INTRODUCTION AND IMPORTANCE: Androgen-producing tumors are rare in females, typically arising from the ovaries or adrenal glands. Sertoli-Leydig cell tumors (SLCTs) are uncommon sex cord-stromal ovarian tumors that cause hyperandrogenism, leading to virilization symptoms such as hirsutism, deep voice, and amenorrhea. Their rarity and similarity with other androgen-secreting conditions make diagnosis challenging. This case report describes an SLCT in a 16-year-old girl, underscoring the value of early diagnosis and fertility-sparing surgery.

Both Leydig cell tumor (LCT), which is a sex cord-stromal tumor, and spermatocytic tumor (ST), which is a germ cell tumor, are rare testicular tumors. Synchronous LCT and ST in the unilateral testis are extremely rare because of their different origins. Herein, we report the case of an 84-year-old male patient with right scrotal swelling due to a testicular tumor. His age was out of the onset peak age of LCT but within ST. To the best of our knowledge, this is the first case with synchronous LCT and ST in the unilateral testis.

BACKGROUND: Testicular Leydig cell tumours (LCTs) are the most common type of sex cord-stromal tumour in men, representing 1%-3% of all testicular neoplasms. Among testicular sex cord-stromal tumours, CTNNB1 mutations and nuclear expression of β-catenin have been typically associated with Sertoli cell tumour. Tetramethyl bisphenol A (TMBPA) is a commonly used bisphenol analog, used as a fire retardant. However, whether it inhibits the function of Leydig cells in late puberty remains unclear.
