MR findings of exaggerated fluid in facet joints predicts instability.

STUDY DESIGN: Retrospective radiographic review. OBJECTIVE: To determine the incidence of exaggerated fluid signal in lumbar facet joints on magnetic resonance imaging (MRI) and evaluate the correlation of this finding with radiographic evidence of instability. SUMMARY OF BACKGROUND DATA: Lumbar spine MRI is routinely used to evaluate low back pain. The presence of exaggerated fluid in the facets joints on MRI has been correlated with dynamic instability. To our knowledge, this correlation has never been evaluated in an objective fashion. METHODS: One hundred thirty-four consecutive lumbar …

Cystic pseudotumours in both upper ankle joints in gouty arthritis.

This case demonstrates a yet unreported clinical entity of bilateral tophaceous cystic lesions of the tibiotalar joints of an 37-year-old white male who presents with moderate painful swelling in both upper ankle joints for 2 years. Radiodiagnostic studies show cystic lesions in both upper ankle joints and the right talus considering neoplastic processes in the differential diagnosis. The incisional biopsy reveals chalk-like material of the intraosseous lesions which was pathognomonic for tophaceous gout. Antihyperuricemic medication led to a stagnation of growth of the lesions without a sign …

[Magnetic resonance imaging of the peripheral joints in psoriatic arthritis].

OBJECTIVE: Magnetic resonance imaging (MRI) has been widely used for the evaluation of rheumatoid arthritis (RA), with only a minority of studies considering other types of arthritis. This review is concerned with an evaluation of the MRI appearance of peripheral joints in psoriatic arthritis (PsA). METHODS: A Medline search was performed to identify all publications from the years 1985 to 2006 concerning MRI of the peripheral joints and PsA. Additional papers were retrieved by scanning the references to the Medline-listed articles. Articles written in English, French, German, and Italian wer…

Repair and regeneration of osteochondral defects in the articular joints.

People suffering from pain due to osteoarthritic or rheumatoidal changes in the joints are still waiting for a better treatment. Although some studies have achieved success in repairing small cartilage defects, there is no widely accepted method for complete repair of osteochondral defects. Also joint replacements have not yet succeeded in replacing of natural cartilage without complications. Therefore, there is room for a new medical approach, which outperforms currently used methods. The aim of this study is to show potential of using a tissue engineering approach for regeneration of osteoch…

[Arthrodesis of interphalangeal joints by means of external frame fixation].

PURPOSE OF THE STUDY: The indications for arthrodesis of interphalangeal joints include pain, instability, deformity or irreparable damage to the relevant flexor or extensor tendon. The general principle of arthrodesis is to resect the affected joint ends in the flexion desired so that a highest possible surface of contact may be provided. Subsequently, retention by means of internal or external fixation is performed. The aim of this study is to evaluate our method of interphalangeal joint arthrodesis, using an external frame fixator, to present the results and to point out the advantages of t…

Posterolateral dislocation of the knee joints: analysis of 9 cases.

OBJECTIVE: To analyze the traumatic pathological characteristics of posterolateral dislocation of knee joints and its treatment. METHODS: Nine cases of posterolateral dislocation of knee joint, 5 cases of fresh injuries (the fresh injury group) and 4 cases of old injuries ( the old injury group) were reviewed and analyzed. In the fresh injury group 4 cases failed in close reduction due to "buttonholing" through the medial joint, among them 3 cases underwent repair of the damaged ligaments. In the old injury group 2 cases underwent ACL and MCL repair only in acute stage, but re-dislocated. Of …

[Ultrasound diagnosis of hand and finger joints. Changes in arthritis].

Arthritis of the hand and finger joints is seen with unspecific and specific signs, as known from the hip and knee joints. The examination has to be done with special applicators (small sized with 7-13 MHz). Especially signs such as erosions and surrounding synovitis ensure the diagnosis of rheumatoid arthritis.

Orientation and tropism of lumbar facet joints in degenerative spondylolisthesis.

The orientation and tropism of the lumbar facet joints at L4-5 level was assessed by magnetic resonance imaging (MRI) in 53 patients with degenerative L4-5 spondylolisthesis and 53 age- and sex-matched normal control subjects. The degree of disc degeneration at the L4-5 level and of vertebral slip on lateral radiographs was also evaluated. Patients with degenerative spondylolisthesis had more sagittally orientated facet joints (P < 0.01) and more significant facet joint tropism (P < 0.05) than normal control subjects. For patients with degenerative spondylolisthesis, the facet joint tropism wa…

Clinical and radiological features of atlantoaxial joints in rheumatoid arthritis.

Atlantoaxial (AA) instability is frequent radiological finding in patients with rheumatoid arthritis (RA). Mostly no serious neurological disorders are expected in such patients. The purpose of the study was to assess the sagittal spinal canal diameter according to Steel's rule of third and its relationship to clinical symptoms. Radiological and clinical evaluation was performed in 65 in-patients with RA. Fifty four patients complained of neck pain, 39 had vertebrobasilar symptoms, and 25 mild neurological disorders. Hyperreflexy tendon responses were registered in 16 patients. Only 1 patient

Volar dislocation of multiple carpometacarpal joints: report of four cases.

Four acute cases of volar carpometacarpal dislocations of the medial four metacarpals were treated in our hospital's emergency room. Our initial diagnosis after clinical examination was confirmed by routine x-rays of the hand. Closed reduction was followed by percutaneous fixation of the carpometacarpal joints with Kirschner wires. Satisfactory function of the hand was obtained in all cases.

[Trigeminal motor paralysis and dislocation of the temporo-mandibular joints].

A 64-year-old woman with diabetes mellitus was admitted to our hospital with left hemiparesis of sudden onset. A brain MRI demonstrated a cerebral infarction in the ventral part of the right lower pons. When left hemiparesis worsened, she had dislocation of the temporo-mandibular joints repeatedly. Then, her lower jaw deviated to the right when she opened her mouth. Also, there was decreased contraction of the right masseter when she clenched her teeth. These findings suggest that there was trigeminal motor paralysis on the right side resulting from involvement of the intrapontine trigeminal m…

Results of arthrodesis of the tarsometatarsal joints after traumatic injury.

We retrospectively reviewed the records of thirty-two patients who had had an arthrodesis of the tarsometatarsal joints for intractable pain after a traumatic injury of the midfoot. The arthrodesis was performed at a mean of thirty-five months (range, six to 108 months) after the injury. All of the procedures were performed with use of rigid internal fixation, and twenty-four patients, in whom a defect had been created by debridement of the joints, were managed with an autogenous bone graft. Nine patients had at least one concomitant procedure, including a claw-toe procedure (eight patients), …

Presence of neuroreceptors in normal and diseased temporo-mandibular joints.

An immunohistochemical study was performed on the articular disk and periarticular tissues of the temporo-mandibular joint (TMJ) in order to ascertain the presence of neuroreceptors (NRec) in these sites. The study was carried out with the APAAP technique on tissue samples obtained from 10 subjects without known TMJ disease and from 5 patients with severe TMJ arthritis or arthrosis. The antibodies used were directed against the following antigens: Gliofibrillary Acidic Protein (GFAP), Leu-7, Myelin Basic Protein (MBP), Neurofilaments 68 kD (NF), Neuron Specific Enolase (NSE), S-100 protein (S-

Mycoplasma genitalium in the joints of two patients with arthritis.

Mycoplasma genitalium was sought in synovial fluids from 13 patients, of whom five had Reiter's syndrome, four had rheumatoid arthritis, and one each had systemic lupus erythematosus, psoriatic arthritis, rheumatic fever and undefined arthritis. The mycoplasma was detected by a PCR assay in the knee joint of a 25-year-old man with Reiter's syndrome, from whom urethral ureaplasmas were isolated and whose synovial fluid mononuclear cells responded to ureaplasmal antigens in a proliferation assay. Mycoplasma genitalium was also detected in the knee joint during an exacerbation of arthritis in a 5

Contrast radiography of equine joints, tendon sheaths, and draining tracts.

Arthrography, tenography, and fistulography (or sinography) can provide additional information about the soft-tissue components of joints, tendon sheaths, and draining tracts, respectively. Arthrography is a valuable aid in the diagnosis of synovial masses, such as villonodular synovitis, and osteochondrosis. Tenography is particularly useful in evaluating chronic tendon sheath distention. Fistulography is an effective means of identifying acute traumatic damage to synovial structures and determining the cause and extent of draining tracts.

Clinical and radiological findings in temporomandibular joints with disc perforation.

Sixteen temporomandibular joints from 15 patients, reported to have disc perforation at arthrography, were studied. Pain from the TMJ region was reported by 11 patients. Clicking was found in 6 joints and crepitation in 6 joints. Nine patients displayed a deviation of the mandible at maximum mouth opening towards the affected side. The muscles of the affected side were tender on palpation more frequently than those on the contralateral side. The disc perforation was located in the posterior attachment in most joints. An anterior disc displacement was found in all joints except one, which exhib…

[Results of magnetic resonance tomography (MRT) in 100 temporomandibular joints].

The results obtained by MRT of 100 temporomandibular joints in 12 probands with healthy dentitions and 38 patients are reported. The findings were dominated by dislocations of the articular disk. Diagnosis of this condition presented no difficulties, as the interarticular disk can be well differentiated from the adjoining bone structures and the bilaminar zone. The high quality of the soft tissue images renders MRT a valuable procedure completing other examination methods such as panoramic or Schüller radiographs for diagnosis of TMJ diseases.
