versus Routine Intraoperative Blood Pressure Targets in Noncardiac Surgery: A Systematic Review and Meta-analysis with Trial Sequential Analysis of Randomized Trials.

BACKGROUND: Observational studies consistently link intraoperative hypotension to adverse postoperative outcomes, leading guidelines to recommend maintaining mean arterial pressure 60 to 65 mmHg or greater during noncardiac surgery. Whether targeting higher intraoperative blood pressure values improves clinical outcomes remains uncertain. METHODS: The authors conducted a Preferred Reporting Items for Systematic Reviews and Meta-analyses-guided search on PubMed, Cochrane Central Register of Controlled Trials, Scopus, and Embase from inception to April 2026. Randomized trials comparing higher i

[Peroperative and early postoperative hypoxemia].

Arterial hypoxaemia is observed during operation and also in the recovery room. Hypoxaemia during operation may be due to altered pulmonary mechanical conditions, drug-induced physiological changes and technical errors. Early postoperative hypoxaemia is frequently observed after general anaesthesia and may, among other things, be due to diffusion hypoxaemia, hyperventilation-induced hypoventilation, residual curarization, hypoventilation induced by sedatives or analgesics and ventilation/perfusion alterations. The clinical significance of peroperative and early postoperative hypoxaemia is not

Assessing the value of preoperative medical clearance in patients with primary rhegmatogenous retinal detachment.

PURPOSE: To determine rates of intraoperative and postoperative systemic and ocular adverse events and establish the value of preoperative medical assessment in patients undergoing surgery for primary rhegmatogenous retinal detachment repair at a single academic center. PATIENTS AND METHODS: Retrospective cohort study of 185 patients undergoing surgery for repair of primary rhegmatogenous retinal detachment (RRD) at a single academic center. Medical records were reviewed for medical comorbidities, completion of preoperative medical examination, anesthesia used during surgery, intraoperative a

Predictors and consequences of intraoperative microvascular problems in autologous breast reconstruction.

INTRODUCTION: Microvascular complications can have devastating consequences in breast reconstruction. Because of the relatively low rate of complications, it can be difficult to identify significant predictors of failure. This study explores the importance of intraoperative microvascular problems in the development of postoperative complications and seeks to identify risk factors that may predict such events. METHODS: Three hundred and eighty-four patients undergoing 562 microvascular breast reconstructions were eligible for inclusion in this study. Bivariate and multivariate logistic regress
