Psychosocial and economic factors related to better health among older adults in Europe - a systematic review.

INTRODUCTION: Healthy and positive aging are increasingly recognized in public health as multidimensional processes that integrate physical, psychological, and social resources to support autonomy, active engagement, and quality of life in later life. This systematic review synthesizes recent European evidence on psychosocial and socioeconomic determinants of healthy aging among older adults and contributes to the conceptualization of positive aging from a public health perspective. METHODS: A systematic search of PubMed, Web of Science, and Embase identified empirical studies published in En

Behind closed doors. A case study exploring the lived experiences of a family of a person with the behavioral variant of frontotemporal dementia.

OBJECTIVE: The behavioral variant of frontotemporal dementia is characterized by profound changes in personality and behavior that often start before the age of 65 years. These symptoms impact family life, particularly if (adult) children live at home. In research on young-onset dementia or frontotemporal dementia, the family itself is hardly ever a unit of analysis. Insight in the perspectives of different family members from the same household helps to obtain a deeper understanding of the complex impact of the symptoms on family dynamics. METHODS: This case study explored the perspectives o

Management of patients with multiple myeloma beyond the clinical-trial setting: understanding the balance between efficacy, safety and tolerability, and quality of life.

Treatment options in multiple myeloma (MM) are increasing with the introduction of complex multi-novel-agent-based regimens investigated in randomized clinical trials. However, application in the real-world setting, including feasibility of and adherence to these regimens, may be limited due to varying patient-, treatment-, and disease-related factors. Furthermore, approximately 40% of real-world MM patients do not meet the criteria for phase 3 studies on which approvals are based, resulting in a lack of representative phase 3 data for these patients. Therefore, treatment decisions must be tai

The European educational platform on thoracic surgery.

As the largest scientific organisation world-wide exclusively dedicated to general thoracic surgery (GTS), the European Society of Thoracic Surgeons (ESTS) recognized that one of its priorities is education. The educational platform designed ESTS addresses not only trainees, but also confirmed thoracic surgeons. The two main aims are (I) to prepare trainees to graduation and to the certification by the European Board of Thoracic Surgery and (II) to offer opportunities for continuous medical education in the perspective of life-long learning and continuous professional development to certified

Aligning obstructive sleep apnoea care with underlying drivers: A lifestyle-metabolic perspective.

Continuous positive airway pressure (CPAP) remains the cornerstone of obstructive sleep apnoea (OSA) treatment and can provide rapid symptomatic relief. However, long-term adherence is often suboptimal, and a device-focused model may overlook upstream lifestyle and metabolic drivers. Increasing evidence shows that OSA commonly coexists with, and is partly driven by, modifiable factors such as poor diet quality, physical inactivity, alcohol use, circadian disruption, and adiposity. Emerging anti-obesity pharmacotherapies further support the concept that modifying mechanical and metabolic contro

Prenatal and postnatal antiviral therapies for the prevention and treatment of congenital cytomegalovirus infections.

PURPOSE OF REVIEW: Congenital cytomegalovirus infection (cCMV) is the leading infectious cause of sensorineural hearing loss and lifelong neurodevelopmental disabilities. Studies suggest antiviral therapy can prevent fetal infection after maternal primary infection, as well as halt the progression of hearing loss and neurodevelopmental disabilities in newborns with symptomatic cCMV. With growing worldwide momentum on early detection and diagnosis of cCMV, this review describes the exciting recent advances in antiviral therapies in CMV infected pregnant mothers and babies, as well as emerging e

The MYRACLE protocol study: a multicentric observational prospective cohort study of patients with multiple myeloma.

BACKGROUND: Despite recent advances in the treatment of multiple myeloma, the disease constantly relapses and is still considered as incurable. The current knowledge about the biological mechanisms underlying resistance to the different class of drugs in multiple myeloma remains poor. The primary objective of the MYRACLE (Myeloma Resistance And Clonal Evolution) cohort, a multicenter prospective cohort of patients with multiple myeloma, is to address this limitation. We here describe the study background, design and methods used for this cohort. METHODS/DESIGN: All patients (> 18 year old) di

Mindfulness-based stress reduction as perceived by individuals with pathological mental fatigue after an acquired brain injury.

After acquired brain injury (ABI), some individuals suffer from long-term fatigue and emotional distress, which affects their work ability and daily life. Mindfulness-based stress reduction (MBSR) has shown promising results in quantitative studies as a complementary treatment for pathological mental fatigue (PMF) after ABI. This study aims to explore how people suffering from lasting PMF after ABI experience MBSR in relation to their PMF, with the intention of better meeting the rehabilitation needs of those affected. Seven individuals (mean age 42 years) who had suffered from long-lasting PM

Activity-dependent conduction failure: molecular insights.

Weakness and fatigue are commonly encountered symptoms in neurological disorders and significantly impair quality of life. In the case of motor axons, conduction block contributes to weakness and fatigue and may be associated with aberrant nerve activity including fasciculations and cramp. These symptoms result from dysfunction of the constituent channels and pumps of the axonal membrane. In critically conducting axons, impulse conduction can be impaired by the effects of activity or by other mechanisms that produce a significant shift in membrane potential. Conduction failure may be accentuat

BACKGROUND: Cystic fibrosis (CF) is a complex systemic disease involving numerous organ systems. With improved treatment options and increasing life expectancy of persons with CF (PwCF), extrapulmonary manifestations are coming increasingly into the focus. From birth, almost all PwCF have radiologically detectable pathologies in the upper airways attributable to CF-associated chronic rhinosinusitis (CF-CRS). OBJECTIVE: The aim of this work is to provide an up-to-date overview of CF-CRS from the otorhinolaryngology perspective and to provide the reader with background knowledge and current dev

The burden of a brain tumor: guiding patient centric care in neuro-oncology.

PURPOSE: Brain tumor patients report an overwhelming sense of uncertainty when navigating the course of their terminal disease. Historically, organizational experts and/or treating physicians have established neuro-oncology programs. However, given the disease burden and incurable nature of current medical treatments, patient-centric care should be prioritized alongside institutional and academic objectives. Integrating patient perspectives into interdisciplinary programmatic development can improve comprehensive care and empower patients to advocate for their own quality healthcare needs. ME

A caregiver's perspective on clinically relevant symptoms in behavioural variant frontotemporal dementia: tools for disease management and trial design.

BACKGROUND: Adequate detection of symptoms and disease progression in behavioural variant frontotemporal dementia (bvFTD) is complex. Dementia cohorts usually utilize cognitive and functional measures, which fail to detect dominant behavioural and social cognitive deficits in bvFTD. Moreover, since patients typically have a loss of insight, caregivers are important informants. This is the first qualitative study to investigate caregiver relevant symptoms during the disease course of bvFTD, aiming to improve tools for diagnosis, progression, and future clinical trials. METHODS: Informal caregi

Multidisciplinary approach to implementing ferric carboxymaltose for IV iron replacement in heart failure with iron deficiency.

Iron deficiency (ID) is a common comorbidity in patients with heart failure (HF). Treatment with intravenous (IV) iron is recommended by the ACC/AHA/HFSA and ESC guidelines to improve HF symptoms, exercise capacity, and quality of life, and to reduce the risk of hospitalization. Ferric carboxymaltose (FCM) is indicated for ID in adults with HF (NYHA class II/III) to improve exercise capacity. There are challenges in ensuring that eligible patients with HF and ID achieve iron repletion and associated optimal outcomes in routine practice. These challenges include inpatient formulary restrictions

Nanozymes for Accelerating the Foot Wound Healing: A Review.

Diabetic foot is a most common and life-threatening complication of diabetes. It often results in a protracted course of wound healing. Currently, treatments for diabetic foot include wound dressings, antibiotics, and debridement. However, these treatments are often inadequate due to the complexity of diabetic foot. Various adjuvant therapies are currently under investigation to accelerate wound healing. In recent years, the emergence of nanozymes, nanomaterial with natural enzyme-like characteristics and activities, have attracted much attention. As a promising candidate in biomedicine, the b

Quality of Care for Salivary Gland Diseases.

The treatment of salivary gland diseases represents an important segment of otorhinolaryngology. The individual otorhinolaryngologist might, however, see only a few cases per year from a large variety of salivary gland diseases. Surgical and endoscopic minimal-invasive therapy concepts play a key role. Therefore, gain of knowledge cannot only be provided by prospective clinical trials but also by meta-analyses and potentially also by registry data. Many reliable indicators are established to measure the function of a diseased salivary gland or the improvement of its function after therapy. In
