In the first 1000 arthroscopic operations performed by one surgeon 136 patients had two or more procedures, making a total of 1168 during the 1000 operations. The indications for operation were internal mechanical derangements in 565 patients, anterior knee pain in 246, disorders of the synovium in 77, ligament injuries in 63, and degenerative joint disease in 49. UNLABELLED: Tibial dislocation in the knee joint with an incidence of 0.2--3% is a rare event but inevitably bears severe consequences. From 1970--1978 we observed 7 cases. Most frequently there was a complex instability of the joint, accompanied by blood vessel and nerve lesions in 5 cases.
THERAPY: Early suture or reinsertion of ligament rupture. Strict priority of blood vessel injuries. Conservative treatment results in poor functional results with necessity of secondary reconstructive procedures. Interdisciplinary cooperation is of crucial value in these injuries.

Spontaneous osteonecrosis was diagnosed in ninety knees in which a roentgenographically visible lesion had developed within six months after onset of symptoms. Scintimetry can be used to differentiate between osteonecrosis and other painful conditions of the knee. The prognosis of osteonecrosis is dependent on the size of the radiolucent lesion. Three congenital synostoses of the knee occurred in 2 patients. Complete bony epiphyseal fusion was present on the radiographs. This condition is a failure of joint formation in the early mesenchymal stage at 4--5 weeks gestation and appears as connected cartilaginous cell masses which later ossify. Involvement of other joints has been reported in the literature, but these may be the first documented cases of the condition in knee joints.

For more than 10 years the authors have had a rather differentiated attitude to osteochondritis dissecans. The indication depends on the age, topography of the lesion, its volume and stage of evolution. The results of three techniques are discussed: simple ablation of the sequestrum, re-implantation of the sequestrum after treatment of the niche and cortico-spongiosa grafting.

This paper describes transcutaneous therapeutic procedures performed during arthroscopy. Sixty-six patients were treated. Small loose bodies were flushed out through the arthroscope in 7 cases. In 19 of 24 knees a loose body 0.5-2 cm in size was extracted either with a Dormia stone-dislodger or a pituitary rongeur. In 5 patients we failed to remove the loose body transcutaneously and arthrotomy was performed. The clinical usefulness of arthroscopies of the knee joint has been evaluated on the basis of 282 examinations. The procedures were performed under general anesthesia with sterility precautions as for arthrotomy. In nearly all cases, direct observation of articular surfaces, synovial membranes and the menisci of the ventral two thirds of the joint was possible. The dorsal recessus, the posterior third of medial meniscus, the posterior cruciate and Baker's cyst could not be inspected. Four patients are described with documented coccicioidal synovitis of the knee joint. In two of them, no previous diagnosis of coccidioidomycosis had been made prior to diagnosis of coccidioidal synovitis. The other two had active disease after courses of amphotericin B administered both parenterally and intra-articularly for disseminated coccidioidomycosis and coccidioidal synovitis.
