[Effect of transcutaneous electrical acupoint stimulation on labor analgesia].

OBJECTIVE: To observe the effect of transcutaneous electrical acupoint stimulation (TEAS) on labor pain. METHODS: A total of 151 primiparas who were vaginal delivery were randomized into a TEAS group (76 cases) and a peridural group (75 cases). In the peridural group, peridural blockage was applied to analgesia. In the TEAS group, TEAS was applied at Hegu (LI 4),Neiguan (PC 6), Jiaji T10~L1 (EX-B 2) and Ciliao (BL 32), disperse-dense wave (2 Hz/100 Hz), 15-50 mA in current intensity. The analgesic time was from 3 cm to completely opening of cervix. The visual analogue scale (VAS) scores were

The effects of epidural analgesia on labor, maternal, and neonatal outcomes: a systematic review.

Mothers given an epidural rather than parenteral opioid labor analgesia report less pain and are more satisfied with their pain relief. Analgesic method does not affect fetal oxygenation, neonatal pH, or 5-minute Apgar scores; however, neonates whose mothers received parenteral opioids require naloxone and have low 1-minute Apgar scores more frequently than do neonates whose mothers received epidural analgesia. Epidural labor analgesia does not affect the incidence of cesarean delivery, instrumented vaginal delivery for dystocia, or new-onset long-term back pain. Epidural analgesia is associat

Labor characteristics and intrapartum interventions in women with vaginal birth after cesarean section.

BACKGROUND: With the development of China's two-child-policy, vaginal birth after cesarean section (VBAC) has aroused public concern. It is important to understand the labour characteristics and intrapartum management of women attempting VBAC to enhance the rates of successful VBAC. The purpose of our research was to investigate the differences in the characteristics of labor, intervention measures and perinatal outcomes between women who had a VBAC and primiparas or multiparas not undergoing VBAC, providing clinical references of intrapartum management for women who are planning a VBAC. The single pregnancy predicting model of 1 minute Apgar score less than 7 after preterm birth: A retrospective study.

Preterm delivery is greatly associated with perinatal mortality and morbidity, while there is no objective way to identify high-risk newborns currently. This study aimed at discovering the risk factor for Apgar score less than 7 at 1 minute of preterm neonates born with vaginal delivery. A retrospective study was performed in single pregnancy women with a vaginal delivery before 37 weeks of gestation. All the preterm infants were categorized into three types: very preterm birth (28 to 32 weeks), moderate preterm birth (32 to 34 weeks) and late preterm birth (34 to 37 weeks). Risk factors were

Effectiveness of Warm Compression at the Lumbosacral Region on Labor Pain Intensity During the First Stage of Labor Among Parturient Women: A Randomized Controlled Trial.

INTRODUCTION: Labor pain is a fundamental, complex, and subjective experience during childbirth, characterized by severe discomfort that can be difficult for many women to tolerate. There are many non-pharmacological therapies available for the relief of labor pain, such as aromatherapy, music therapy, acupressure, yoga, breathing exercises, etc. The present study was undertaken to assess the effectiveness of warm compression at the lumbosacral region on the intensity of labor pain during the first stage of labor. METHODS: A randomized controlled trial was conducted at the labor room of a ter

A simpler approach to labor induction using lipid-based prostaglandin E2 vaginal suppository.

The outcome of labor induced by use of a glyceride-based vaginal suppository of prostaglandin E2 (PGE2) inserted 3 hours before amniotomy, when the cervix is favorable, has been assessed. Using 5 mg PGE2 for primigravidas and 2.5 mg for multigravidas, 63% of the former and 81% of the latter established labor and were delivered of their infants without oxytocin augmentation, allowing ambulation during early labor. No maternal complications were detected as a result of the PGE2 treatment. Compared with patients undergoing conventional induction by amniotomy and immediate oxytocin titration there

Exposure to maternal vaginal flora during labor and long-term infectious morbidity of the offspring.

PURPOSE: Cesarean delivery (CD) has been linked to increased long-term infectious morbidity in offspring, potentially due to limited exposure to the maternal vaginal microbiome, which may influence immune development. We hypothesized that the degree of exposure to vaginal microbiota during labor would be associated with differences in long-term infectious morbidity. METHODS: We conducted a population-based cohort study including 348,332 singleton deliveries. Offspring were classified into four groups: vaginal delivery (VD, reference), CD for non-progressive labor in the first stage (NPL1), CD

Impact and safety of perineal massage in late pregnancy on delivery outcomes among primiparous women: a propensity score matching analysis.

BACKGROUND: Primiparas face a higher risk of perineal injury, which may adversely affect maternal and infant health. Perineal massage has been proposed as an intervention to improve birth outcomes, but evidence of its effectiveness remains inconsistent, and concerns about its safety persist. This study aimed to evaluate the effects and safety of late-pregnancy perineal massage on birth outcomes in primiparas using propensity score matching. METHODS: A retrospective cohort study was conducted involving 914 primigravid women who received routine perineal massage at a tertiary A-level maternity

Evidence Based Management of Labor.

IMPORTANCE: Induction of labor (IOL) is a common obstetric intervention. Augmentation of labor and active management of the second stage is frequently required in obstetric practice. However, techniques around labor and induction management vary widely. Evidence-based practice regarding induction and labor management can reduce birth complications such as infection and hemorrhage and decrease rates of cesarean delivery. OBJECTIVE: To review existing evidence on IOL and labor management strategies with respect to preparing for induction, cervical ripening, induction and augmentation, and secon

Assisted vaginal birth in 21st century: current practice and new innovations.

Assisted vaginal birth rates are falling globally with rising cesarean delivery rates. Cesarean delivery is not without consequence, particularly when carried out in the second stage of labor. Cesarean delivery in the second stage is not entirely protective against pelvic floor morbidity and can lead to serious complications in a subsequent pregnancy. It should be acknowledged that the likelihood of morbidity for mother and baby associated with cesarean delivery increases with advancing labor and is greater than spontaneous vaginal birth, irrespective of the method of operative birth in the se

Intrapartum risk factors associated with pelvic organ prolapse at 6 months postpartum.

BACKGROUND: Pregnancy and childbirth are known risk factors associated with the development of pelvic organ prolapse; specific intrapartum risk factors are not well characterized. OBJECTIVE: This study aimed to determine intrapartum factors associated with increased risk of pelvic organ prolapse identified after delivery. STUDY DESIGN: A planned secondary analysis of a multicenter randomized clinical trial of delayed vs immediate pushing among nulliparous women at ≥37 weeks of gestation in labor with neuraxial analgesia was conducted at 6 academic and community hospitals in the United States

An Evidence-Based Approach to Vaginal Birth.

This is a review of evidence-based management of vaginal birth, which is critical to optimizing the outcomes of birthing people and neonates. The current literature supports the use of immediate pushing in the second stage of labor, neuraxial anesthesia administration for vaginal birth for adequate pain control, use of warm compresses in labor to prevent obstetric lacerations, delayed cord clamping for term and preterm neonates, the utility of umbilical cord milking in neonates who require immediate resuscitation, and active management of the third stage of labor with oxytocin administration.

Twin and singleton births in Ghana--a case-control study.

A retrospective study involving 623 twin and 1246 singleton births was conducted to compare the two groups with regard to selected maternal, fetal and labor and delivery characteristics and outcomes. Maternal age and parity were significantly higher for twins. The risks of preterm delivery, arrival in the labor ward in second stage of labor, cesarean births and postpartum haemorrhage were significantly higher in twin than in singleton births. In vaginal deliveries twin mothers were significantly less likely to have had episiotomies or perineal lacerations. There was no difference in the durati

Evidence-based guidelines in labor management.

Evidence-based care of women in labor requires a thorough understanding of both "normal" and abnormal labor progress. In response to the growing cesarean delivery rate for dystocia at our institution, a multidisciplinary team of attending physicians, nurse-midwives, resident physicians, and nurses was established to review the literature and create evidence-based guidelines. This article describes the background literature and consensus guidelines reached for the diagnosis of active phase labor, active phase arrest, second-stage arrest, protraction of the active phase, and failed induction of
