Short-Term Incidence of Sequelae of HCV Infection Among Medicaid Beneficiaries in Oregon.

OBJECTIVES: Given the known high morbidity and mortality of hepatitis C virus (HCV) infection in Oregon, we sought to develop a practical method of estimating the severe sequelae of HCV infection among Medicaid beneficiaries in Oregon. METHODS: We assembled a retrospective cohort that identified all Oregon Medicaid beneficiaries with HCV infection enrolled for at least 1 year during 2009-2013. We linked this cohort to 3 data sets to identify HCV-related deaths, cases of hepatocellular carcinoma (HCC), and first hospitalizations for advanced liver disease (ALD). We calculated incidence density…

Randomized Trial of Interventions for Smoking Cessation Among Medicaid Beneficiaries With Mental Illness.

OBJECTIVE: Medicaid beneficiaries with severe mental illnesses are a financially disadvantaged group with high rates of smoking and poor cessation outcomes. This study examined whether abstinence-contingent monetary incentives improved outcomes when added to cessation treatments at community mental health centers: prescriber visit for pharmacotherapy only (PV only), prescriber visit and facilitated quitline (PV+Q), and prescriber visit and telephone cognitive-behavioral therapy (PV+CBT). METHODS: During 2012-2015, a total of 1,468 adult, daily smoking Medicaid beneficiaries with mental illnes…

Second-Generation Antipsychotic Prescribing Patterns for Pediatric Patients Enrolled in West Virginia Medicaid.

OBJECTIVE: The prescribing of second-generation antipsychotics for young people has increased dramatically. Studies have shown that children enrolled in Medicaid are more likely than those with private insurance to receive antipsychotics, leading many states to require prior authorization (PA) for their use. However, little is known about how PA programs affect prescribing patterns for antipsychotics or other psychotropic medications. This study examined a PA program for second-generation antipsychotic use for children under 18 in West Virginia Medicaid. Prescribing rates for antipsychotics an…

Psychotropic Treatment Pattern in Medicaid Pediatric Patients With Concomitant ADHD and ODD/CD.

OBJECTIVE: To describe psychotropic treatment pattern and evaluate the association of socio-demographic factors and psychotropic combination therapy in children with ADHD and oppositional defiant disorder/conduct disorder (ODD/CD). METHOD: This is a cross-sectional drug utilization study based on Medicaid fee-for-service programs in 26 U.S. states (1999-2006). Children aged 4 to 18 with concomitant ADHD and ODD/CD were included. We calculated the prevalence of psychotropic drugs and used logistic regression to evaluate the role of socio-demographic factors in psychotropic combination therapy.…

Telemedical cervical cancer screening to bridge medicaid service care gap for rural women.

BACKGROUND: The Arkansas Medicaid program for low-income women provides cervical cancer screening, in the form of Pap smears, and treatment but no diagnostic means of bridging the two, such as a procedure called "colposcopy." Telemedicine offers a viable means to bridging this gap. Previously telecolposcopy has been used in small demonstration projects as a means to deliver colposcopy services to at-risk rural populations at a comparable quality to in-person colposcopy. SUBJECTS AND METHODS: The University of Arkansas for Medical Sciences' Antenatal & Neonatal Guidelines, Education and Learni…

Benefit policy and disenrollment of adult Medicaid beneficiaries from the Oregon health plan.

Oregon's Medicaid program experienced a dramatic decrease in its non-categorically eligible adult members after implementing a new benefit policy in February 2003 for these beneficiaries. The policy included four main elements: premium increases for some enrollees; a more stringent premium payment policy; elimination of some benefits, including mental health and substance abuse treatment; and, the imposition of co-payments. The study compared monthly disenrollment rates eight months before and after the policy change. The new premium payment policy was found to be the main driver of disenrollm…

Medicaid prescription drug policies and medication access and continuity: findings from ten states.

OBJECTIVES: The aims of this study were to compare medication access problems among psychiatric patients in ten state Medicaid programs, assess adverse events associated with medication access problems, and determine whether prescription drug utilization management is associated with access problems and adverse events. METHODS: Psychiatrists from the American Medical Association's Masterfile were randomly selected (N=4,866). Sixty-two percent responded; 32% treated Medicaid patients and were randomly assigned a start day and time to report on two Medicaid patients (N=1,625 patients). RESULTS…

PTSD and physical comorbidity among women receiving Medicaid: results from service-use data.

Patterns of physical comorbidity among women with posttraumatic stress disorder (PTSD) were explored using Michigan Medicaid claims data. PTSD-diagnosed women (n = 2,133) were compared with 14,948 randomly selected women in three health outcome areas: ICD-9 categories of disease, chronic conditions associated with sexual assault history in previous research, and reproductive health conditions. PTSD was associated with increased risk of all categories of diseases (OR range = 1.3-4.8), endometriosis (OR = 2.7), and dyspareunia (OR = 3.4). When PTSD was not complicated by other mental health cond…

Academic managed care organizations and adverse selection under Medicaid managed care in Tennessee.

CONTEXT: Health plans competing in a managed care system may face serious financial consequences if they are disproportionately selected by enrollees with expensive health conditions. Academic medical centers (AMCs) have traditionally provided medical care for the sickest patients and may be at particularly high risk for adverse selection, but whether this occurs is not known. OBJECTIVE: To determine whether managed care organizations (MCOs) representing AMCs are adversely selected by Medicaid managed care (MMC) enrollees with expensive chronic health conditions. DESIGN AND SETTING: Observat…

Medicare/Medicaid program; revisions in reporting, recordkeeping, and other requirements--HCFA. Final rule.

This final rule amends several regulations containing collection of information requirements. These requirements affect the Medicare providers of outpatient physical therapy and speech pathology services; physical therapists in independent practice; portable X-ray services; and Medicaid contracts with health maintenance organizations (HMOs) and prepaid health plans (PHPs). This final rule also sets forth other changes affecting outpatient physical therapy and speech pathology services and physical therapists in independent practice. These changes, authorized by sections 2341 and 2342 of Pub. L…

Preconception, Prenatal and Postnatal PM2.5 Exposure and Neurodevelopmental Disorder Risk among Medicaid Recipients.

Evidence linking ambient fine particulate matter (PM2.5) exposure to childhood neurodevelopmental disorders (NDDs) remains mixed. We conducted a cohort study (2001-2013) of 1,547,244 Medicaid-enrolled mother-child pairs to assess associations between zip-code-level PM2.5 exposure during preconception (12-week), prenatal (37-week), and postnatal (3-year) periods and children's NDD risk. Cox proportional hazards models estimated risks of autism spectrum disorder (ASD), attention-deficit/hyperactivity disorder (ADHD), learning difficulty (LD), speech/language disorder, developmental coordination …

Contraceptive and Enzyme-Inducing Antiseizure Medication Coprescription Among US Medicaid Beneficiaries With Epilepsy.

BACKGROUND AND OBJECTIVES: People with epilepsy of child-bearing potential (PWECP) require access to highly effective contraception, yet many antiseizure medications (ASMs) can reduce the effectiveness of some hormonal methods by inducing hepatic enzymes. In this retrospective cohort study, we aimed to identify the proportion of PWECP prescribed a susceptible hormonal contraceptive method who were concurrently prescribed an interacting ASM and patient characteristics associated with coprescription. METHODS: We obtained Medicaid claims data from 2016 to 2021 for female beneficiaries and define…

What Comes Next? When and How Medicaid Home- and Community-Based Services End.

BACKGROUND: As Medicaid long-term services and supports shift towards home-and-community-based settings (HCBS), it is important to understand trajectories of HCBS use among older adults, including duration and living circumstances at cessation, to inform decisions about whether state Medicaid programs can afford to sustain or expand HCBS benefits. METHODS: This study examined determinants of HCBS episode duration and how they vary by status at cessation (community discharge, nursing home, or deceased). We used Southern Community Cohort Study (SCCS) data linked with Medicaid and Medicare claim…

Association of Continuous Medicaid Eligibility With Postpartum Coverage and Opioid Use Disorder Treatment.

INTRODUCTION: Over three quarters of pregnant people with opioid use disorder are enrolled in Medicaid at delivery, but pregnancy-related Medicaid eligibility has historically ended 60 days postpartum, leading to periods without insurance and decreased access to health care during the first year postpartum. The Families First Coronavirus Response Act, passed in March 2020, prevented states from disenrolling Medicaid enrollees, including postpartum enrollees, during the COVID-19 Public Health Emergency. METHODS: A difference-in-difference model was used to compare the continuity of coverage am…

The Objectives of Medicaid: The Legal Definition Behind America's Most Important Health Program.

Medicaid work requirements were found in 2018 to be contrary to the Medicaid statute because, according to the D.C. Circuit, "the objective of Medicaid was to furnish medical assistance." Even though the Secretary of Health and Human Services determined that the work requirements were "likely to assist in improving health outcomes," the court concluded that "better health outcomes as [an] objective of Medicaid … lacks textual support. Indeed, the statute makes no mention of that objective."With the second Trump administration making Medicaid work requirements a centerpiece of its budget, the "…
