BACKGROUND: Corneal densitometry, based on the Scheimpflug principle, is a useful noninvasive tool for quantitative evaluation of the optical quality of the cornea. It enables us to evaluate corneal transparency in the different corneal layers and different annuli. In this article, we discuss the basis of application of corneal densitometry measurements and summarize findings in patients with keratoconus.
METHODS: This article is based on a selective literature review and analysis of own data.
RESULTS: Patients with keratoconus have significant higher corneal densitometry values compared to …

PURPOSE: To investigate the effect of the severity of keratoconus on the corneal endothelium using specular microscopy.
METHODS: Seven hundred and twelve eyes from the Homburg Keratoconus Center (HKC) database were included in this retrospective study. Corneal endothelium was evaluated using the Tomey EM-3000 specular microscope. Keratoconus-related topographic and tomographic data were obtained from Scheimpflug-based tomography (Oculus Pentacam® HR). Eyes were classified into stages 0 (healthy) to 4 (severe keratoconus) according to the Topographic Keratoconus Classification (TKC). Subgroups…

BACKGROUND: Keratoconus is a chronic, bilateral, usuallly asymmetrical, non-inflammatory, ectatic disorder, being characterized by progressive steepening, thinning and apical scarring of the cornea. Initially, the patient is asymptomatic, but the visual acuity gradually decreases, resulting in significant vision loss due to the development of irregular astigmatism, myopia, corneal thinning and scarring. The classic treatment of visual rehabilitation in keratoconus is based on spectacles and contact lenses (CLs).
OBJECTIVE: To summarize the types of CLs used in the treatment of keratoconus. Th…

Corneal scarring is the result of a disease, infection or injury. The resulting scars cause significant loss of vision or even blindness. To-date, the most successful treatment is corneal transplantation, but it does not come without side effects. One of the corneal dystrophies that are correlated with corneal scarring is keratoconus (KC). The onset of the disease is still unknown; however, altered cellular metabolism has been linked to promoting the fibrotic phenotype and therefore scarring. We have previously shown that human keratoconus cells (HKCs) have altered metabolic activity when comp…

Over the last decade, corneal collagen cross-linking (CXL) has become a conventional treatment method for progressive keratoconus. Laboratory studies have shown that CXL increases the diameter of collagen fibers and also the number of intra- and interfibrillar cross-links, thus, increasing biomechanical strength of the irradiated cornea. As confirmed by a series of clinical and randomized controlled trials, CXL is able to slow down and, perhaps, to stop the progression of keratoconus. In most post-CXL patients visual acuity improves, while keratometric readings, spherical equivalent, and highe…

Keratoconus as the most common cause of ectasia is one of the leading cause of corneal transplants worldwide. The current available therapies do not modify the underlying pathogenesis of the disease, and none of the available approaches but corneal transplant hinder the ongoing ectasia. Several studies document Crosslink defect between collagen fibrils in the pathogenesis of keratoconus. Collagen cross link is a relatively new approach that with the application of the riboflavin and ultraviolet A, new covalent bands reform. Subjective and objective results following this method seem to be prom…

PURPOSE: We investigated whether a group of patients with keratoconus (KTCN) harbor mutations in the mitochondrial genome.
METHODS: We sequenced the full mitochondrial genome in a group of Saudi patients with KTCN (n = 26) and 100 ethnically matched controls who had no KTCN by examination.
RESULTS: A total of 10 KTCN patients (38.5%) had potentially pathogenic nonsynonymous mtDNA mutations. Of the nonsynonymous sequence changes detected, 4 (40%) were in Complex I, one was in the tRNA(Glutamine), one was in tRNA(Tryptophan), one was in tRNA(Asparagine), one was in tRNA(Histidine), and two wer…

Keratoconus (KC) is a noninflammatory conical protrusion of the central part of the cornea of still unknown etiology and poorly explored pathogenesis. By now a great number of theories and hypotheses on both etiology and pathogenesis of KC have been suggested: genetic, metabolic, immunological, endocrinological, ecological, etc. There are plenty of reports and research results supporting either of the theories and evidencing the link of KC to other diseases. Nevertheless, the question of the underlying reason for keratoconus development remains unresolved.

The review discusses literature data on the treatment of keratoconus with intrastromal Myoring implantation into a corneal pocket, formed with a Pocket Maker microkeratome and a femtosecond laser alone and in combination with cross-linking of corneal collagen. The method shows high effectiveness as to refractive and functional outcomes and stabilization of keratoconus over the whole follow-up period (more than 3 years).

Keratoconus is a progressive, ectatic disease of the cornea leading to thinning and highly irregular astigmatism. Until recently all treatment options, such as prescription of glasses or contact lenses were symptomatic and neither keratoplasty nor the implantation of intracorneal rings can heal the disease. Riboflavin ultraviolet A (UVA) collagen cross-linking (CXL) cannot heal keratoconus either but promises to halt the progression. The therapeutic principle is a photochemical reaction of riboflavin and UVA light leading to free oxygen radicals in the corneal stroma that induce covalent linki…

Up-to-date models of intrastromal corneal segments are reviewed. Different techniques of up-to-date models of intrastromal corneal segments implantation in keratoconus are described. Mechanical and laser techniques of forming of corneal tunnels are analyzed. Information about potential complications of intrastromal corneal segment implantation is given.

Comparative analysis of intrastromal corneal segments Keraring implantation using either femtosecond laser or manual technique to create corneal tunnel in 47 patients (47 eyes) with keratoconus stage II-III is presented. Refractional and functional results in groups with femtosecond laser and manual technique did not show significant difference. The main advantage of femtosecond laser revealed in the study is technical simplification of intrastromal tunnels creation.

PURPOSE: To report 2 cases of keratopathy after uneventful cross-linking procedures for grade 3 keratoconus.
METHODS: Two patients (21 and 11 years old) with grade 3 keratoconus underwent uneventful cross-linking in 1 eye. In both patients, multiple peripheral stromal precipitates, which extended centripetally, were observed 48 hours after the procedure. Sample cultures were negative for bacteria, fungi, and parasites.
RESULTS: Cornea infiltrates slowly resolved after combined topical antibiotic/antifungal/povidone/steroids treatment. Final best spectacle-corrected visual acuity was 20/25 fo…

OBJECTIVE: To report the concomitant presentation of keratoconus and corneal dystrophies at Wills Eye Hospital for the 10-year period from January 1, 1997, to December 31, 2006.
METHODS: Patients with concomitant keratoconus and corneal dystrophies were identified using a computer database. Complete ophthalmologic examination, keratometry, pachymetry, and computerized videokeratography were performed in all patients. When present, cornea guttata were confirmed by clinical examination and specular microscopy. Histopathologic examination with special stains of excised corneal buttons was perfor…
