The "three delays model" illustrates how issues around obstetric emergency can lead to maternal deaths. This study applied in-depth interviews of key community gatekeepers in 16 rural communities across two states in northern Nigeria to evaluate the presence and functionality of key community maternal support systems for reducing maternal mortality. Findings show that only one out of the 16 communities had all the key support systems. Five rural communities reported that pregnant women have standing permission to visit health facilities during obstetric emergencies. A quarter of the communitie…

Introduction: Many pregnancy-related deaths can be avoided if health care workers reliably provide prompt, evidence-based care during the final stages of pregnancy and delivery. We report the impact on the institutional maternal mortality ratio (iMMR) of a quality improvement (QI) initiative that focused on establishing timely and reliable obstetric care in public Brazilian hospitals for the primary causes of maternal deaths. Methods: The QI initiative used a quasi-experimental time-series design implemented in 19 Brazilian maternity hospitals comparing 3 periods: baseline (January 2018 to Nove…

Maternal lifestyle influences the health of both the mother and fetus. Acute stress is detrimental to health and increases the risk of obstetric complications. However, the consequences of chronic mild stress are less understood. We investigated the effect of chronic mild maternal stress on gestation and its influence on the immune response at the maternal-fetal interface. BALB/c female mice were exposed to a model of mild stress involving reduced litter material compared with the control condition. Also, a shaking protocol of 1 min every other day was initiated two weeks before mating and con

IMPORTANCE: Maternal and perinatal mortality remain high in the US despite growing rates of prenatal services and spending, and little rigorous evidence exists regarding the impact of prenatal care intensity on pregnancy outcomes. Patients with an expected date of delivery just after their 35th birthday may receive more intensive care owing to the advanced maternal age (AMA) designation; whether this increase in prenatal care is associated with improvements in outcomes has not been explored.
OBJECTIVE: To determine the association between the AMA designation and prenatal care services, severe

BACKGROUND: The Sustainable Development Goal target 3.1 aims to reduce the global maternal mortality ratio to less than 70 per 100,000 live births. Great disparities reported in maternal mortality ratio between and within countries make this target unachievable. To gain more insight into such disparities and to monitor and describe trends, confidential enquiries into maternal deaths are crucial.
OBJECTIVE: We aimed to study the trend in maternal mortality ratio, causes, delay in access and quality of care, and "lessons learned" in Suriname, over almost 3 decades with 3 confidential enquiries …

The aim of the study was to evaluate the maternal mortality cases attributed to pulmonary embolism (PE). PE constituted 7.58% of maternal deaths in 2013. Risk factors for PE were present in 15 (88.2%) of the women. Five women (29.4%) were overweight, and 5 (29.4%) were obese. Four women (23.5%) had cardiac diseases. PE occurred in the postpartum period after caesarean delivery in 9 (52.9%) patients. Eleven (64.7%) of the maternal deaths were recognised as preventable. More deaths attributed to PE occurred in the postpartum period (n = 11) than the antepartum period (n = 5). One other maternal …
