Neuropathological analysis of dementia in a Japanese leprosarium.

In a neuropathological study of consecutive autopsies, prevalence and cause of dementia in a Japanese leprosarium were investigated, where more than 95% of inpatients with a mean age of 70 years are now free from active leprosy. In 10 years (1983-1992), clinically overt dementia at death was 35/136 (25.7%) in the age group over 65 years (mean age 79.4). Autopsy was performed in 85 cases (mean age 81 years), and clinically overt dementia was seen in 25 subjects (29.4%). Neuropathologically, Alzheimer's disease (AD) was seen in 9 cases (10.6%), vascular dementia (VD) in 9 cases (10.6%), mixed ty

Posterior cortical atrophy. Neuropathologic correlations.

OBJECTIVE: A subgroup of patients with progressive dementia has been reported with a marked predominance of symptoms attributed to the dysfunction of the posterior parieto-occipital cortex. These cases have been referred to as posterior cortical atrophy. The objective of this study was to determine whether posterior cortical atrophy is associated with distinct, uniform neuropathologic findings. DESIGN: Three individuals with progressive dementia that began with higher visual dysfunction (posterior cortical atrophy) were followed up to definitive neuropathologic diagnosis. RESULTS: Three sepa

Neuropathologic findings after liver transplantation.

Neuropathologic findings are described in 37 patients autopsied after one or more orthotopic liver transplants. Gross or microscopic lesions were observed in almost all patients, including anoxic-ischemic change, hemorrhages and/or infarcts, and opportunistic infections by fungi (most commonly Aspergillus) and rarely viruses (cytomegalovirus). Central pontine and extra-pontine myelinolysis was commonly observed, and appeared to result from severe multifactorial metabolic abnormalities in the perioperative period. Low-grade (microglial nodule) encephalitis without an obvious pathogen was often

Dystonia--a clinical, neuropathological and therapeutic review.

Dystonia is a syndrome characterized by sustained muscle contractions frequently causing twisting and repetitive movements or abnormal posture. For diagnosis, prognosis and therapy, it is useful to classify dystonia with regard to types of abnormal movements present, their mode of activation and topographical distribution taking into account age of onset, and etiology. The majority of cases are idiopathic, or primary dystonias, while in a minority environmental, structural, or metabolic causes can be identified. Primary dystonias can be familial or sporadic. The most important neurophysiologic

[Neuropathological aspects of normal and abnormal aging].

Several senile changes in the central nervous system in cadavers were examined. The pattern of extension of Alzheimer's neurofibrillary tangles (NFT) and senile plaques (SP) in the olfactory bulbs of 100 specimens was examined during routine autopsy by immunohistochemical staining. NFT were first observed in the anterior olfactory nucleus after the age of 60, and incidence rose with increasing age. Senile plaques were found in the nucleus when there were many SP in the cerebral cortex. Of 25 non-demented amyotrophic lateral sclerosis patients, SP were found in the cerebral cortices of 10, and

[Diagnosis and significance of forensic neuropathological findings].

Among the diseases of the nervous system, Alzheimer-type dementia, head injuries, brain death and fat embolism were discussed on the pathological diagnosis and medico-legal significance. 1. Alzheimer-type dementia 1) There were marked differences in microscopic findings of ATD group and control group of elderly subjects whose intellects have been maintained. The main histological features of ATD were presence of numerous senile plaques and neurofibrillary changes, and both of them were thought to be helpful diagnostic markers. 2) Double stains by GFAP stain and Congo red stain were performed.

Periventricular leukomalacia: ultrasonic and neuropathological correlations.

Ultrasound scans of a preterm neonate (27 weeks gestation) at seven weeks of age showed periventricular echo-free cavities, but these were no longer visible at 15 weeks, three weeks before the infant died. At autopsy, a linear glial scar, extending from the periventricular white-matter into the white axis of the parasagittal gyrus, was found in the area occupied by the periventricular cysts. The larger cavity was reduced to a slit-like excavation in the midst of glial tissue. Unsuspected focal infarcts in the cerebral cortex were also found. This observation demonstrates that transient echo-fr

Brain lesions in alcoholics: neuropathological observations.

Examination of the autopsy material from a city hospital in Oslo gave the following results: 1. The brain weight of male alcoholics was significantly lower than that of age-matched controls in all 10-year age groups from 40 to 69 years. The mean weight reduction was 31 g. 2. Combined morphometric and histologic studies demonstrated atrophy of the anterior vermis in 42 per cent of all male alcoholics under 70 years of age. In cases over 70 years, 33 per cent of the non-alcoholic controls showed atrophy of the anterior vermis. This is assumed to be a consequence of physiologic ageing. The preval

Neuropathological and clinical correlations in Hurler disease.

We report studies on two patients (1 and 2) with Hurler disease. They both had all of the non-neurological features of Hurler disease to a similar and extreme degree and similar signs of brain damage on computed tomography. However, intellectual function was unusually well-preserved in patient 1, but seriously and typically impaired in patient 2. The reason for this discrepancy has been investigated by reference to the neuropathological findings, the results of alpha-L-iduronidase assays using different substrates and comparisons to other cases (patients 3 and 4). We suggest that patient 1 is

Neuropathologic aspects of arthrogryposis multiplex congenita.

Arthrogryposis multiplex congenita is not a specific disorder but rather a symptom complex of congenital joint contractures associated with both neurogenic and myopathic disorders. Pathologic studies, including autopsy evaluation and muscle biopsy alone, were performed on 74 children with features of arthrogryposis. In 69 children (93%), the deformities were found to be neurogenic in origin, while five (7%) had myopathic disorders. The children in these two groups could be subdivided into 17 specific disorders, depending on the site of the pathologic lesions: anterior horn cells, roots, periph

Cerebral cladosporiosis--a neuropathological and microbiological study.

Cerebral cladosporiosis is a rare mycotic infection of the brain. So far only 21 culture-proven cases of cerebral cladosporiosis have been listed in literature (FETTER et al. 1967, FETTER and KLINTWORTH, 1978). In view of the rarity of this mycotic infection and its clinical importance we report two cases of culture-proven cerebral cladosporiosis. They presented as meningoencephalitis with extensive areas of haemorrhagic infarctions of frontoparietal lobes. Both patients were middle-aged farmers from Karnataka State, South India. The clinical, neuropathological and mycological features are pre

A neuropathological and neuropsychological study of Turner's syndrome.

A neuropathological study was made in 2 women with Turner's syndrome. Neuropsychological investigation in one of them correlated with what has previously been found in Turner's syndrome as well as with the localization of the most pronounced neuropathological aberration which was of atherosclerotic nature, most pronounced in the right temporo-parietal area. These findings as well as the findings of acidophile hypoplasia of the pituitary gland are discussed.

Communicating hydrocephalus. Cisternographic and neuropathologic studies.

Chronic communicating hydrocephalus was produced in adult dogs by injection of silastic into the subarachnoid space. Electron microscopy was used to verify the sequence of pathologic changes in the ventricular wall. The pathologic findings were correlated with cisternographic images and measurements of cerebrospinal fluid (CSF) pressure. Early in hydrocephalus, the CSF pressure was increased and cisternograms showed ventricular entry and clearing; the ependyma was stretched and fluid accumulated in subependymal regions. In animals with chronic hydrocephalus, the CSF pressure was normal and cis

Neuropathological features of levodopa-responsive parkinsonism in multiple system atrophy: an autopsy case report and comparative neuropathological study.

BACKGROUND: In typical patients with multiple system atrophy with predominant parkinsonism (MSA-P) levodopa is ineffective. However, there are some of these patients who respond well to levodopa treatment. Levodopa efficacy in MSA-P patients is thought to be related to the degree of putaminal damage, but the pathological causation between the putaminal involvement and levodopa efficacy has not been established in detail. OBJECTIVE: This study aimed to evaluate the neuropathological features of the nigrostriatal dopaminergic system in a "levodopa-responsive" MSA-P patient in comparison with "l

Vascular Cognitive Impairment and Dementia: Clinical Features, Neuropathology, and Biomarkers.

Vascular cognitive impairment and dementia (VCID), ie, cognitive impairment secondary to cerebrovascular disease (CeVD), is the second most common form of dementia after Alzheimer's disease (AD), accounting for 15% to 20% of all cases. CeVD, in fact, contributes to dementia alongside other neuropathologies in up to 75% of dementia cases. CeVD and AD not only frequently co-occur in the brain, but they may also interact, and some VCID risk factors (midlife hypertension and diabetes) also increase AD risk. Because CeVD and cardiovascular disease share risk factors and pathophysiology, the cardiov
