A Case of Myasthenia Gravis Diagnosed After Hospitalization in a Patient Receiving Home Oxygen Therapy for Respiratory Failure.

INTRODUCTION: Respiratory failure has various causes, including neuromuscular disorders. We report a case of myasthenic crisis requiring intensive care after a delayed diagnosis. CASE PRESENTATION: A woman in her 70s had been on home oxygen therapy for 10 months due to dyspnea. She was admitted with worsening dyspnea, type II respiratory failure, and pneumonia requiring mechanical ventilation. Although her pneumonia improved, ventilator weaning was unsuccessful. Muscle weakness raised suspicion of neuromuscular disease. Myasthenia gravis was a differential diagnosis, but she showed no thymoma…

Immune checkpoint inhibitor cardiomyotoxicity: a case series of five patients and development of a clinical management algorithm.

Immune checkpoint inhibitor (ICI) therapies have revolutionised the management of a range of malignancies. However, the syndrome of ICI induced cardiomyotoxicity, also reported as triple "M" syndrome (myasthenia gravis, myositis and myocarditis) is an unfortunate severe consequence in up to 1%-2% of patients receiving ICI, with an associated mortality of up to 50%. Based on international evidence and local experience, we have developed a clinical pathway in our institution as a guide to the early identification and targeted management of this condition.

Physical Exercise in Myasthenia Gravis: A Systematic Review.

Background: Myasthenia gravis (MG) is a chronic autoimmune disorder characterized by fluctuating skeletal muscle weakness and fatigue, leading to reduced functional independence and impaired quality of life (QoL). Although exercise has historically been discouraged due to concerns about symptom exacerbation, emerging evidence suggest that structured exercise programs may be safe and beneficial in clinically stable patients. This systematic review critically evaluates current evidence on exercise and physical activity interventions in MG, focusing on effectiveness, safety, and impact on functio…

Finding the Sweet Spot: Exploring Nurse Practitioner Job Satisfaction, Burnout and Resilience.

Nurse practitioners (NPs) report concerning levels of burnout that can negatively impact patient outcomes and job turnover. Little is known about how NPs effectively navigate their practice and mitigate worklife stressors. Using a constructivist grounded theory approach, this study explored this gap by investigating burnout, job satisfaction and resilience among 10 hospital-based NPs. Data analysis revealed three key themes: (1) making a difference … keeps me going; (2) finding the sweet spot; and (3) being the safety net. Promoting a healthy work environment requires a targeted plan that invo

Diagnostic Challenges of LRP4 Antibody Testing in Myasthenia Gravis: A Case Series of Four LRP4 Positive Cases With Uncertain Diagnostic Significance.

Myasthenia gravis is an autoimmune disorder characterized by muscle weakness due to impaired neuromuscular transmission. While antibodies against the acetylcholine receptor and muscle-specific kinase are commonly used for diagnosis, a subset of patients remains seronegative, necessitating alternative biomarkers. Low-density lipoprotein receptor-related Protein 4 antibodies have emerged as a potential diagnostic marker in seronegative myasthenia gravis cases. However, misleading positive results have obscured diagnosis and management. This study explores the clinical relevance of low-density li…

Construction of patient trajectories to model clinical trial outcomes: application to myasthenia gravis.

INTRODUCTION: Accurate prediction of patient outcomes in clinical trials is crucial for the timely assessment of treatment efficacy. This study proposes a novel approach to predict patient response using longitudinal clinical data. METHODS: We construct temporal trajectories from longitudinal data and extrapolate these trajectories to forecast individual patient outcomes. Additionally, we assess when new patients align with established response patterns. The approach is evaluated using data from the MGTX trial involving patients with myasthenia gravis. RESULTS: Our analysis demonstrates the …

Severe Hypogammaglobulinemia (IgG) During Efgartigimod Therapy in Neurological Practice: A Real-World Case Series.

Background Efgartigimod is a neonatal Fc receptor (FcRn) antagonist approved for adults with generalized myasthenia gravis (gMG) who are anti-acetylcholine receptor antibody-positive, and for chronic inflammatory demyelinating polyradiculoneuropathy (CIDP). By blocking FcRn-mediated IgG recycling, efgartigimod lowers circulating IgG and reduces pathogenic autoantibodies. Clinical trials have demonstrated efficacy and an overall acceptable safety profile, but patients with low baseline IgG were generally not well represented. Objective The main objective of this study is to describe severe redu…

Myasthenia gravis complicated with autoimmune encephalitis: a review.

OBJECTIVE: This review aims to provide updated information about myasthenia gravis (MG) complicated with autoimmune encephalitis. BACKGROUND: MG and autoimmune encephalitis are both antibody-mediated disorders with a need for active and individually adapted immunosuppressive treatment. The two disorders can co-exist. This represents therapeutic and diagnostic challenges but may help in elucidating disease mechanisms and risk factors. METHODS: A literature search combined the words "myasthenia gravis," "autoimmune encephalitis," and further combined each specific antibody associated with auto…

Ginsenoside Rg1 ameliorates experimental autoimmune myasthenia gravis by regulating T cell subsets, inhibiting PI3K-AKT pathway and modulating gut microbiota and metabolites.

BACKGROUND: Ginsenoside Rg1 (GRg1), a bioactive saponin derived from ginseng, exhibits diverse pharmacological properties, including anti-inflammatory, antioxidant, and neuroprotective effects. However, its role in myasthenia gravis (MG) remains unexplored. This study investigated the therapeutic potential of GRg1 in experimental autoimmune myasthenia gravis (EAMG). METHODS: The EAMG model was established in Lewis rats using the AChR97-116 peptide. ELISA was employed to determine serum anti-AChR antibody levels in rats, while Flow cytometry or PCR was used to analyze T cell subsets. Gut micro…

Immune-related markers define clinically relevant prognostic subgroups in thymic epithelial tumors.

BACKGROUND: Characterizing the immune landscape of thymic epithelial tumors (TETs) is essential for patient stratification for emerging immu … T subtypes. Epithelial and lymphocytic OX40L expressions were significantly higher in patients with myasthenia gravis (vs. those without this autoimmune disorder; p = 0.004 and p = 0.045, respectively). Importantly, hierarchical clustering identified four distinct TET subgroups based on their combined immune marker expression profiles, which displayed …

Gender differences in Myasthenia Gravis in a neuromuscular reference center in Belgium.

BACKGROUND: It is well known that scientific literature has a significant gender gap, as gender differences were not considered until very recently. Women were often excluded from studies, and even when included, the results are rarely analyzed separately by gender. This study aims to investigate gender-specific differences in patients with Myasthenia Gravis (MG) by segregating data from our cohort of patients in Antwerp (Belgium). METHODS: We analyzed the data of our previously published cohort of 163 patients with MG visited in the Antwerp University Hospital between 2019 and 2021, segregat…
