Experimental Muscle Pain Impairs the Synergistic Modular Control of Neck Muscles.

A motor task can be performed via different patterns of muscle activation that show regularities that can be factorized in combinations of a reduced number of muscle groupings (also referred to as motor modules, or muscle synergies). In this study we evaluate whether an acute noxious stimulus induces a change in the way motor modules are combined to generate movement by neck muscles. The neck region was selected as it is a region with potentially high muscular redundancy. We used the motor modules framework to assess the redistribution of muscular activity of 12 muscles (6 per side) in the nec

Cervical Spondylotic Myelopathy presenting as mechanical neck pain: a case report.

Cervical Spondylotic Myelopathy (CSM) is the most common type of myelopathy in adults over 55 years of age. In the early stages symptoms may include local neck pain and stiffness that might mimic the presentation of non-specific mechanical neck pain (NSMNP). The patient was a 79 year old male, who complained of eight weeks of neck pain. He had been referred for physiotherapy by his family physician with a diagnosis of NSMNP. Initial presentation was consistent with the referral, but further assessment by the physiotherapist revealed findings suggestive of CSM. He was referred for an urgent cer…

Conservative approach to tardive dyskinesia-induced neck and upper back pain.

The management of schizophrenia typically involves the use of antipsychotics (neuroleptics). Use of such medications, however, can result in tardive dyskinesia, or the involuntary contracture of muscles, and associated symptomatic somatic dysfunction. The authors present a case of a 29-year-old woman who presented to a family medicine clinic for ongoing management of schizophrenia with noticeable tardive dyskinesia and complaints of neck and upper back pain. Conventional management of tardive dyskinesia involves either a change in or reduction of the offending antipsychotic. In the present cas

Acute severe neck pain and dysphagia following cervical maneuver: diagnostic approach.

INTRODUCTION: Overlooking an etiologic hypothesis in acute neck pain with dysphagia may lead to misdiagnosis. CASE REPORT: A 51-year-old man who had received cervical manipulation came to the emergency unit with evolutive acute neck pain, cervical spine stiffness and odynophagia, without fever or other signs of identified pathology. Cervical X-ray and CT angiography of the supra-aortic vessels ruled out traumatic etiology (fracture or arterial dissection) and revealed an accessory bone, orienting diagnosis toward retropharyngeal abscess, which was, however, belied by endoscopy performed under…

Predictors of neck pain after motor vehicle collisions: a prospective survey.

PURPOSE: To identify possible predictors (psychosocial) of neck pain in patients with acute injuries following motor vehicle trauma. METHODS: 221 male and 85 female patients aged 18 to 85 (mean, 38) years who had acute fractures of the humerus, radius, ulna, pelvis, femur, patella, tibia, talus or calcaneus following motor vehicle trauma were prospectively recruited. No patient sustained a cervical spine injury. Patients sustaining minor trauma were excluded. As putative predictors of neck pain, data relating to demographics, injury, socio-economic status, and compensation were collected at t…

Neck and shoulder pain: differentiating cervical spine pathology from shoulder pathology.

Problems of the cervical spine and shoulder frequently have very similar presentations and can be difficult to differentiate. However, with a careful history, physical exam, imaging studies, and judicious use of diagnostic injections, the true source of a patient's symptoms can be deciphered and treated. Cervical spondylosis not only causes pain in the neck and shoulder area, but can also cause radiating pain in the arm and forearm that can be confused with rotator cuff pathology, nerve compression in the shoulder area, or brachial neuritis.

March 2002: 28-year-old woman with neck and back pain.

Following a car accident a 28-year-old female, complained of a sharp pain of the anterior and posterior base of the neck on expiration and with exertion. Subsequently, she noticed a feeling of discomfort in her back when lifting her arm above her head. Imaging studies revealed a tumor mass involving the third intercostal nerve on the right side of T2. The differential diagnosis included neurofibroma and neurilemmoma. This was followed annually and five years later an increase in size warranted a transthoracic, transpleural removal en bloc of this lesion. At surgery, a 3 cm soft tissue tumor en

Neck pain due to acute suppurative thyroiditis and thyroglossal duct abscess.

One case of acute suppurative thyroiditis and one case of thyroglossal duct abscess are presented and discussed. Although they may often present similar symptoms and signs, they need to be differentiated from each other and from other causes of painful thyroid-related neck masses to ensure appropriate therapeutic intervention. Techniques and new developments helpful in differential evaluation, including thyroid radioscintigraphy and ultrasonography, are discussed.

Dispositional and situational personal features and acute post-collision head and neck pain: Double mediation of pain catastrophizing and pain sensitivity.

Pain variability can be partially attributed to psycho-cognitive features involved in its processing. However, accumulating research suggests that simple linear correlation between situational and dispositional factors may not be sufficiently explanatory, with some positing a role for mediating influences. In addition, acute pain processing studies generally focus on a post-operative model with less attention provided to post-traumatic injury. As such, this study aimed to investigate a more comprehensive pain processing model that included direct and indirect associations between acute pain in

Do central hypersensitivity and altered pain modulation predict the course of chronic low back and neck pain?

OBJECTIVES: Widespread central hypersensitivity and altered conditioned pain modulation (CPM) have been documented in chronic pain conditions. Information on their prognostic values is limited. This study tested the hypothesis that widespread central hypersensitivity (WCH) and altered CPM, assessed during the chronic phase of low back and neck pain, predict poor outcome. METHODS: A total of 169 consecutive patients with chronic low back or neck pain, referred to the pain clinic during 1 year, were analyzed. Pressure pain tolerance threshold at the second toe and tolerance time during cold pre…

Prevalence of Myofascial Pain Syndrome Among Overhead Athletes With Neck and Shoulder Pain: A Cross-Sectional Study.

BACKGROUND/OBJECTIVE: The occurrence of shoulder or neck pain in overhead sports is nontraumatic because of chronic overuse, biomechanical variability, and repetitive impaired techniques adapted during the performance. Myofascial Pain Syndrome (MPS) is of both acute and chronic types and is caused by repetitive movements, microtrauma to muscle fibres, and faulty postures. MPS can cause pain, decrease the Range of Motion (ROM), and influence the Overhead Athletes' (OA) performance. The present study aimed to estimate the prevalence of MPS among OA with neck and shoulder pain. METHODS: A prospe…

Origin of neck pain and direction of movement influence dynamic cervical joint motion and pressure pain sensitivity.

BACKGROUND: Patients with neck pain normally showed alterations in cervical motion and pressure pain sensitivity. Cervical joints show scattered motions opposite to (anti-directional) the primary motion direction (pro-directional) during dynamic cervical flexion and extension. This study aimed to assess dynamic cervical joint motion and pressure pain sensitivity when pain originated from different cervical muscles which may have clinical relevance in diagnosis of impairments related with neck pain. METHODS: Fluoroscopic video recordings of cervical flexion and extension were collected from fi…

Process evaluation of a participatory ergonomics programme to prevent low back pain and neck pain among workers.

BACKGROUND: Both low back pain (LBP) and neck pain (NP) are major occupational health problems. In the workplace, participatory ergonomics (PE) is frequently used on musculoskeletal disorders. However, evidence on the effectiveness of PE to prevent LBP and NP obtained from randomised controlled trials (RCTs) is scarce. This study evaluates the process of the Stay@Work participatory ergonomics programme, including the perceived implementation of the prioritised ergonomic measures. METHODS: This cluster-RCT was conducted at the departments of four Dutch companies (a railway transportation compa…

Myofascial pain, widespread pressure hypersensitivity, and hyperalgesia in the face, neck, and shoulder regions, in survivors of head and neck cancer.

PURPOSE: Medical treatment for head and neck cancer may induce the presence of inflammation, pain, and dysfunction. The purpose of the current study was to assess the presence of myofascial trigger points (TrPs) and their relationship with widespread pressure hypersensitivity and hyperalgesia in survivors of head and neck cancer (sHNC). METHODS: TrPs and pressure-pain thresholds (PPTs) were quantified in different muscles/joints in the head and neck of 30 sHNC (59.45 ± 13.13 years) and 28 age- and sex-matched controls (58.11 ± 12.67 years). RESULTS: The sHNC had more TrPs in all muscles on t
