Refined classification and phenotype-driven analysis of PIEZO1 variants in hereditary red blood cell and iron disorders.

Interpreting genetic variants in complex genes such as PIEZO1 remains challenging because of marked allelic heterogeneity, relative tolerance to missense variation, and overlapping clinical phenotypes. Gain-of-function variants in PIEZO1 cause dehydrated hereditary stomatocytosis (DHS1, or hereditary xerocytosis), a pleiotropic syndrome characterized by anemia of variable severity and iron overload. In this study, we provided and applied an integrative framework combining the American College of Medical Genetics and Genomics guidelines, quantitative in silico predictions, structural domain ann

Transient Nephrotic Syndrome in an Infant With Heterozygous Variants of Uncertain Significance in LMX1B and TTC21B.

Infantile nephrotic syndrome is rare and frequently associated with an underlying genetic cause. Oligogenic inheritance has been proposed in selected cases, although supporting evidence remains limited. We report the case of an 8-month-old infant presenting with nephrotic syndrome characterized by massive proteinuria (25 g/g creatinine), hypoalbuminemia (17 g/L), and preserved renal function. Genetic testing using a targeted nephrotic syndrome panel identified two heterozygous variants of uncertain significance (American College of Medical Genetics and Genomics (ACMG) class 3) in LMX1B and TTC

Genomic Profiling of Anophthalmia/Microphthalmia-Associated CNVs Reveals Complex Genotype-Phenotype Correlations and Incomplete Penetrance.

BACKGROUND: Anophthalmia/microphthalmia (A/M) is a severe congenital ocular malformation characterized by the complete absence or small size of the eye bulb. Interpreting copy number variations (CNVs) in A/M is challenged by variable genotype-phenotype correlations and reduced penetrance. This study investigated the genetic etiology of A/M-associated CNVs. METHODS: Genomic profiling was performed on four unrelated families presenting with ocular anomalies or harboring A/M-susceptible CNVs. Variants were evaluated by integrating American College of Medical Genetics and Genomics (ACMG) guidelin

Hybrid-RViT: Hybridizing ResNet-50 and Vision Transformer for Enhanced Alzheimer's disease detection.

Alzheimer's disease (AD) is a leading cause of disability worldwide. Early detection is critical for preventing progression and formulating effective treatment plans. This study aims to develop a novel deep learning (DL) model, Hybrid-RViT, to enhance the detection of AD. The proposed Hybrid-RViT model integrates the pre-trained convolutional neural network (ResNet-50) with the Vision Transformer (ViT) to classify brain MRI images across different stages of AD. The ResNet-50 adopted for transfer learning, facilitates inductive bias and feature extraction. Concurrently, ViT processes sequences

Deep learning-based automatic detection and grading of disk herniation in lumbar magnetic resonance images.

Magnetic resonance imaging of the lumbar spine is a key technique for clarifying the cause of disease. The greatest challenges today are the repetitive and time-consuming process of interpreting these complex MR images and the problem of unequal diagnostic results from physicians with different levels of experience. To address these issues, in this study, an improved YOLOv8 model (GE-YOLOv8) that combines a gradient search module and efficient channel attention was developed. To address the difficulty of intervertebral disc feature extraction, the GS module was introduced into the backbone net

Caregiver report of social-emotional functioning in infants and young children after inflicted traumatic brain injury.

Social-emotional difficulties are common sequelae of traumatic brain injury (TBI). Children who have experienced inflicted TBI (iTBI) may be at increased risk for social-emotional problems due to the risk factors associated with both early neurologic injury and with child maltreatment. We characterized the associations among injury severity, caregiver type (i.e., biological parents, non-kinship, kinship), and child social-emotional functioning in 41 infants and young children who had sustained iTBI and were seen in a large, regional children's hospital. This study was a retrospective analysis,

"Grasping the grey": patient understanding and interpretation of an intermediate allele predictive test result for Huntington disease.

Since the discovery of the genetic mutation underlying Huntington disease (HD) and the development of predictive testing, the genetics of HD has generally been described as straightforward; an individual receives either mutation-positive or negative predictive test results. However, in actuality, the genetics of HD is complex and a small proportion of individuals receive an unusual predictive test result called an intermediate allele (IA). Unlike mutation-positive or negative results, IAs confer uncertain clinical implications. While individuals with an IA will usually not develop HD, there re

The role of obesity in cancer survival and recurrence.

Obesity and components of energy imbalance, that is, excessive energy intake and suboptimal levels of physical activity, are established risk factors for cancer incidence. Accumulating evidence suggests that these factors also may be important after the diagnosis of cancer and influence the course of disease, as well as overall health, well-being, and survival. Lifestyle and medical interventions that effectively modify these factors could potentially be harnessed as a means of cancer control. However, for such interventions to be maximally effective and sustainable, broad sweeping scientific

Managing hereditary gastrointestinal cancer syndromes: the partnership between genetic counselors and gastroenterologists.

A standard of care for the recognition and management of patients and families with hereditary gastrointestinal cancer syndromes is emerging, and both gastroenterologists and genetic counselors have an important role. This Review considers these roles and outlines the partnership needed between gastroenterologists and genetic counselors. The role of the gastroenterologist involves careful sampling of polyps and tumors, and requires a basic working knowledge of the features of hereditary gastrointestinal cancer syndromes. The role of the genetic counselor is to gather and synthesize family hist

In vitro methods to study chemically-induced hepatotoxicity: a literature review.

Understanding the hepatotoxicity of drugs and chemicals is essential for progress in the pharmaceutical industry, medical science and academic research. The study of hepatotoxicity in vitro is complicated by the difficulty of maintaining hepatocytes in culture due to a lack of understanding of the humoral and matrix requirements of these cells. A variety of in vitro models of the liver have been developed, such as perfused livers, liver slices and three-dimensional perfused bioreactors, but the static cell culture is the most commonly used system. In this review we present the advantages and d

Hiccup and apparent myoclonus after hydrocodone: review of the opiate-related hiccup and myoclonus literature.

The author recently encountered a patient with hiccups, intermittently accompanied by apparent focal rhythmic diaphragmatic myoclonus after hydrocodone administration. Review of the literature disclosed a paucity of previous reports of hiccup, but many reports of myoclonus after opiate administration. A wide variety of opiates and routes of administration have been implicated, but high doses and the presence of other agents (antipsychotics, antiemetics, nonsteroidal antiinflammatory agents, antidepressants) may pose special risks. Review of the literature suggests three types of opiate-related

Radiological and Biological Dictionary of Radiomics Features: Addressing Understandable AI Issues in Personalized Prostate Cancer, Dictionary Version PM1.0.

Artificial intelligence (AI) can advance medical diagnostics, but interpretability limits its clinical use. This work links standardized quantitative Radiomics features (RF) extracted from medical images with clinical frameworks like PI-RADS, ensuring AI models are understandable and aligned with clinical practice. We investigate the connection between visual semantic features defined in PI-RADS and associated risk factors, moving beyond abnormal imaging findings, and establishing a shared framework between medical and AI professionals by creating a standardized radiological/biological RF dict

Integrating Interpretability in Machine Learning and Deep Neural Networks: A Novel Approach to Feature Importance and Outlier Detection in COVID-19 Symptomatology and Vaccine Efficacy.

In this study, we introduce a novel approach that integrates interpretability techniques from both traditional machine learning (ML) and deep neural networks (DNN) to quantify feature importance using global and local interpretation methods. Our method bridges the gap between interpretable ML models and powerful deep learning (DL) architectures, providing comprehensive insights into the key drivers behind model predictions, especially in detecting outliers within medical data. We applied this method to analyze COVID-19 pandemic data from 2020, yielding intriguing insights. We used a dataset co

Prolonged Background Suppression Induced by Anticonvulsants Misleading Amplitude-Integrated Electroencephalography's Interpretation: A Single Case Report.

Prognostic role of the amplitude-integrated electroencephalography (aEEG) is undeniable, but few works focused on the contribution of medications in misleading its interpretation. We report the case of an asphyxiated newborn enrolled for therapeutic hypothermia (TH) that required the simultaneous use of three anticonvulsants, whose administration resulted in a comatose state and in a switch to a flat trace on the aEEG. The prolonged flat trace on the aEEG, pointing toward a state of irreversible brain damage, led to early stop of TH to prevent therapeutic obstinacy; unexpectedly, once Midazola
