A systematic review of impairment focussed technology in neurology.

OBJECTIVE: We provide an overview of some biomedical technologies able to relieve everyday impairments in neurological patients. METHODS: Two literature searches from 2009 to 2020 were conducted using PubMed, Embase, Cinahl and Scopus. The studies meeting the criteria for eligibility constituted 224 of the 6257 identified studies. RESULTS: The first literature search resulted in the identification of 53 different neurological impairments. The following impairments were selected as the most general: six motor (walking/gait abnormality, paralysis/paresis, hypertonia, dystonia, tremor and ataxi

Development, Implementation, and Use of a Neurology Therapeutics Committee.

Innovative therapeutics are transforming care of children with previously untreatable neurological disorders. However, there are challenges … party payers in approving therapeutic use based on safety and efficacy. We report our creation of a Neurology Therapeutics Committee for pediatric patients. We review the committee's mechanisms, describe its use and report outcomes, and suggest the Neurology Therapeutics Committee's broader applicability.

[Encephalitis lethargica. The epidemic at the dawn of neurology].

Lethargic encephalitis is a neurological illness that shows a wide range of symptoms and signs, including neurological and psychiatric spectrum. It presented in an epidemic way, following influaenza relapses. The last relapse started at the beginning of 20th century and it was deeply described by Constantin von Economo. The illness described first in Europe and North America, was described in many others countries including Chile. There were beautiful descriptions by Chilean physicians like Lea-Plaza, Tello, Iturra and Cienfuegos. Their works showed the complexity of the illness like European

Clinical Neurology and Epidemiology of the Major Neurodegenerative Diseases.

Neurodegenerative diseases are a common cause of morbidity and cognitive impairment in older adults. Most clinicians who care for the elderly are not trained to diagnose these conditions, perhaps other than typical Alzheimer's disease (AD). Each of these disorders has varied epidemiology, clinical symptomatology, laboratory and neuroimaging features, neuropathology, and management. Thus, it is important that clinicians be able to differentiate and diagnose these conditions accurately. This review summarizes and highlights clinical aspects of several of the most commonly encountered neurodegene

Enhancing diversity in academic neurology: From agnosia to action.

Although recent years have seen a rapid increase in the diversity of patient populations and of society in general, individuals who belong to historically disadvantaged groups continue to struggle to achieve fuller representation and success in academic medicine. This is particularly true in academic neurology, where faculty from racial and ethnic minority groups are grossly under-represented. Raising awareness of this problem in the field is an important first step toward developing coordinated, strategic approaches to enhancing diversity in neurology. Therefore, the aims of this brief articl…

[American Academy of Neurology, Washington, 18-25 April 2015].

CEREBROVASCULAR DISEASES: The benefit of the thrombectomy using stents retrievers in the acute stroke phase is now demonstrated when there is a proximal occlusion of an intracranial artery, whatever its mechanism. The place of the anticoagulants in the management of cervical artery dissections remains uncertain, while the benefit of the blood pressure control in the secondary prevention of deep and lobar intracerebral hemorrhages is critical. The development of cardiac MRI, prolonged cardiac monitoring and transcranial doppler seems to improve the diagnosis of cardio-embolic sources of stroke.

[On the borders of the neurology and the rheumatology].

The links between the rheumatology and the neurology, are ancient and established of one intricacy of pathologies and symptoms. It is not a question here of approaching the inflammatory myositis nor the neurological, essentially central signs, during the inflammatory diseases still called systematic as the lupus or the syndrome of Gougerot-Sjögren, neither compression occurring during rheumatoid arthritis and ankylosing spondylarthritis. It is not either a question of reviewing the compression of osseous origin, as for example the narrow cervical spine, the narrow lumbar spine or the basilaire…

[Spreading of protein misfolding: A new paradigm in neurology].

Protein misfolding and spreading ("transconformation") are being better understood. Described in Prions diseases, this new paradigm in the field of neurodegenerative disorders and brain aging also implies sporadic inclusion myositis, type 2 diabetes, some cancers, sickle cell disease... Misfolding is transmitted from a protein or peptide to a normally folded one. Often associated with a stress of the endoplasmic reticulum, it may spread along the neurites, following anterograde or retrograde axonal transport. In the central nervous system, it occurs in a few cells and there is invasion of adja

[Tourette syndrome--in the borderland between neurology and psychiatry].

Tourette syndrome is a hereditary tic disorder. The symptoms consist of compulsory movements and vocalizations. Stress has an aggravating effect on tics. Unfortunately tics are easily mistaken for oppositional and defiant behavior. Tourette syndrome is most frequently seen with psychiatric comorbidity. Tics are best treated with low arousal techniques, which may be supplemented with pharmacotherapy.

Current role of melatonin in pediatric neurology: clinical recommendations.

BACKGROUND/PURPOSE: Melatonin, an indoleamine secreted by the pineal gland, plays a key role in regulating circadian rhythm. It has chronobiotic, antioxidant, anti-inflammatory and free radical scavenging properties. METHODS: A conference in Rome in 2014 aimed to establish consensus on the roles of melatonin in children and on treatment guidelines. RESULTS AND CONCLUSION: The best evidence for efficacy is in sleep onset insomnia and delayed sleep phase syndrome. It is most effective when administered 3-5 h before physiological dim light melatonin onset. There is no evidence that extended-rel

Pathophysiology of immune-mediated demyelinating neuropathies--Part II: Neurology.

In the second part of this review we deal with the clinical aspects of immune-mediated demyelinating neuropathies. We describe the relationship between pathophysiology and symptoms and discuss the pathophysiology of specific disease entities, including Guillain-Barré syndrome, chronic inflammatory demyelinating polyneuropathy, multifocal motor neuropathy, anti-myelin-associated glycoprotein neuropathy, and POEMS syndrome.

Alpha-fetoprotein, a fascinating protein and biomarker in neurology.

Alpha-fetoprotein (AFP) is present in fetal serum in concentrations up to 5,000,000 μg/l. After birth, AFP gene expression is turned down with a subsequent fall of the serum concentrations of this albumin-like protein to 'adult values' of circa 0.5-15 μg/l from the age of 2 years onwards. Irrespective of its assumed important functions, individuals with AFP deficiency appear fully healthy. The other way around, the presence of AFP in the circulation after the first years of life doesn't seem to harm, since individuals with 'hereditary persistence of AFP' are also without clinical abnormalities

Moritz Heinrich Romberg (1795-1873): Early founder of neurology.

Moritz Heinrich Romberg (1795-1873) began his pursuit of neurology in 1820 by translating into German Andrew Marshall's The Morbid Anatomy of the Brain. In 1830, Romberg was hired as Privatdozent of special pathology and therapy in the Charité, the University Hospital of Berlin. He quickly rose to director of the royal clinic in 1845, at which time he wrote Lehrbuch der Nervenkrankheiten des Menschen, a text generally regarded as the first formal treatise on nervous diseases. He identified the role of proprioception in tabes dorsalis, and became the first neurologist to describe the typical pu…

Translational research in neurology and neuroscience 2011: movement disorders.

We provide an update on the state of translational research in movement disorders, using examples of Huntington disease, Parkinson disease, and dystonia. While substantial progress in our understanding of these disorders has been achieved, development of neuroprotective treatments remains an unrealized goal. Here we highlight some of the emerging research areas that show the most promise for translational research in Huntington disease, Parkinson disease, and dystonia. Aetiology and pathogenesis, biomarker directions, and causal treatment opportunities are discussed for each disease, followed

The neurology of carbonic anhydrase type II deficiency syndrome.

Carbonic anhydrase type II deficiency syndrome is an uncommon autosomal recessive disease with cardinal features including osteopetrosis, renal tubular acidosis and brain calcifications. We describe the neurological, neuro-ophthalmological and neuroradiological features of 23 individuals (10 males, 13 females; ages at final examination 2-29 years) from 10 unrelated consanguineous families with carbonic anhydrase type II deficiency syndrome due to homozygous intron 2 splice site mutation (the 'Arabic mutation'). All patients had osteopetrosis, renal tubular acidosis, developmental delay, short
