Neurological and developmental outcome of neonatal jaundice and sepsis in rural Kenya.

Neonatal jaundice (NJ) and sepsis are common causes of neonatal mortality in sub-Saharan Africa, but little is known about the long-term morbidity in this setting. This study aimed to describe the neurological and developmental sequelae of severe neonatal hyperbilirubinaemia and neonatal sepsis (NS) in a district hospital in rural Kenya. Twenty-three term infants with NJ [total serum bilirubin (TSB) >300 mumol/l] and 24 infants with a history of NS were identified from hospital records. These children were compared to 40 children from the community (CC) without neonatal problems. At ages 18-32…

Reappraisal of C-reactive protein as a screening tool for neonatal sepsis.

AIMS: To assess the usefulness of C-reactive protein (CRP) [either alone or in combination with a full blood examination (FBE) and microbiology of gastric aspirate] in predicting the diagnosis of neonatal sepsis compared with routinely available markers of infection. METHODS: A prospective study of quantitative CRP, FBE, gastric aspirate and surface swab microscopy and culture in predicting neonatal sepsis in a level III perinatal referral hospital was performed. RESULTS: Of 301 episodes evaluated over a 5-month period, there was a 5% rate of major (culture proven) sepsis (3% early onset, <7…

Ankylosis of the temporomandibular joint as a sequela of septic arthritis and neonatal sepsis.

Ankylosis of the temporomandibular joint (TMJ) can be a result of trauma, degenerative changes and infection or of a space-occupying lesion and, when occurring during early childhood, can result in severe functional disability and facial distortion. Septic arthritis of the TMJ is extremely rare in young infants. We report two children with TMJ ankylosis that is believed to have been a consequence of undiagnosed septic arthritis of the TMJ in the neonatal period.

Review of 49 neonates with acquired fungal sepsis: further characterization.

BACKGROUND: Neonatal acquired fungal sepsis (AFS) is a risky condition that warrants every effort for early diagnosis and management. METHODS: We retrospectively reviewed the medical charts of all 4445 neonatal intensive care unit (NICU) admissions in the past 10 years and detected 49 neonates with AFS. We then compared their data with those of 49 matched control neonates who did not have AFS. The following details were collected: gestational, perinatal and neonatal courses; bacterial sepsis; antibacterial therapy; laboratory and imaging investigations; and antifungal therapy and its complica

Increased CD4(+) T cell co-inhibitory immune receptor CEACAM1 in neonatal sepsis and soluble-CEACAM1 in meningococcal sepsis: a role in sepsis-associated immune suppression?

The co-inhibitory immune receptor carcinoembryonic antigen-related cell-adhesion molecule 1 (CEACAM1) and its self-ligand CEACAM1 can suppress T cell function. Suppression of T cell function in sepsis is well documented. Late-onset neonatal sepsis in VLBW-infants was associated with an increased percentage CEACAM1 positive CD4(+) T-cells. Meningococcal septic shock in children was associated with increased serum soluble CEACAM1. In conclusion our data demonstrate increased surface expression of the co-inhibitory immune receptor CEACAM1 in late-onset neonatal sepsis in VLBW-infants, and increas…

Effect of home-based neonatal care and management of sepsis on neonatal mortality: field trial in rural India.

BACKGROUND: Neonatal care is not available to most neonates in developing countries because hospitals are inaccessible and costly. We develo … ture birth or low birthweight, hypothermia, and breast-feeding problems. They diagnosed and treated neonatal sepsis. Assistance by trained traditional birth attendants, health education, and fortnightly supervisory visits were also provided. Other workers recorded all births and deaths in the intervention and the control area (1993-98) to estimate mortality rates …

Colistin therapy and survival outcomes in neonatal carbapenem-resistant gram-negative sepsis.

BACKGROUND: Carbapenem-resistant Gram-negative (CR-GN) sepsis is a growing cause of neonatal mortality, especially in low- and middle-income countries. Colistin remains one of the few effective therapeutic options. METHODS: This retrospective cohort study reviewed hospital files of inborn neonates admitted within six hours of birth to the neonatal intensive care unit (NICU) of Imam Khomeini Hospital, Ahvaz, Iran (March 2019-March 2023), with blood culture-proven CR-GN sepsis. Blood cultures were obtained from peripheral samples and processed in Trypticase Soy Broth; isolates were identified a

Multi-omics insights into immunometabolic dysregulation in neonatal sepsis for precision medicine.

Neonatal sepsis remains a major global health challenge, contributing substantially to morbidity and mortality despite many advances. Conventional diagnostics often fail to capture the disease complexity and immune dysregulation, leading to delayed diagnosis and sub-optimal treatment. Recent advances in multiomics, including genomics, transcriptomics, proteomics, metabolomics and metagenomics are transforming molecular understanding by enabling a precise view of host-pathogen interactions. These approaches also provide critical insights into metainflammation, a state of chronic, low-grade immu…

Neonatal Group B Streptococcus sepsis with negative maternal screening: a case report.

BACKGROUND: Group B Streptococcus (GBS) is a leading cause of early-onset neonatal sepsis (EOS). Intrapartum antibiotic prophylaxis (IAP) based on maternal GBS screening significantly reduces the incidence of neonatal GBS disease. CASE PRESENTATION: We report a term neonate who developed early-onset GBS sepsis and meningitis despite a negative maternal GBS screening result obtained at 39 weeks of gestation. The infant presented with respiratory distress, poor feeding, and hypotonia shortly after birth. Blood culture confirmed GBS bacteremia, and cerebrospinal fluid (CSF) analysis supported co…

Late onset neonatal sepsis: Can plasma gelsolin be a promising diagnostic marker?

Plasma gelsolin (pGSN) correlates with clinical improvement in septic patients. We aimed to investigate pGSN levels as a diagnostic and prognostic marker of neonatal late-onset-sepsis (LOS). A case-control study was done on 184 neonates (92 with LOS and 92 controls). All participants were subjected to detailed history taking, full clinical evaluation, sepsis workup, and pGSN enzyme-linked immunosorbent-assay measurement. We detected significantly lower pGSN level among cases compared to controls (90.63 ± 20.64 vs 451.83 ± 209.59). It was significantly related to the severity of sepsis and mort

How to assess early-onset neonatal sepsis? Comparison of three detection strategies.

INTRODUCTION: Early-onset neonatal sepsis (EONS) can cause significant morbidity and mortality, especially if it is not detected early. Given the decrease in its incidence in the past few decades, it is important to find a balance between reducing the use of diagnostic tests and continuing to detect affected patients. We compared 3 detection strategies in patients with risk factors (RFs) for infection: laboratory screening (S1), the Neonatal Sepsis Risk Calculator (S2) and clinical observation (S3). PATIENTS AND METHODS: Retrospective observational study in neonates born at 34 weeks of gestat…

TRAINED IMMUNITY: A POTENTIAL APPROACH FOR IMPROVING HOST IMMUNITY IN NEONATAL SEPSIS.

Sepsis, a dysregulated host immune response to infection, is one of the leading causes of neonatal mortality worldwide. Improved understanding of the perinatal immune system is critical to improve therapies to both term and preterm neonates at increased risk of sepsis. Our narrative outlines the known and unknown aspects of the human immune system through both the immune tolerant in utero period and the rapidly changing antigen-rich period after birth. We will highlight the key differences in innate and adaptive immunity noted through these developmental stages and how the unique immune phenot

The Multi-Component Causes of Late Neonatal Sepsis-Can We Regulate Them?

Elucidating the mechanisms of bacterial translocation is crucial for the prevention and treatment of neonatal sepsis. In the present study, we aimed to evaluate the potential of lactoferrin to inhibit the development of late-onset blood infection in neonates. Our investigation evaluates the role of key stress factors leading to the translocation of intestinal bacteria into the bloodstream and, consequently, the development of life-threatening sepsis. Three stress factors, namely weaning, intraperitoneal administration of Gram-positive cocci and oral intake of Gram-negative rods, were found to …
