Laparoscopic Heller Myotomy With EGD and Balloon Dilation: A Durable Solution for Achalasia.

BACKGROUND: Laparoscopic Heller myotomy (LHM) and esophageal balloon dilation (BD) are the two mainstays of achalasia treatment-this study examines the outcomes when they are performed simultaneously without fundoplication. METHODS: All patients undergoing LHM&BD were reviewed for demographic and procedural data, and to see if additional procedures for achalasia had been performed. Patients were surveyed using the Eckardt score and the GERD quality-of-life score (GERD-HRQL) to assess the durability of repair. RESULTS: From 2013-2020, 66 patients underwent LHM&BD. There were no esophageal per…

Usefulness of EndoFLIP in Diverticular Peroral Endoscopic Myotomy for Symptomatic Epiphrenic Diverticulum.

BACKGROUND/AIMS: Diverticular peroral endoscopic myotomy (D-POEM) is known to be a safe and feasible technique for managing diverticular diseases of the esophagus. In this study, we aim to report our experience with D-POEM and to investigate the usefulness of endoscopic functional luminal imaging probe (EndoFLIP) in determining the need for cardiomyotomy with septotomy for symptomatic epiphrenic diverticulum. METHODS: Consecutive patients who underwent D-POEM for symptomatic epiphrenic diverticulum between September 2019 and September 2021 were eligible for this study. EndoFLIP and high-resol…

Prevalence and impact of opioid use in patients undergoing peroral endoscopic myotomy.

BACKGROUND AND AIMS: Chronic narcotic use may cause opioid-induced esophageal dysfunction and associated type III achalasia, hypercontractile esophagus (HE), diffuse esophageal spasm (DES), and esophagogastric junction outflow obstruction (EGJOO). The frequency of opioid use and its impact on peroral endoscopic myotomy (POEM) in these patients is unknown. METHODS: Consecutive patients between April 2017 and September 2021 who underwent POEM for type III achalasia, EGJOO, DES, or HE with ≥6 months follow-up were identified. Baseline evaluation was done with EGD, esophageal high-resolution mano…

Evaluation of tongue mobility following release operation of ankyloglossia with genioglossus myotomy.

Ankyloglossia is a congenital anomaly characterized by a short lingual frenulum and severe form needs release surgery. Our goal in this study was to confirm the long-term safety and efficacy of Z-plasty with genioglossus myotomy compared with traditional Z-plasty alone and to develop a proper measurement tool to validate the postoperative results of release operations for ankyloglossia. Patients were divided in 2 groups, release with Z-plasty only group (group 1) and Z-plasty combined with genioglossus myotomy group (group 2) In group 2, the release of central longitudinal fiber of genioglossu…

Diagnosis of congenital esophageal stenosis in adults and treatment with peroral endoscopic myotomy.

BACKGROUND: Congenital esophageal stenosis (CES) in adults is a rare disorder that can present as achalasia, particularly in the distal esophagus. We describe the salient features of CES in adults and identify the feasibility and short-term outcomes of peroral endoscopic myotomy (POEM) for CES. METHODS: In this retrospective, single-center case series, we included 6 patients with a "misdiagnosis" of achalasia established elsewhere, ultimately diagnosed with CES and referred to our institution for POEM. Symptom improvement (clinical success rate), defined as an Eckardt Symptom Score (ESS) of <…

Combined Anterior Osteophytectomy and Cricopharyngeal Myotomy for Treatment of DISH-Associated Dysphagia.

STUDY DESIGN: Retrospective observational case series. OBJECTIVE: To assess the outcome of patients with diffuse idiopathic skeletal hyperostosis (DISH) with dysphagia who underwent cricopharyngeal myotomy (CPM) in conjunction with anterior osteophytectomy (OP). METHODS: This is a retrospective observational study of 9 patients that received combined intervention by neurosurgeons and otolaryngologists. Inclusion criteria for surgery consisted of patients who failed to respond to conservative treatments for dysphagia and had evidence of both upper esophageal dysfunction and osteophyte compres…

Gastric peroral endoscopic myotomy for gastroparesis: A systematic review of efficacy and safety.

INTRODUCTION: To assess the efficacy and safety of gastric peroral endoscopic myotomy for the treatment of gastroparesis. METHODS: PubMed, Embase, Cochrane Library and Web of Science databases were searched from their earliest records to May 2018. The evaluation of clinical efficacy and safety was based on gastric emptying scintigraphy normalization, the improvement in clinical symptoms and adverse event rate. R 3.5.0 software was used to calculate the pooled estimate rates by meta-analysis. The improvement rate of the Gastroparesis Cardinal Symptom Index score was analyzed at different follo…

Risk factors for gas-related complications of peroral endoscopic myotomy in achalasia.

BACKGROUND: Peroral endoscopic myotomy (POEM) is a novel endoscopic technique for the treatment of achalasia. However, there are POEM-associated complications, the most common of which being gas-related. The aim of the current study was to determine the occurrence of and risk factors for gas-related complications of POEM in patients with achalasia. METHODS: Retrospective analyses were performed on the clinical data of 216 achalasia patients receiving POEM at our hospital during the period from August 2011 to November 2013. Univariate and multivariate analyses were conducted to look for potent…

Quality of life comparing dor and toupet after heller myotomy for achalasia.

BACKGROUND: Laparoscopic Heller cardiomyotomy (LHC) is standard therapy for achalasia. Traditionally, an antireflux procedure has accompanied the myotomy. This study was undertaken to compare quality-of-life outcomes between patients undergoing myotomy with Toupet versus Dor fundoplication. In addition, we investigated overall patient satisfaction after LHC in the treatment of achalasia. METHODS: One hundred thirty-five patients who underwent LHC over a 13-year period were identified for inclusion. Symptoms queried included dysphagia, heartburn, and bloating using the Gastroesophageal Reflux …

Myectomy-myotomy for long segment Hirschsprung's disease in a patient with Haddad syndrome.

Hirschsprung's disease is a rare entity with an incidence of 1 in 5000 live born infants. Long segment Hirschsprung's disease occurs in approximately 5% to 10% of this patient population and is defined as a transition zone proximal to the sigmoid colon (Bodian M, Carter CO, Ward BC. Hirschsprung's disease. Lancet. 1951;1:302-309). The association of congenital central hypoventilation syndrome (also known as Ondine's curse) and Hirschsprung's disease is termed Haddad syndrome.

Feeding difficulties in a neonate with primary cricopharyngeal achalasia treated by cricopharyngeal myotomy.

Primary cricopharyngeal achalasia resulting from failure of relaxation of the cricopharyngeus muscle is a rare but significant cause of dysphagia in newborns. Symptoms may be nonspecific. Although surgical management with cricopharyngeal myotomy has been employed successfully for the treatment of this disorder, swallowing difficulties may persist postoperatively, probably due to association with other esophageal abnormalities. Alternative methods of feeding may be required to ensure adequate nutrient intake. We report the case of a full-term, 2-day-old neonate with this rare clinical condition…

Heller myotomy for achalasia: quality of life comparison of laparoscopic and open approaches.

BACKGROUND: Achalasia is a relatively rare disorder with a variety of treatment options. Although laparoscopic Heller myotomy has become the surgical treatment of choice, little data exist on the overall quality of life of patients undergoing this technique versus standard open approaches. METHODS: We prospectively evaluated all patients surgically treated for achalasia by a single surgeon. Laparoscopic Heller myotomy consisted of a long (> or = 6 cm) esophageal cardiomyotomy extending at least 2 cm onto the gastric cardia, with a concomitant Dor fundoplication. Patients were evaluated preope…
