Interest of monitoring diaphragmatic electrical activity in the pediatric intensive care unit.

The monitoring of electrical activity of the diaphragm (EAdi) is a new minimally invasive bedside technology that was developed for the neurally adjusted ventilatory assist (NAVA) mode of ventilation. In addition to its role in NAVA ventilation, this technology provides the clinician with previously unavailable and essential information on diaphragm activity. In this paper, we review the clinical interests of EAdi in the pediatric intensive care setting. Firstly, the monitoring of EAdi allows the clinician to tailor the ventilatory settings on an individual basis, avoiding frequent overassista

Anaesthesia for correction of scoliosis in children.

Surgical correction of spinal deformities in children presents a challenge to the anaesthetist because of the extensive nature of the surgery, the co-morbidities of the patients and the constraints on anaesthetic techniques of intraoperative neurophysiological monitoring of the spinal cord. Adolescent idiopathic scoliosis is the most common deformity. Patients with scoliosis secondary to neuromuscular conditions are at greatest risk of perioperative problems, particularly excessive blood loss and respiratory failure. The risk of spinal cord damage can be decreased by the use of intraoperative

The RECITE Study: A Canadian Prospective, Multicenter Study of the Incidence and Severity of Residual Neuromuscular Blockade.

BACKGROUND: Postoperative residual neuromuscular blockade (NMB), defined as a train-of-four (TOF) ratio of <0.9, is an established risk fact … CLUSIONS: Residual paralysis is common at tracheal extubation and PACU arrival, despite qualitative neuromuscular monitoring and the use of neostigmine. More effective detection and management of NMB is needed to reduce the risks associated with residual NMB.

CSF Diagnostics: A Potentially Valuable Tool in Neurodegenerative and Inflammatory Disorders Involving Motor Neurons: A Review.

Cerebrospinal fluid (CSF) diagnostics has emerged as a valid tool for a variety of neurological diseases. However, CSF diagnostics has been playing a subordinate role in the diagnosis of many neurological conditions. Thus, in the multitude of neuromuscular diseases in which motor neurons are affected, a CSF sample is rarely taken routinely. However, CSF diagnostics has the potential to specify the diagnosis and monitor the treatment of neuromuscular disorders. In this review, we therefore focused on a variety of neuromuscular diseases, among them amyotrophic lateral sclerosis (ALS), peripheral

Electrophysiological tests in intensive care.

Neuromuscular complications encountered in intensive care can be due to peripheral nerves (polyneuropathies), muscles (myopathies), neuromuscular end-plate (transmission deficit) or a combination of these. Neurophysiopathological tests that enable us to diagnose and maybe differentiate between these causes are essentially electroneurographs and electromyographs. These assess, either directly or indirectly, the integrity of the peripheral system, both sensory (centripetal afferent pathways) and motor (lower motor neurones, neuromuscular junction, muscle membrane and contractile substrate). Elec

[Molecular therapies in childhood neuromuscular disorders-definite hope versus unknown pitfalls].

Spinal muscular atrophy and muscular dystrophy Duchenne belong to the group of rare neuromuscular diseases manifesting in early childhood. Therapeutic options for some of these rare monogenic diseases have changed significantly in recent years. Molecular therapies such as direct gene transfer or alternative processing of the disease-specific gene play an important role in this transformation. In particular, the course of 5q-associated spinal muscle atrophy has changed significantly due to the availability of such causal therapies, while the results of ongoing studies are still pending for most

Congenital Myasthenic Syndrome in Pregnancy: A Case Report and Literature Review on Peripartum Management Strategy.

Congenital myasthenic syndromes (CMS) are a group of rare inherited neuromuscular disorders caused by defects in neuromuscular transmission. Unlike autoimmune myasthenia gravis, CMS typically presents in childhood with variable severity and symptoms, and does not involve autoantibodies. This report presents the management of a primigravida diagnosed with CMS who was generally stable on pyridostigmine therapy but experienced a severe exacerbation during pregnancy. The diagnosis of CMS was made on clinical grounds, but the genetic testing for confirmation of the exact mutation was not available.

Validity and Reliability of the Neuromuscular Gross Motor Outcome.

BACKGROUND: Approved treatments in spinal muscular atrophy (SMA) have resulted in unprecedented gains for many individuals. Use of available outcomes, typically developed for a specific type of SMA, do not cover the range of progression, often resulting in a battery of functional testing being completed at visits. Our objective was to validate the Neuromuscular Gross Motor Outcome (GRO) as a tool to quantify function in SMA across the span of abilities. METHODS: Patients with genetically confirmed SMA completed functional testing at each visit including the Neuromuscular GRO and other appropr

Zebrafish as a Model Organism for Research in Rare Genetic Neuromuscular Diseases.

The zebrafish (Danio rerio) has become one of the most popular and valuable model organisms for studying rare neuromuscular diseases. Its unique characteristics, including the high number of offspring produced with each mating, transparent eggs, rapid development, and genetic similarity to humans, make this small vertebrate ideal for investigating complex and rare disorders affecting the skeletal muscle, such as Duchenne Muscular Dystrophy (DMD), Limb Girdle Muscular Dystrophies (LGMDs), and Brody Myopathy (BM). Various zebrafish models, both natural mutants and genetically engineered strains,

Artefacts in the amplitude-integrated EEG background pattern of a full-term asphyxiated neonate caused by diaphragm spasms.

The amplitude-integrated EEG (aEEG) is used to detect neonatal seizures in neonates with asphyxia, and despite limited data-information compared with standard EEG, aEEG is increasingly used as routine monitoring in neonatal intensive care units due to the logistic advantages. In addition, the aEEG background is of prognostic value in these infants. However, aEEG artefacts can lead to an erroneous interpretation of the background pattern. We report a full-term infant with severe perinatal asphyxia with diaphragm spasms that caused a significant alteration in aEEG background pattern. After admin

Feasibility of High-Density Surface Electromyography for the Detection of Neuromuscular Disorders in Children.

INTRODUCTION/AIMS: Diagnosing neuromuscular disorders in children is challenging. Concentric needle electromyography (CNEMG) is the standard for electrophysiological assessments but has limitations in pediatric populations. High-density surface electromyography (HDsEMG) provides a noninvasive technique with superior spatial resolution, enabling the identification and analysis of motor unit (MU) firing dynamics throughout the entire period of MU activity. This study assessed the feasibility of HDsEMG MU decomposition in children and explored parameters that differentiate neuropathy, myopathy, a

Optically pumped magnetometers detect altered maximal muscle activity in neuromuscular disease.

Optically pumped magnetometers (OPM) are quantum sensors that enable the contactless, non-invasive measurement of biomagnetic muscle signals, i.e., magnetomyography (MMG). Due to the contactless recording, OPM-MMG might be preferable to standard electromyography (EMG) for patients with neuromuscular diseases, particularly when repetitive recordings for diagnostic and therapeutic monitoring are mandatory. OPM-MMG studies have focused on recording physiological muscle activity in healthy individuals, whereas research on neuromuscular patients with pathological altered muscle activity is non-exis

Silk Fabric Functionalized by Nanosilver Enabling the Wearable Sensing for Biomechanics and Biomolecules.

Integrating biomechanical and biomolecular sensing mechanisms into wearable devices is a formidable challenge and key to acquiring personalized health management. To address this, we have developed an innovative multifunctional sensor enabled by plasma functionalized silk fabric, which possesses multimodal sensing capabilities for biomechanics and biomolecules. A seed-mediated in situ growth method was employed to coat silver nanoparticles (AgNPs) onto silk fibers, resulting in silk fibers functionalized with AgNPs (SFs@Ag) that exhibit both piezoresistive response and localized surface plasmo

Altered Cisatracurium Pharmacokinetics and Pharmacodynamics in Patients with Congenital Heart Defects.

The neuromuscular blocking agent cisatracurium is frequently used adjunctively in anesthesia to facilitate endotracheal intubation and to provide muscle relaxation during surgery. We aimed to determine the pharmacokinetics (PK)/pharmacodynamics (PD) of cisatracurium in patients with congenital heart defects (CHDs), such as ventricular septal defects and atrial septal defects, and to assess the effects of CHDs on the PK/PD profiles of cisatracurium. A modified two-compartment model with drug clearance from both compartments was best fitted to the PK data to determine the PK parameters. The mode

Occupational Therapy in Acute Branch Atheromatous Disease With Frequent Motor Paralysis Fluctuations: A Case Report.

Branch atheromatous disease (BAD) often progresses during the acute phase. During acute rehabilitation, clinicians should prioritize monitoring neurological symptom progression. We treated a patient with right-sided, finger-limited motor paralysis and frequent transient motor paralysis fluctuations due to BAD by implementing early upper-limb occupational therapy while maintaining rigorous risk management. Occupational therapy included standard upper limb functional training, continuous neuromuscular electrical stimulation, and repetitive facilitation exercises. During occupational therapy, blo
