The authors evaluated the incidence of acute myocardial infarction (AMI) in a group of 355 renal graft recipients operated on at the Institute for Clinical and Experimental Medicine between 1976 and 1984. In the present review the world literature on pregnancy complicated by myocardial infarction is summarized, and two additional cases are presented. It is apparent that the majority of pregnant women who have died after myocardial infarction did so at the time of initial infarction, and maternal mortality was greatest if the infarction was late in pregnancy. Moreover, delivery within two weeks of infarction was associated with increased mortality as was reinfarction during labor. To achieve reperfusion early, an intravenous bolus of 2 million units of urokinase was administered in 50 patients with transmural acute myocardial infarction (AMI) 1.8 +/- 2.5 hours after the onset of symptoms. Three patients with untreated hyperthyroidism at the time of admission are reported. The incidence of non-fatal myocardial infarction among 4830 vasectomised men was 0.9 cases per 1000 man-years during 24 420 man-years of observation. This was slightly lower than the rate in 24 150 non-vasectomised men, matched with a vasectomised man for calendar year of birth and duration of observation. Review of medical records for a matched sample of study subjects indicated no measurable confounding by important cardiac risk factors.

A 40-year-old joiner collapsed while windsurfing. After hospitalization the ECG showed a posterior wall infarction (stage 1) of which the patient shortly died. Autopsy revealed a fresh and a rather older posterior wall infarction, otherwise the coronary artery system was unremarkable. We obtained information on 107 women younger than 46 years who were discharged from a hospital with a diagnosis of acute myocardial infarction. In the series 26 women were otherwise apparently healthy and potentially childbearing. Among these 26 women, 20 (77%) were taking oral contraceptives just prior to admission, and one was taking conjugated estrogens. A 62-year-old patient suffered an acute anterior wall infarction as documented by clinical, electrocardiographic and enzymatic evidence. His ECG returned to a near normal pattern on the 5th day. The next day the patient died suddenly. Plasma calcium concentrations were measured in 18 patients suffering from acute myocardial infarction. Estimations were performed for several days after admission to hospital and a fall in plasma calcium was observed in 13 cases. Plasma albumin levels also decreased over this period, and correction of plasma calcium values to a fixed albumin level removed the apparent tendency to hypocalcaemia in these patients. Three patients with myocardial ischaemia showed similar decreases in plasma calcium and albumin. The observed changes are probably related to confinement to bed.

Serial precordial mapping was done in 30 patients with acute anterior myocardial infarction, 27 transmural and 3 subendorcardial. The sum of ST elevations in the 48 lead map was designated as sigma ST. Normal sigma ST was calculated as 27.1+/-3.1 in males and 14.3+/-2.8 in females. Tricuspid regurgitation developed in two patients after inferior wall myocardial infarction. Neither patient had preexisting valvular heart disease or evidence of endocarditis, and neither had suffered chest trauma. 30 diabetic patients with myocardial infarction were compared with 30 non-diabetic myocardial infarction patients during their hospitalization in an Intensive Care Unit, to observe possible differences at the onset and throughout the course of the infarction syndrome. Myocardial infarction in diabetic cases has an oligosymptomatic onset, often painless, which evolves towards cardiocirculatory decompensation. The frequence of arrhythmias is higher, especially in serious or mortal arrhythmias. Possible pathogenetic reasons for this are briefly discussed.
