Risk factors associated with renal lithiasis during uricosuric treatment of hyperuricemia in patients with gout.

OBJECTIVE: To find factors associated with the development of renal colic during uricosuric therapy. METHODS: We performed a prospective co … excretion. There were 21 clinical events. Two variables showed increased risk hazard for developing lithiasis: clearance of UA at baseline and undissociated urinary UA concentration during followup. When only patients with underexcretion of UA were included in the analysis, undissociated urinary UA during followup remained the only statistically significant …

Helicobacter species are associated with possible increase in risk of biliary lithiasis and benign biliary diseases.

BACKGROUND: Hepato-biliary tract lithiasis is common and present either as pain or as asymptomatic on abdominal ultrasonography for other causes. Although the DNA of Helicobacter species are identified in the gallbladder bile, tissue or stones analyzed from these cases, still a causal relationship could not be established due to different results from different geographical parts. METHODS: A detailed search of pubmed and pubmedcentral was carried out with key words Helicobacter and gallbladder, gallstones, hepaticolithiasis, cholelithiasis and choledocholithiasis, benign biliary diseases, liv…

[Medical treatment of uric acid lithiasis of the urinary tract and usefulness of double J catheter].

OBJECTIVE: To analyze the utility of chemolysis and the double-J ureteral catheter in patients with uric acid calculi. METHODS: The study comprised 30 patients (28 men and 2 women) treated with chemolysis for uric acid calculi and who consulted for renal colic. The indication for placement of a double-J ureteral stent were pain, moderate-to-severe hydronephrosis, fever and urinary tract infection. The calculi were < or = 10 mm in size and were located in the renal pelvis (3), upper ureter (5), middle ureter (2) and lower ureter (23). RESULTS: The uric acid calculi were dissolved with intrave

Extracorporeal shock wave lithotripsy in the treatment of ureteral lithiasis: methodological controversies and therapeutic efficacy.

The treatment of ureteral stones has undergone a radical change in the last 15 years. First, the increased use of endoscopic procedures and then the introduction of extracorporeal lithotripsy relegated traditional surgery to a marginal role for this type of disorder. The best available treatment modality for ureteral lithiasis, particularly distal ureteral stones, is still a matter of great controversy among urologist. With the introduction in clinical use of second- and third generation lithotripters, which are even less invasive and require no anesthesia, interest has increased in treating p…

[Recurrence and prevention of recurrence after dissolution of gallbladder lithiasis by chenodeoxycholic acid in 22 patients].

Dissolution of gallstones was observed 33 times in 22 patients. For 11 of them a recurrent lithiasis was dissolved by means of a second or third medical treatment. 21 patients have been followed for 3 to 6 years (median over 4 years). Without longstanding treatment the recurrence rate was 50 p. 100 in 3 years and 87.5 p. 100 in 6 years. Most of the patients should have relapsed in less than 9 years. Recurrences were always successfully treated with the same treatment but relapsed if the treatment was stopped (21 recurrences in 14 patients). A-1 month every 4 months-long standing treatment did …

Lithiasis of Minor Salivary Gland Mimicking a Venous Malformation: Contextual Diode Laser Photocoagulation and Excision.

Sialolithiasis is the most common disease of the major salivary gland. Lithiasis of minor salivary gland, instead, is very rare. Diagnosis is often challenging as frequency, clinical appearance and symptoms are relatively unknown. We report on a case of minor salivary gland lithiasis of the cheek resembling a venous malformation and its management by diode laser.

Bilateral Ureteral Lithiasis and Postrenal Acute Kidney Injury: A Rare Complication of Diabetic Ketoacidosis in a Child.

Diabetic ketoacidosis (DKA) is a common initial presentation of type 1 diabetes mellitus (T1DM) in children occurring in up to 40% of cases. DKA can also be associated with severe complications, including nephrolithiasis. We present the case of a 12 years and 8-month-old boy who developed acute kidney injury (AKI) secondary to bilateral urinary lithiasis during the onset of T1DM with DKA. After conventional treatment for DKA, laboratory tests showed increased creatinine and azotemia. 24 hours later, he developed lumbar pain and anuria. Plain radiography, ultrasonography, and computed tomograph…

Is the use of fully covered metal stents effective in the treatment of difficult lithiasis of the common bile duct?

BACKGROUND: The management of difficult lithiasis of common bile duct (CBD) often requires a multimodal approach. Fully Covered Metal Stents (FCSEMS) could fragment the stones, dilate strictures of the CBD and progressively enlarge the papillary orifice. The aim of the study was to evaluate the efficacy of FCSEMS in the treatment of difficult lithiasis of CBD. METHODS: All consecutive cases of difficult lithiasis of CBD after an attempt of clearance with complete sphincterotomy were retrospectively considered from 2009 to 2019. Intrahepatic lithiasis cases were excluded. Multivariable analysi…

Endoscopic ultrasound-guided hepaticogastrostomy and antegrade clearance of biliary lithiasis in patients with surgically-altered anatomy.

BACKGROUND AND STUDY AIMS: Endoscopic retrograde cholangiography (ERC) in patients with complex surgically-altered anatomy (SAA) is technically demanding and has limitations. Developments in EUS-guided procedures allow alternative approaches for patients with altered gastrointestinal anatomy and biliary lithiasis. PATIENTS AND METHODS: Single-center, retrospective review of prospectively entered patients with SAA who underwent EUS-guided hepaticogastrostomy (HGS) followed by an interval antegrade endoscopic clearance of biliary lithiasis. RESULTS: 9 patients with Roux-en-Y anatomy underwent …

Transoral sialolitectomy combined with sialoendoscopy versus open submaxilectomy for the management of hilar lithiasis of the submandibular gland.

INTRODUCTION: The treatment of salivary obstructive pathology by minimally invasive techniques has been an object of assessment by different studies for decades. Lithiasis at the level of the salivary duct will be the most frequent cause of obstruction, representing more than 50% of the pathology of the major salivary glands and almost 80% of these affecting the Wharton duct. MATERIAL AND METHODS: A prospective study comparing the results of combined transoral sialolitectomy (CTS) and open submaxillectomy techniques by cervicotomy in 2 groups of patients treated by lithiasis at the level of t…

[Liver resection in the treatment of intrahepatic lithiasis. Immediate and long-term results in a single-center series].

OBJECTIVE: intrahepatic lithiasis (IHL) is an uncommon entity in our environment. When associated with Caroli s disease or stenosis of the biliary radicals it may be necessary to perform liver resection to provide definitive resolution. We present immediate and long term results in a Spanish series of patients with hepatic resection to treat hepatolithiasis. PATIENTS: between January 1996 and December 2007 we performed a liver resection (LR) in 8 patients for IHL. The IHL was associated with Caroli s disease in 3 cases and with stenosis of segmentary radicals in the other 5 cases. It manifest…

Pathogenesis of pancreatic and biliary CaCO3 lithiasis: the solubility product (K'sp) of calcite determined with the Ca++ electrode.

There are three digestive secretory fluids with high bicarbonate and high pH values: bile, pancreatic juice, and saliva. Each is subject to development of CaCO3-containing stones. In bile, calcium precipitation is a requisite event in the initiation and growth of all pigment gallstones and is postulated to initiate cholesterol gallstone formation by forming a nidus for cholesterol precipitation. In the pancreas, stones in both humans and cattle are composed largely of calcite. Knowledge of appropriate solubility product constant (K'sp) values is essential in defining lithogenicity in each of t
