In this article, we describe the operative method for correction of thin lips in patients with bilateral cleft lips. Two transverse triangular flaps were designed in the lower vermilion and elevated along with the labial arteries and the surrounding tissues. Secondary cleft lip-palate deformities frequently involve a combination of skeletal, cartilagenous, dental and muscular deformities. Correction of these deformities should be based on normalization of function by restoring normal anatomy as much as possible. Functional correction of the lip musculature is best done at the same time as surgical correction of the skeletal-dental deformities. In this article, we outline a fundamental technique for secondary lip-nose correction based on muscle reorientation and a concomitant Le Fort I maxillary osteotomy.

Upper lip lifting is a very popular procedure but there is no objective guidelines on how much skin has to be removed to obtain an optimal result. We have measured and compared the philtral height in two groups of young and old female Lebanese subjects. We have found a mean philtral height of 14.3 ± 1.9 mm in the young group and 19.8 ± 2.4 mm In the old group, with a mean philtral lengthening of 5.5 ± 2.9 mm. We suggest using these results to plan the amount of skin that has to be removed during a lip lift procedure.
