Why kidneys fail in autosomal dominant polycystic kidney disease.

The weight of evidence gathered from studies in humans with hereditary polycystic kidney disease (PKD)1 and PKD2 disorders, as well as from experimental animal models, indicates that cysts are primarily responsible for the decline in glomerular filtration rate that occurs fairly late in the course of the disease. The processes underlying this decline include anatomic disruption of glomerular filtration and urinary concentration mechanisms on a massive scale, coupled with compression and obstruction by cysts of adjacent nephrons in the cortex, medulla and papilla. Cysts prevent the drainage of …

The renin-angiotensin-aldosterone system and the kidney: effects on kidney disease.

The renin-angiotensin-aldosterone system regulates renal vasomotor activity, maintains optimal salt and water homeostasis, and controls tissue growth in the kidney. However, pathologic consequences can result from overactivity of this cascade, involving it in the pathophysiology of kidney disease. An activated renin-angiotensin-aldosterone system promotes both systemic and glomerular capillary hypertension, which can induce hemodynamic injury to the vascular endothelium and glomerulus. In addition, direct profibrotic and proinflammatory actions of angiotensin II and aldosterone may also promot…

[Malignant tumors of the normotopic kidneys and ureters following kidney transplantation].

There is an 4.2-23% incidence of cancer in renal transplant recipients. A closely meshed radiological follow-up is important as shown in 3 patients who developed a carcinoma of the kidney or ureter within 1-5 years after renal transplantation. This includes routine sonography of the whole abdomen, in case of pathological findings CT respectively MRI, i.v. urography, retrograde urography and angiography if needed.

[Treatment of kidney lithiasis with extracorporeal shockwave lithotripsy in horseshoe kidney].

Following a brief overview of the general features of the horseshoe kidney, we report on ten patients diagnosed as having this anomaly and renal calculi. One patient had bilateral renal calculi. The clinical cases are described and treatment with extracorporeal shock wave lithotripsy (ESWL1) and its advantages and disadvantages relative to other treatment modalities (PCN, open surgery) are discussed. Although we do not advocate the use of a single therapeutic approach, our results show the usefulness of ESWL1 in the treatment of this frequently recurring condition.

Chronic kidney disease, severe arterial and arteriolar sclerosis and kidney neoplasia: on the spectrum of kidney involvement in MELAS syndrome.

BACKGROUND: MELAS syndrome (MIM ID#540000), an acronym for Mitochondrial Encephalopathy, Lactic Acidosis and Stroke-like episodes, is a genetically heterogeneous mitochondrial disorder with protean manifestations and occasional kidney involvement. Interest in the latter is rising due to the identification of cases with predominant kidney involvement and to the hypothesis of a link between mitochondrial DNA and kidney neoplasia. CASE PRESENTATION: We report the case of a 41-year-old male with full blown MELAS syndrome, with lactic acidosis and neurological impairment, affected by the "classic"…

Targeted degradation of kidney injury molecule 1 ameliorates acute kidney injury.

Acute kidney injury (AKI) is a global public health issue with a high incidence and mortality rate, without effective treatments currently available. Kidney injury molecule 1 (KIM1), an FDA-approved biomarker for AKI, is dramatically induced on the membrane of injured renal tubular epithelial cells. However, whether it works as a therapeutic target remains unclear. Herein, by using a modified Proteolysis Targeting Chimera (PROTAC) strategy, we constructed KIM1-targeted degradable liposome (KIM1-TDL) by conjugating a KIM1-targeting peptide TKP4 that we previously designed, and an E3 ligase liga…

Outcomes of kidney transplants from pediatric donors with acute kidney injury.

Pediatric deceased donor kidneys with acute kidney injury (ped-AKI) are at increased risk for nonutilization. To evaluate the posttransplant outcomes of ped-AKI recipients, we conducted a retrospective cohort study, comparing 17,731 adult recipients of kidneys from pediatric donors without AKI (ped-non-AKI, terminal serum creatinine <1 mg/dL) to 1589 ped-AKI recipients (serum creatinine ≥2 mg/dL). We used weighted logistic regression to estimate the association between ped-AKI and delayed graft function (DGF), and weighted Cox regression to estimate the association between ped-AKI and primary …

Endothelial biomarkers predict kidney and cardiovascular outcomes after acute kidney injury.

BACKGROUND: Acute kidney injury (AKI) is common in hospitalized patients, and post-injury biological processes activated after the initial insult may determine recovery or sustained damage. Identifying these processes may offer opportunities for early intervention, and biomarkers can serve as valuable tools in this context. We hypothesized that endothelial biomarkers-reflecting endothelial dysfunction or damage-predict adverse clinical outcomes following AKI. METHODS: In a single-center cohort of 93 consecutive patients with in-hospital mild AKI, a panel of endothelial (sFlt-1, CD138, and sVC…

Nephrectomy in Kidney Transplant Candidates With Autosomal Dominant Polycystic Kidney Disease.

BACKGROUND: Native nephrectomy in patients with autosomal dominant polycystic kidney disease (ADPKD) undergoing kidney transplantation remains controversial, particularly regarding indications, timing, and laterality. This study evaluated surgical strategies and outcomes of native nephrectomy in ADPKD kidney transplant candidates. METHODS: We retrospectively analyzed adult patients with ADPKD who underwent kidney transplantation at a single center between 1999 and 2021. Demographic, transplant-related, nephrectomy-related, surgical, complication, and histopathological data were compared betwe…

Kidney allograft outcomes in combined kidney with other solid organ transplantation.

With advances in solid organ transplantation, the option of combined kidney with other solid organ transplantation is an enticing option for patients with advanced kidney disease and concomitant other solid organ failure. Kidney allograft dysfunction is well known to be associated with increased adverse outcomes post solitary kidney transplant however, outcomes for patients and the kidney allograft are somewhat understudied in the setting of kidney transplantation when combined with other solid organ transplantation such as in a simultaneous liver-kidney transplant. We will provide an overview…

NAD+ metabolism in acute kidney injury and chronic kidney disease transition.

Disturbances in kidney tubular cell metabolism are increasingly recognized as a feature of acute kidney injury (AKI). In AKI, tubular epithelial cells undergo abnormal metabolic shifts that notably disrupt NAD+ metabolism. Recent advancements have highlighted the critical role of NAD+ metabolism in AKI, revealing that acute disruptions may lead to lasting cellular changes, thereby promoting the transition to chronic kidney disease (CKD). This review explores the molecular mechanisms underlying metabolic dysfunction in AKI, with a focus on NAD+ metabolism, and proposes several cellular processe…

Kidney organoid models of polycystic kidney disease: challenges and future directions.

Kidney organoids are an increasingly established model of polycystic kidney disease (PKD). Derived from human pluripotent stem cells (hPSCs), organoids may be generated from induced pluripotent stem cells (iPSCs) of patients that bear naturally occurring mutations or from CRISPR mutant hPSCs by virtue of their genetic tractability. PKD is the leading inheritable cause of kidney failure (KF), accounting for ∼5%-10% of the kidney transplant and dialysis needs worldwide. PKD is a disorder of considerable genetic heterogeneity, composed of typical adult-onset autosomal dominant (ADPKD) and fetal-o…

Chronic Kidney Disease-Mineral Bone Disease Biomarkers in Kidney Transplant Patients.

BACKGROUND: Kidney transplant patients frequently suffer from Chronic Kidney Disease associated with Mineral Bone Disease (CKD-MBD), a complex condition that affects mainly kidney transplant patients. Post-transplantation bone disease is complex, especially in patients with pre-existing metabolic bone disorders that are further affected by immunosuppressive medications and changes in renal allograft function. Main biochemical abnormalities of mineral metabolism in kidney transplantation (KTx) include hypophosphatemia, hyperparathyroidism (HPTH), insufficiency or deficiency of vitamin D, and hy…

Gut-Kidney Axis Investigations in Animal Models of Chronic Kidney Disease.

Chronic kidney disease (CKD) is an incurable disease in which renal function gradually declines, resulting in no noticeable symptoms during the early stages and a life-threatening disorder in the latest stage. The changes that accompany renal failure are likely to influence the gut microbiota, or the ecosystem of micro-organisms resident in the intestine. Altered gut microbiota can display metabolic changes and become harmful to the host. To study the gut-kidney axis in vivo, animal models should ideally reproduce the disorders affecting both the host and the gut microbiota. Murine models of C…
