Cultivated autologous limbal epithelial cell (CALEC) transplantation for limbal tem cell deficiency: a phase I/II clinical trial of the first xenobiotic-free, serum-free, antibiotic-free manufacturing protocol developed in the US.

We developed a two-stage manufacturing process utilizing cultivated autologous limbal epithelial cells (CALEC), the first xenobiotic-free, serum-free, antibiotic-free protocol developed in the United States, to treat blindness caused by unilateral limbal stem cell deficiency (LSCD) and conducted a single-center, single-arm, phase I/II clinical trial. Primary outcomes were feasibility (meeting release criteria) and safety (ocular infection, corneal perforation, or graft detachment). Participant eligibility included male or female participants age 18 to <90 years old and ability to provide writt…

Safety profile and complications of autologous limbal conjunctival transplantation for primary pterygium.

PURPOSE: Primary pterygium is a fibrovascular proliferation over the nasal cornea, probably resulting from the limbal stem cell deficiency. Intraoperative mitomycin-C application seems to associate with reduced recurrences, however produced ocular surface problems and vision threatening complications. The present clinical study investigated the safety profile of autologous limbal conjunctival transplantation in terms of recurrence rate, as the main outcome measure and complications as the secondary outcome. METHODS: The present study was randomised, interventional and prospective clinical stu…

Matrix revolution: molecular mechanism for inflammatory corneal neovascularization and restoration of corneal avascularity by epithelial stem cell transplantation.

Corneal neovascularization (CNV) associated with severe limbal stem cell (LSC) deficiency remains a challenging ocular surface disease in that corneal inflammation may persist and progress, and the condition will not improve without LSC transplantation. A prominent feature after successful LSC transplantation is the suppression of corneal inflammation and CNV, which is generally attributed to the endogenous anti-angiogenic/anti-inflammatory factors secreted by corneal epithelial cells. In addition, corneal epithelial basement membrane (EBM) plays a unique role in the regulation of angiogenesis

Clinical evidence and critical review of mesenchymal stromal cells for corneal and ocular surface diseases.

Mesenchymal stromal cells (MSCs), owing to their regenerative and immunomodulatory properties, have emerged as a potential therapeutic option for disorders affecting the cornea and ocular surface. Early-phase clinical studies have begun to demonstrate the safety and, to some extent, efficacy of MSC-based therapies in conditions such as dry eye disease, persistent corneal epithelial defects, ocular chemical injuries, corneal scarring, keratoconus, and limbal stem cell deficiency. However, evidence from some studies suggests that MSC-related improvements may be short-lived. Currently, the approp…

Delayed Periocular and Anterior Segment Complications Following Locally Administered Chemotherapy for Vitreous Seeding in Retinoblastoma.

BACKGROUND: Long-term follow-up is essential for retinoblastoma survivors, as treatment-related sequelae can emerge years after therapy. METHODS: This case series reviews 67 eyes treated with periocular and/or intravitreal chemotherapy from 2005 through 2020. RESULTS: Periocular chemotherapy was associated with orbital fibrosis in 79.1% of cases, with risk increasing concomitantly with the number of injections. Cataracts developed in 19.4% of eyes, with 10 out of 13 requiring cataract extraction. Other complications included chronic conjunctivitis and limbal stem cell deficiency (3% each), w…

Regeneration of Corneal Epithelium With Dental Pulp Stem Cells Using a Contact Lens Delivery System.

PURPOSE: The corneal epithelium is sloughed off surface of the eye by the action of blinking and is continually replaced by division and maturation of the limbal stem cells (LSCs). In the case of injury or disease, LSCs can be lost or damaged to a point at which the corneal epithelial layer is no longer maintained. leading to LSC deficiencies (LSCDs). When this occurs, the opaque conjunctiva overgrows the anterior surface of the eye, leading to vision impairment or loss. Dental pulp stem cells (DPSCs) are promising candidates as autologous LSC substitutes. In this study, contact lenses (CLs) a

Insights Into the Pathophysiology of Superior Limbic Keratoconjunctivitis in Chronic Ocular Graft-Versus-Host Disease.

Superior limbic keratoconjunctivitis (SLK) is a chronic inflammatory ocular surface disorder affecting the superior conjunctiva and limbus. Its pathogenesis entails a vicious cycle of microtrauma and inflammation. Besides its well-known association with thyroid disorders, SLK has also been associated with systemic conditions involving severe ocular surface disease. One condition that can secondarily develop SLK is ocular graft-versus-host disease (oGVHD). Ocular graft-versus-host disease, a complication of allogeneic hematopoietic stem-cell transplantation, is a pan-ocular surface disease that

Vernal Keratoconjunctivitis: Impact on Children.

Vernal keratoconjunctivitis (VKC) is a seasonally recurring form of allergic conjunctivitis that leads to bilateral inflammation of the cornea and conjunctiva. Symptoms may include itching, foreign body sensation, discharge, and conjunctival hyperemia. Severe cases may result in corneal complications including erosions, shield ulcers, plaques, microbial keratitis, corneal scarring, limbal stem cell deficiency, and keratoconus. Epidemiological data reveal that it mainly affects children and young adults, with male predominance and a higher prevalence in warmer climates. The etiology is multifac…

Regeneration of the corneal epithelium with conjunctival epithelial equivalents generated in serum- and feeder-cell-free media.

PURPOSE: An alternative autologous tissue for ocular surface reconstruction is a potential treatment for the patients with bilateral limbal stem cell deficiency. For the purpose of regenerative procedures in patients, it is desirable to eliminate the involvement of xenogeneic components, such as nonhuman sera and feeder cells. In the present study, we examined the behavior and phenotypic features of cultured conjunctival epithelial sheets generated in serum- and 3T3-free culture conditions when transplanted into the de-epithelialized limbal corneal surface. METHODS: Epithelial cells from norm…

Effects of Exosomal microRNAs on Oral Mucosal Epithelial Cells Cocultured with Limbal Niche Cells.

Autologous cultivated oral mucosal epithelial transplantation (COMET) is an important method for the treatment of limbal stem cell deficiency (LSCD), but the appearance of peripheral corneal neovascularization after COMET has prevented its widespread use in clinical practice. Using limbal niche cells (LNCs) as feeders in the process of coculturing could inhibit the postoperative corneal neovascularization. However, the specific mechanism is still unclear. In this study, LNCs were used as feeder cells to alter the phenotype of cultured oral mucosal epithelial cells (COMECs) by mimicking the pri…

Dry eye disease in mice activates adaptive corneal epithelial regeneration distinct from constitutive renewal in homeostasis.

Many epithelial compartments undergo constitutive renewal in homeostasis but activate unique regenerative responses following injury. The clear corneal epithelium is crucial for vision and is renewed from limbal stem cells (LSCs). Using single-cell RNA sequencing, we profiled the mouse corneal epithelium in homeostasis, aging, diabetes, and dry eye disease (DED), where tear deficiency predisposes the cornea to recurrent injury. In homeostasis, we capture the transcriptional states that accomplish continuous tissue turnover. We leverage our dataset to identify candidate genes and gene networks

Anti-fibrosis potential of pirarubicin via inducing apoptotic and autophagic cell death in rabbit conjunctiva.

This study investigated the potential efficacy of pirarubicin (THP) in modulating rabbit conjunctival fibrosis both in vitro and in vivo and characterized the underlying mechanisms. Primary rabbit conjunctival fibroblasts (RCF) were cultured and treated with THP or mitomycin C (MMC) for 5 min, followed by assaying for cell viability, cell cycle distribution, apoptotic and autophagic pathways. The production of reactive oxygen species (ROS) and chemotaxis of macrophages by RCF were evaluated using 2',7'-dichlorofluorescein diacetate (DCFH-DA) labeling and transwell migration assay, respectively

Chronic Ocular Sequelae of Stevens-Johnson Syndrome: Recent Advances in Understanding of Patho-Physiology and Management.

BACKGROUND: Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) are severe mucocutaneous reactions primarily triggered by drugs or infections. Although acute management focuses on systemic stabilization, chronic ocular sequelae remain a significant concern with vision-threatening complications arising in up to 75% of the affected patients. Ocular manifestations range from dry eye disease, lid margin keratinization (LMK), and conjunctival cicatrization to more severe complications like limbal stem cell deficiency (LSCD), cicatricial entropion, symblepharon, corneal perforation a…

Use of an Eyelid Pressure Patch Concomitantly with a Decellularized Dehydrated Amniotic Membrane for Ocular Surface Disease Management.

INTRODUCTION: Persistent corneal epithelial defects (PCEDs) occur when conditions like dry eye disease (DED), neurotrophic keratitis (NK), and limbal stem cell deficiency impair corneal healing, leading to risks of infection, scarring, or perforation. Decellularized, dehydrated pure amniotic membrane basement membrane (AMBM) supports healing by promoting cell adhesion, growth, and inflammation reduction. Eyelid pressure patching helps stabilize the AMBM, protects the cornea, and enhances its therapeutic effects. METHODS: This retrospective study analyzed 144 eyes treated with either a single-…
