Successful Aflibercept Treatment for Choroidal Neovascularization in a Rare Case of Optic Disc Melanocytoma.

BACKGROUND Optic disc melanocytoma (magnocellular nevus of the optic disc) is a benign congenital pigmented tumor, usually unilateral, asymptomatic, and mostly discovered in routine examinations in adult patients; however, it is associated with choroidal neovascularization. It can be difficult to differentiate it from juxta-papillary choroidal melanoma. Aflibercept is a monoclonal antibody targeting vascular endothelial growth factor A (anti-VEGF-A). This report describes a 54-year-old man with a diagnosis of optic disc melanocytoma complicated by choroidal neovascularization who was treated w

Pointillist nevi.

BACKGROUND: Atypical melanocytic nevi and cutaneous melanoma are often marked by variation in color. However, there are examples of "benign" explanations for irregularities in pigmentation, such as perifollicular hypopigmentation or hyperpigmentation. OBJECTIVE: The purpose of this study was to correlate the clinical and histologic features of 3 unusual melanocytic nevi consisting exclusively of multiple, tiny, dark brown to black dots on a skin-colored background, which we have termed pointillist nevi. METHODS: Histologic examination was performed of the single pointillist nevus from each o

7. Pseudometastatic lesions of the choroid.

A 64-year-old man with a six-month history of visual loss was found to have multiple darkly pigmented lesions scattered throughout the choroid of both eyes. Some lesions, up to several disc diameters in size, were flat, but others were elevated one to two millimeters. The right eye also contained a darkly pigmented ciliary body mass. Impairment of light transmission was striking. Ultrasonography of the elevated lesions demonstrated solid masses with high internal reflectivity. The initial diagnostic differential included the possibility of pigmented metastases from a primary tumor elsewhere, a

Topical sirolimus for the treatment of verrucous epidermal nevus associated with an HRAS mosaic mutation: a case report.

INTRODUCTION: Verrucous epidermal nevus is a benign epidermal hamartoma associated with somatic mosaic mutations, which shows poor response to conventional therapies and has a high risk of recurrence. To date, there are no standardized treatment guidelines for this condition. In recent years, targeted treatments for specific gene mutations have provided new directions for the clinical management of this disease. CASE REPORT: We report a case of a 13-year-old child with verrucous epidermal nevus in whom genetic testing revealed an HRAS mosaic mutation. The child showed poor response to convent

[Blaschkitis in adults].

Many dermatoses may exhibit lesions distributed in a linear fashion: the linearity may be related to anatomical structures (blood or lymph vessels, nerves), to mechanical factors (urticarial dermographism, striae distensae cutis, frictional melanosis) or to some other provocative injuries being themselves linear (for instance Köbner's phenomenon). Some congenital or acquired dermatoses, either inherited or sporadic, have a linear distribution following the embryonic lines described in 1901 by A. Blaschko. Most of them are nevoid skin lesions present at birth or having a later onset: epidermal

Cutaneous melanocytic lesions: selected problem areas.

Cutaneous melanocytic proliferations are diverse morphologically, and their behavioral attributes may be difficult to discern with certainty. As a consequence, diagnostic anatomic pathologists regularly encounter problems in the interpretation of such lesions. This review considers several selected issues in that subject area, including proliferative congenital nevi, architecturally disordered (dysplastic) nevi, morphologic variants of Spitz nevus, atypical lentiginous melanocytic proliferations, nevoid melanoma, diagnostically deceptive histologic variants of melanoma, "epidermotropic" metast

Acral Speckled Lentiginous Nevus.

Speckled lentiginous nevus (SLN), also referred to as nevus spilus, is a common benign melanocytic neoplasm typically occurring as a small, café-au-lait-colored "speckled" patch studded with numerous darkly pigmented macules or papules. Herein, we present a unique case of SLN arising on the sole of the right foot in a young woman, which was reported to be gradually enlarging since a pregnancy two years prior. The lesion was removed by shave biopsy, and histopathology confirmed the diagnosis and ruled out atypical features. While SLN is quite common, the occurrence on acral skin is extremely ra

Biopsy of an anterior episcleral nodule as an aid in managing a ciliary body melanocytic tumor.

PURPOSE: To demonstrate the value of a diagnostic biopsy of a fixed episcleral nodule overlying a uveal mass. METHOD: Clinicopathologic report with immunohistochemical investigations. RESULTS: B-scan ultrasonographic biomicroscopy disclosed in a 67-year-old man an asymptomatic placoid ciliary body tumor measuring 1.28 mm in thickness underlying a poorly pigmented, fixed episcleral nodule 0.56 mm in thickness. Biopsy of the episcleral nodule displayed benign nevus-type spindle cells with small nuclei, punctate nucleoli, no mitoses, and scattered melanophages. Immunohistochemistry demonstrated

Inflammatory juvenile compound conjunctival nevi. A clinicopathological study and literature review.

AIM: The conjunctival nevus affecting children and adolescents is a rare condition and the literature showed only few reports on this issue. The aim of this article is to determine the histopathological features for the correct diagnosis of an inflammatory juvenile compound nevus of the conjunctiva (IJCNC) in order to make the difference between this tumor and other lesions, like conjunctival melanoma or lymphoma, very similar from a gross point of view. This article is a clinical pathological study of two cases of IJCNC with particular histopathological characteristics, who were admitted at t

[Atypical lentiginous nevus: a clinical and histopathologic study of 14 cases].

BACKGROUND: Atypical lentiginous nevus (of the elderly) is a peculiar form of dysplastic nevus. Clinically, this condition can resemble malignant melanoma and histologically, it has a lentiginous pattern with variable degrees of atypia and an absence of dermal nests. These features may lead to an erroneous diagnosis of lentigo maligna melanoma or lentiginous melanoma. MATERIAL AND METHODS: We reviewed 14 cases of atypical lentiginous nevus diagnosed at the dermatology department of Hospital General de Valencia in Valencia, Spain between December 2007 and March 2009. We studied the clinical an

Pigmented lesion with characteristics of malignancy: a case report.

Pigmentations of the oral mucosa include a range of lesions or conditions that manifest as changes in the color of oral tissues; these changes may show melanocytic activity. A melanotic macule is a small, well-circumscribed melanocytic benign lesion. It can occur on the lips and intraorally and ranges in color from brown to black. Microscopically, it is characterized by elevated levels of melanin production by basal melanocytes, which appear normal in terms of number, morphology, and distribution. A 48-year-old woman sought treatment for a pigmented lesion that had been present for 4 months. I

Assessing the Impact of Simulated Color Vision Deficiency on Ophthalmologists' Ability to Differentiate between Choroidal Melanoma and Choroidal Nevus.

Background: Color vision deficiency (CVD) is an often-overlooked issue within the medical community, and its consequences remain insufficiently explored. We aim to evaluate how CVD affects diagnostic accuracy and distinguish between malignant choroidal melanoma and benign choroidal nevus among ophthalmologists. Methods: In this cross-sectional study, we engaged ophthalmologists through a web-based survey distributed via the professional ophthalmology society's social media channels. The survey encompassed a series of three fundus images representing normal fundus, choroidal nevus, and choroida

Hereditary spastic paraplegia masqueraded by congenital melanocytic nevus syndrome: Dual pathogenesis of germline non-mosaicism and somatic mosaicism.

Neurocutaneous disorders are caused by germline and/or somatic mutations and involve the integument and central nervous systems. Congenital melanocytic nevus syndrome is characterized by melanotic skin lesions caused by somatic mutations at codon 61 in NRAS. A large cutaneous lesion raises the risk of central nervous system involvement. We report an 8-year-old girl with a congenital giant pigmented nevus that covered almost her entire back. Despite the absence of any radiological evidence of intracranial melanosis, the patient exhibited progressive limb spasticity with preserved intellectual a

Clear Cell Papulosis Associated With Multiple Developmental Abnormalities of the Skin.

Clear cell papulosis of the skin is a rare condition in children characterized by multiple small hypopigmented macules or flat papules, predominantly distributed in the pubic area and over the abdomen, chest, and axillae, along the milk lines, and by mucin- and cytokeratin 7-positive large clear cells within the epidermis. Associations of clear cell papulosis with other diseases or disorders have been rarely described. We present a case of clear cell papulosis associated with a congenital sacral dermoid cyst, nevus of Ota, presumed accessory breast tissue, thin and lightly pigmented hair, cong

Agminated blue nevus - Case report.

Blue nevi are benign melanocytic lesions located in the deeper reticular dermis, consequence of failure of melanocytic migration into the dermal-epidermal junction from the neural crest. Lesions are usually asymptomatic and solitary, but may present in a multiple or agminated (grouped) pattern. The agminated subtype is formed when bluish-pigmented lesions cluster together in a well-defined area. Lesions can be flat or raised. We report the case of a patient who presented multiple bluish macules (1-3 mm in diameter) grouped on the left upper back. Dermoscopy and anatomic pathological examinatio

Lentigo maligna in a woman with oculocutaneous albinism.

A 54-year-old black woman with oculocutaneous albinism was found to have a lentigo maligna and, subsequently, a pigmented compound nevus on non-sun-exposed areas of her body. The diagnosis for these lesions was confirmed by the results of histopathologic study. To our knowledge, this is the first such case reported in the literature. This patient reemphasizes the fact that melanoma development is dependent on the presence of melanocytes, but not necessarily on melanogenesis. We propose that the rarity of melanomas in these patients may be caused by the protective effect of unknown systemic fac

Treatment of Ota's nevus by Q-switched alexandrite laser : therapeutic outcome in relation to clinical and histopathological findings.

Ota's nevus is a dermal melanocytic disease which causes serious cosmetic problems for affected individuals. Recently Q-switched lasers with a pulse duration of 100 nsec or less became available for patient treatment. We evaluated the clinical efficacy of the Q-switched alexandrite laser (755 nm, 100 nsec) in relation to the histopathological findings. Fifty-five Korean patients with Ota's nevus were treated with a Q-switched alexandrite laser for three sessions (7.5 J/cm2) at three month intervals. Skin biopsies were taken in all of the patients before treatment and immediately after treatmen
