Treatment strategies for Mycobacterium avium pulmonary disease.

Macrolides are known to play a key role in the treatment of nontuberculous mycobacteria. However, the optimal combination and number of drugs remains uncertain. Nagawawa et al. and Zweijpfenning et al. investigated these questions in two randomised controlled trials exploring treatment regimens for Mycobacterium avium complex (MAC) infections, while Nguyen et al. summarised some of these questions in a meta-analysis. Nagawawa et al. compared an intermittent three-drug regimen to daily therapy, while Zweijpfenning et al. compared rifampicin or clofazimine as adjuncts to ethambutol and a macroli…

Candidate transmission survival genome of Mycobacterium tuberculosis.

Mycobacterium tuberculosis (Mtb), a leading cause of death from infection, completes its life cycle entirely in humans except for transmission through the air. To begin to understand how Mtb survives aerosolization, we mimicked liquid and atmospheric conditions experienced by Mtb before and after exhalation using a model aerosol fluid (MAF) based on the water-soluble, lipidic, and cellular constituents of necrotic tuberculosis lesions. MAF induced drug tolerance in Mtb, remodeled its transcriptome, and protected Mtb from dying in microdroplets desiccating in air. Yet survival was not passive: …

P3MA: A Promising Mycobacteriophage Infecting Mycobacterium abscessus.

BACKGROUND/OBJECTIVES: Mycobacterium abscessus is an opportunistic pathogen causing infections mainly in patients with immunosuppression and chronic pulmonary pathologies. Extended treatment periods are needed to tackle this pathogen, bacterial eradication is rare, and recurrence can take place with time. New alternative treatments are being investigated, such as bacteriophage therapy. This work describes the characterization of the mycobacteriophage P3MA, showing its ability to infect clinical and standard M. abscessus strains. METHODS: Phylogenetic analysis, electron microscopy, growth curv…

Virulence hierarchies within the Mycobacterium tuberculosis complex.

The Mycobacterium tuberculosis complex (MTBC) includes M. tuberculosis (M. tb), the primary cause of human tuberculosis, M. bovis, the classical zoonotic pathogen and cause of bovine tuberculosis, and M. orygis, a recently recognized multihost pathogen. Given that M. tb, M. bovis, and M. orygis pose significant threats to the health of humans and animals, we sought to understand fundamental differences in pathogenicity among these closely related organisms. Building upon historical observations, we conducted a comparative virulence assessment of these pathogens using both bovine and murine inf…

Pleuritis Complicating Pulmonary Infection of Mycobacterium kyorinense.

Mycobacterium kyorinense is a rare mycobacterium. A 58-year-old man presented with sputum production and left-sided chest pain. Thickened left pulmonary cyst and pleural effusion were observed. A culture of the pleural fluid was negative, but the adenosine deaminase level was elevated. The patient was clinically diagnosed with M. kyorinense infection because this mycobacterium was identified in sputum samples on two occasions. The effusion and chest pain improved following treatment with levofloxacin, clarithromycin, and streptomycin. Identification of M. kyorinense is clinically significant b…

Glomerulonephritis during Mycobacterium tuberculosis infection: scoping review.

INTRODUCTION: People with Tuberculosis (TB) infection may present with glomerulonephritis (GN). The range of presentations, renal pathologies, and clinical outcomes are uncertain. Whether clinical features that establish if GN etiology is medication or TB related, and possible benefits of immunosuppression remain uncertain. METHODS: A scoping review was completed, searching MEDLINE, EMBASE, Cochrane Central Register of Controlled Trials, Web of Science and Conference Abstracts from Inception to December, 2023. The study population included patients with TB infection who developed GN and under

Heme alters biofilm formation in Mycobacterium abscessus.

Mycobacterium abscessus (Mabs) is commonly found in the cystic fibrosis (CF) lung. During infection, Mabs can form biofilms in the lung which reduce both the ability of the immune response to clear infection and the effectiveness of antibiotic therapy. In the CF lung, heme and hemoglobin levels are increased and may provide both iron and heme to Mabs cells. In this work, we show that exogenous heme altered Mabs biofilm formation and measured the effects of exogenous heme on protein level and metabolism in Mabs. Our findings suggest that heme impacts iron homeostasis in Mabs and affects other a…

[Severe pulmonary mycobacteriosis caused by Mycobacterium simiae].

We report a case of pulmonary mycobacterial infection due to Mycobacterium simiae followed between 1968 and 1985 and leading to unilateral pulmonary destruction. Formal bacteriological proof was obtained by culturing from the pneumonectomy specimen. None of the usual features of immunosuppression were found. This is the first case suffering from this germ described in this part of the world.

Establishing a competence model in discriminating Mycobacterium species and Mycobacterium abscessus subspecies by using surface-enhanced Raman spectroscopy.

Surface-enhanced Raman spectroscopy (SERS) is a powerful laser-based technique with high sensitivity and rapid detection. The present study aimed to establish a model for distinguishing Mycobacterium abscessus (nontuberculous mycobacteria; NTM) from M. tuberculosis H37Ra (MTB-H37Ra; a nonvirulent strain) based on genomic DNA detection, as well as distinguishing M. abscessus subspecies abscessus (MAB) from M. abscessus subspecies massiliense (MMAS). OnSpec-Prime SERS chips and a portable Raman spectrometer device were used. Machine learning approaches, including linear discriminant analysis (LD…

Mycobacterium haemophilum infection with cutaneous involvement: two case reports and an updated literature review: Mycobacterium haemophilum skin infection.

Mycobacterium haemophilum (MH) is a slow-growing, non-tuberculous Mycobacterium that most commonly causes infections in immunocompromised patients. The skin is the most prevalent site of infection and can be an isolated presentation or part of a disseminated disease. Herein, we reported a case of isolated MH infection of the hand and a case of disseminated MH infection with multiple skin lesions. In addition, other MH cases with cutaneous involvement over the last 10 years, from 2011-2022, were reviewed and analyzed. Among the 79 included cases, the common skin findings in MH infections includ…

TNFRp75-dependent immune regulation of alveolar macrophages and neutrophils during early Mycobacterium tuberculosis and Mycobacterium bovis BCG infection.

TNF signalling through TNFRp55 and TNFRp75, and receptor shedding is important for immune activation and regulation. TNFRp75 deficiency leads to improved control of Mycobacterium tuberculosis (M. tuberculosis) infection, but the effects of early innate immune events in this process are unclear. We investigated the role of TNFRp75 on cell activation and apoptosis of alveolar macrophages and neutrophils during M. tuberculosis and M. bovis BCG infection. We found increased microbicidal activity against M. tuberculosis occurred independently of IFNy and NO generation, and displayed an inverse corr…

Nationwide analysis of Mycobacterium chimaera and Mycobacterium intracellulare isolates: Frequency, clinical importance, and molecular and phenotypic resistance profiles.

The burden of tuberculosis (TB) in Slovenia is low due to good ongoing preventive measures. However, analysis of data obtained from the Registry for Tuberculosis and National Reference Laboratory for Mycobacteria at University Clinic of Respiratory and Allergic Diseases Golnik from January 2000 to December 2017 revealed that the number of nontuberculous mycobacterial (NTM) isolates is increasing. A group of slowly growing NTM and a common cause of NTM disease among humans is the Mycobacterium avium complex, the taxonomy of which is rapidly changing. M. intracellulare and M. chimaera are part o…

Protection by novel vaccine candidates, Mycobacterium tuberculosis ΔmosR and ΔechA7, against challenge with a Mycobacterium tuberculosis Beijing strain.

Mycobacterium tuberculosis, the etiological agent of tuberculosis (TB), infects over two billion people, claiming around 1.5 million lives annually. The only vaccine approved for clinical use against this disease is the Bacillus Calmette-Guérin (BCG) vaccine. Unfortunately, BCG has limited efficacy against the adult, pulmonary form of tuberculosis. This vaccine was developed from M. bovis with antigen expression and host specificity that differ from M. tuberculosis. To address these problems, we have designed two novel, live attenuated vaccine (LAV) candidates on an M. tuberculosis background:…

Clinical and prognostic importance of serotyping Mycobacterium avium-Mycobacterium intracellulare complex isolates in human immunodeficiency virus-negative patients.

We studied whether the serotypes of Mycobacterium avium-Mycobacterium intracellulare complex (MAC) isolates determine the prognosis for pulmonary MAC disease. We prospectively monitored a cohort of 68 patients with pulmonary MAC disease for whom the serotype-specific glycopeptidolipids in isolates were identified using thin-layer chromatography and fast atom bombardment mass-spectrometry in 1990 and 1995. Serovar 4 Mycobacterium avium was detected in 40/68 patients (58.8%). Other serotypes were serotypes 1 (five cases), 6 (three cases), 8 (seven cases), 9 (three cases), 14 (four cases), and 16…

Taxonomic differences between Mycobacterium avium and Mycobacterium intracellulare elucidated in man by skin tests with three new tuberculins.

New tuberculins called Aviumins A, B and C prepared from M. avium, its proposed subspecies M. avium brunense and M. intracellulare respectively have been skin tested on school children in six countries as a part of larger studies. The results show that the reagents are recognised separately in most cases by the human immune system. Although there is some significant interdependence in sensitization to these reagents, the frequency of meeting the organisms must be very high since 33% of children with a mean age of 8 years produced positive responses to Aviumin A and 40% of them produced positiv
