Three neonates with spontaneous perforation of the common bile duct have been described. All were treated by laparotomy in the seventh week, with attempted closure of the perforation and a simple biliary diversion by cholecyst-jejunostomy with jejunojejunostomy below it. They have done well and represent 100% salvage of our cases to date. One is now 12 years of age. As far as we are aware, this method of dealing with the condition has not been described before.

We report our experience with endoscopic management of 3 men (aged 62, 63 and 65 years) with duodenal diaphragm disease following NSAID use for 5-15 years. In the first patient a 24 F through-the-scope balloon dilatation was attempted but failed; he subsequently underwent gastro-jejunostomy. The other two patients subsequently underwent radial incisions of the web with mixed cutting and coagulation current using a standard 5 F sphincterotome.

Protein-calorie malnutrition is prominent among patients undergoing gastrointestinal tract operations who are admitted as an emergency measure to the intensive care unit. Immediate feedings postoperatively with an elemental diet through a jejunostomy catheter can provide an alternative method of nutritional support in these critically ill patients. In patients with extremely high metabolic demands, it may be necessary to combine enteral feeding with parenteral support to achieve positive nitrogen balance.

A new technique is described for dealing with late-presenting spontaneous esophageal ruptures. This method requires only a short period of general anesthesia to drain the periesophageal abscess by a drainage tube inserted into the abscess cavity from the esophagus with the aid of a gastroscope and fluoroscopy. Gastric fluids are diverted from the esophageal rupture with a gastrostomy, and a jejunostomy is used for enteral feeding. The esophagus is retained, and closure of the fistula with resumption of normal swallowing is documented with serial sinograms.
