Evaluation of an experimental oblique plate for osteosynthesis of mandibular condyle fractures.

OBJECTIVE: The aim of this study was to test the bone thickness and potential screw length for osteosynthesis of condylar base fractures (according to the Loukota classification) with an experimental titanium plate, placed in an ideal position against 2 types of conventional plates. STUDY DESIGN: After exclusion of completely edentulous mandibles, 28 dentate macerated mandibles available at the time of the study were included. Linear regression models 1 and 2 compared the sums of the 2 cranial bone thicknesses and the 3 caudal thicknesses among the 3 different plate designs, and linear models

[Research progress on surgical approaches for the treatment of mandibular condylar fractures].

With rapid advancements in internal fixation and new surgical instruments, surgical treatment has gradually emerged as the first treatment choice for mandibular condylar fractures. An appropriate surgical incision is the first and determinant step during surgery to avoid postoperative complications. Hence, surgical approaches in relation to oral and maxillofacial trauma surgery for the treatment of mandibular condylar fractures have been extensively investigated. This review summarizes the research progress on surgical approaches in the management of mandibular condylar fractures.

Closed treatment of unilateral mandibular condyle fractures in adults: a systematic review.

Of all mandibular fractures, 25-35% are condylar. Many studies have focused on whether to treat such fractures via open or closed modalities. A uniform protocol for closed treatment is lacking, but such a protocol could ensure good clinical practice. The aims of this systematic review were to provide an overview of the published studies exclusively pertaining to closed treatment and to summarize the existing modalities for closed treatment and their clinical outcomes. Sixteen studies were selected for detailed analysis. The treatments given were highly variable, ranging from doing nothing to a…

Biomechanical analysis of titanium fixation plates and screws in mandibular angle fractures.

OBJECTIVE: The aim of this study was to evaluate the mechanical behavior of different rigid fixation methods in mandibular angle fractures. MATERIALS AND METHODS: Three different three-dimensional finite element models of the mandible were developed to simulate the biomechanical responses of titanium plates and screws. The fracture lines were fixed with double 4-hole straight, 4-hole square, and 5-hole Y plates with monocortical screws. 150 N incisal occlusal loads were simulated on the models. The commercial ANSYS software was utilized to calculate the Von Mises stresses on fixative applianc

Prospective study of the quality of life after treatment of mandibular fractures.

Health-related quality of life (QoL) has become increasingly important, but few studies have dealt with that of patients who have been treated for mandibular fractures. Our aim was to assess this. Patients with mandibular fractures (n=148) were studied prospectively and QoL after treatment was assessed using the General Oral Health Assessment Index (GOHAI). The male-female ratio was 8.3:1 and their ages ranged from 14 to 70 years. QoL after treatment of the fractures declined initially (on the first postoperative day) but thereafter improved steadily. There was no significant difference betwee…

Osteogenesis Imperfecta Diagnosed from Mandibular and Lower Limb Fractures: A Case Report.

Osteogenesis imperfecta (OI) is a congenital disease characterized by bone fragility and low bone mass. Despite the variety of its manifestation and severity, facial fractures occur very infrequently. Here, we report a case of an infant diagnosed with OI after mandibular and lower limb fractures. A boy aged 1 year and 3 months was brought to his neighboring hospital with a complaint of facial injury. He was transferred to our hospital to undergo operation 3 days later. Computed tomography images revealed multiple mandibular fractures including complete fracture in the symphysis and dislocated …

A Complex Facial Trauma Case with Multiple Mandibular Fractures and Dentoalveolar Injuries.

The principles of management of mandibular fractures differ in children when compared to adults and depend on the specific age-related status of the growing mandible and the developing dentition. This paper presents a case report with a complex facial trauma affecting the mandibular body and condyle region and dentoalveolar complex. Clinical examination revealed soft tissue injuries, limited mouth opening, lateral deviation of the mandible, an avulsed incisor, a subluxated incisor, and a fractured crown. CBCT examination revealed a nondisplaced fracture and an oblique greenstick fracture of th…

Surgical versus non-surgical treatment of mandibular condylar fractures: a meta-analysis.

The aim of the present study was to test whether there is a significant difference in the clinical outcomes between surgical and non-surgical treatment of mandibular condylar fractures. An electronic search was undertaken in February 2014. Eligibility criteria included clinical human studies, either randomized or not. The search strategy resulted in 36 publications. The estimates of an intervention were expressed as the risk ratio (RR) and mean difference (MD) in millimetres. A statistically significant effect was observed for the outcome of post-treatment malocclusion (RR 0.46, P<0.00001), la

Characterization and management of mandibular fractures: lessons learned from Iraq and Afghanistan.

Fractures of the mandible are among the most common facial injuries. Invasiveness of treatment should be determined by the extent of injury: degree of displacement, number of fractures, the patient's health status, and concomitant injuries. Complex, comminuted, and avulsive injuries frequently seen in combat will require coordination with multiple specialties to provide the best treatment. Stabilization treatment with arch bars or external fixators and splints is often desirable when fractures are highly comminuted or the soft tissue envelope is compromised by tissue loss or burns. In severe i

Hyperostosis as a late sequel of parasymphyseal mandibular fractures in 2 children.

BACKGROUND: The potential problem of growth anomalies affecting a mandible following a fracture of a mandibular condyle in childhood is well established. However, there have been no previous reports of this phenomenon affecting other fracture sites in the mandible. PATIENTS: Two patients who had parasymphyseal fractures treated in childhood presented at skeletal maturity with hyperostosis at the fracture site, producing chin asymmetry in their teens. RESULTS: In both cases the hyperostosis produced significant chin asymmetry without disturbance of the occlusion. Both patients were managed wi

Intracapsular fractures of mandibular condyle: diagnosis, treatment, and anatomical and pathological evaluations.

The aim of this study was to attempt to establish a correlation between condylar localized fracture, onset of anatomicopathological lesions of the articular structures, and determination of ankylosis so as to define appropriate diagnostic and therapeutic procedures. Clinical, arthroscopic, and histological studies were carried out in two patients with a severe temporomandibular joint disorder after a condylar localized fracture. Treatment included removal of the displaced fragments, condylar surface remodeling, suture of retrodiskal perforations, and diskal repositioning. The histological stud

[The clinical characteristics of mandibular fractures in middle-aged and elderly subjects].

The authors analyze case histories of 642 patients aged over 50 years with mandibular fractures, treated in 1988-1998 at Clinic for Maxillofacial Surgery of Russian State Medical University. Multiple fractures occurred in patients with mandibular adentia 1.5 times more often than in patients with teeth. The sites of fractures were different in patients with intact occlusion and in those without teeth. Contrary to the traditional opinion, the study showed a high incidence of open mandibular fractures in patients both with and without teeth. Inflammatory complications develop more often than in …

Mandibular fractures following third molar extraction. A retrospective clinical and radiological study.

Twelve patients with 13 mandibular fractures following third molar extraction were treated in our Department between 1980 and 1995. Clinical and radiographic data relating to these patients were analysed retrospectively to determine complication characteristics. Only one fracture occurred during surgery. The first week after the operation was found to be most critical for fracture occurrence. A "cracking" sound from the jaw while eating frequently indicated that a fracture had occurred, but repeated radiographic examination may be necessary for definitive diagnosis. Most teeth removed belonged…

[Neurologic examinations for facial nerve damage in surgically treated mandibular collum fractures].

Transcranial magnetic stimulation is a new method for objective evaluation of the facial nerve without constraints for the patient. By the first time, especially cortical lesions can be detected objectively. Using this method, a permanent disturbance of the ramus marginalis mandibularis after operative treatment of fractures of the mandibular condyle could be certainly excluded. Therefore, because of the clear presentation and the possibility of enlargement, the deep submandibular approach, using a correct operative technique, can be strictly recommended.

The use of rigid internal fixation in mandibular fractures complicated by osteomyelitis.

PURPOSE: Reports on treatment of "infected" mandibular fractures with open reduction and rigid internal fixation (RIF) consist of either isolated cases or mixed series of patients with soft tissue infections or inadequately documented osteomyelitis. The definition of "infected fracture" is often vague and may include both soft tissue or bone sepsis. MATERIALS AND METHODS: In this retrospective study, seven patients with mandibular fractures and documented osteomyelitis were treated by a protocol that included open reduction and RIF. Frequency and type of dental traumas in mandibular body and condyle fractures.

Dental injuries in association with 207 mandibular fractures were evaluated from patient files and radiographs. It was shown that 32% of the dentulous patients with condylar fractures had dental injuries, approximately 3.7 traumatised teeth per patient, most of which were dental hard tissue injuries and situated typically in the maxillary molar region. Dental injuries were diagnosed in 30% in association with mandibular corpus fractures, approximately 3.6 traumatised teeth per patient, and injuries were more often dental luxations in the anterior part of the mandible. It was found that 143 tee…
