Diagnostic significance of peritumoral enhancement in distinguishing between muscle-invasive and non-muscle-invasive bladder cancer.

OBJECTIVE: The aim of this study was to assess the prevalence of peritumoral enhancement (PTE) in patients with muscle-invasive bladder cancer (MIBC) and non-MIBC (NMIBC) and to propose a modified diagnostic criterion for Vesical Imaging Reporting and Data System (VI-RADS) that incorporates PTE. MATERIALS AND METHODS: This retrospective study included 95 patients with bladder cancer (age, 72 ± 11; 77 men; 36 MIBCs and 59 Non-MIBCs) who underwent multiparametric MRI in our referral center between 2011 and 2023. The images were interpreted by four radiologists. The readers classified the possib

Persistent follicular cystitis following intravesical gemcitabine: a case series and literature review.

BACKGROUND: Intravesical gemcitabine is widely used for non-muscle-invasive bladder cancer (NMIBC) due to its favorable safety profile. While transient irritative symptoms are common, persistent inflammatory cystitis remains poorly characterized. METHODS: We conducted a retrospective observational study at a tertiary referral center. Patients receiving intravesical gemcitabine following transurethral resection of bladder tumor (TURBT) between 2022 and 2024 were screened using a stringent operational definition: (1) irritative symptoms lasting ≥ 8 weeks; (2) persistent sterile pyuria with nega

Prognostic difference of molecular subtypes in non-muscle invasive bladder cancer.

In recent years, 2 molecular types have been defined in urothelial carcinoma, as luminal and basal. The molecular subtyping is generally based on genetic studies in non-muscle invasive bladder cancer (NMIBC) however the studies with immunohistochemical reflection of this subtyping in NMIBC is limited. In our study, we aimed to use GATA3 and keratin 5/6 for subtyping of NMIBC, and to investigate the follow-up results. Materials and methods: Patients who underwent transurethral resection for the first time and whose final diagnosis was reported as NMIBC were prospectively included. A block repre

mRNA-Based Urine Test Performance in High and Very-High Risk Non-Muscle-Invasive Bladder Cancer Patients Undergoing Contextual Endoscopic Follow-up (VERNAL: Vesical Tumor Early Monitoring: mRNA-Based Follow-up).

INTRODUCTION AND OBJECTIVES: Non-muscle-invasive bladder cancer (NMIBC) patients need a strict follow-up with cystoscopy (UCS) and voided urinary cytology (vUC) due to high rate of recurrence and progression. To reduce invasiveness and costs, a new diagnostic biomarker, namely Xpert Monitor BCⓇ-detecting 5 mRNAs in voided urine-, has been proposed. We test Xpert Monitor BCⓇ ability to detect tumor recurrence at an early point during follow-up of high risk (HR) or very high risk (VHR) NMIBC patients, aiming at evaluating reliability of this test as a single procedure. MATERIALS AND METHODS: Be

Extended Outcomes of Intravesical Valrubicin and Docetaxel as a Secondary Salvage Treatment for Recalcitrant High-risk Non-muscle-invasive Bladder Cancer.

BACKGROUND AND OBJECTIVE: After multiple treatment failures, clinical practice guidelines recommend that patients with high-risk non-muscle-invasive bladder cancer (HR-NMIBC) undergo radical cystectomy (RC). However, since many patients will be either unfit or averse to radical surgery, additional bladder-sparing therapies are needed. Herein, we report the efficacy of sequential intravesical valrubicin and docetaxel (Val/Doce) as a salvage therapy for patients with recurrent HR-NMIBC. METHODS: We retrospectively identified all patients with recurrent HR-NMIBC treated with Val/Doce between 201

Protocol of the Comparison of Intravesical Therapy and Surgery as Treatment Options (CISTO) study: a pragmatic, prospective multicenter observational cohort study of recurrent high-grade non-muscle invasive bladder cancer.

BACKGROUND: Bladder cancer poses a significant public health burden, with high recurrence and progression rates in patients with non-muscle-invasive bladder cancer (NMIBC). Current treatment options include bladder-sparing therapies (BST) and radical cystectomy, both with associated risks and benefits. However, evidence supporting optimal management decisions for patients with recurrent high-grade NMIBC remains limited, leading to uncertainty for patients and clinicians. The CISTO (Comparison of Intravesical Therapy and Surgery as Treatment Options) Study aims to address this critical knowledg

Biomarkers in patients treated with BCG: an update.

INTRODUCTION: Bacillus Calmette-Guerin (BCG) instillations are the recommended treatment for non-muscle invasive bladder cancer but high recurrence and progression rates remain after treatment. Despite patients risk stratification, BCG effectiveness remains unpredictable. A close, invasive and expensive follow up is mandatory. To improve or even replace this heavy surveillance in this high risk population, validated biomarkers were developed. MATERIALS AND METHODS: To identify the useful tools for the urologist in monitoring bladder cancer patients, we reviewed the literature focusing on plas

OncoTherad® is an immunomodulator of biological response that downregulate RANK/RANKL signaling pathway and PD-1/PD-L1 immune checkpoint in non-muscle invasive bladder cancer.

INTODUCTION: Bladder cancer is the second most common urinary tract cancer. Above 70% of the occurrence of bladder cancer is superficial (pTis, pTa, and pT1), non-muscle invasive tumor (NMIBC), and the incidence of invasive disease is occasional. Treatments for NMIBC consist of transurethral resection (TUR) and subsequently intravesical immunotherapy with Bacillus Calmette-Guérin (BCG), intending to prevent tumor progression and decrease recurrence. However, 20-30% of these tumors have progression, and 70% have a recurrence after exclusive TUR treatment. The immunomodulator of biological respo

Pulmonary adenocarcinoma with micropapillary component: an immunohistochemical study. Case report.

Micropapillary carcinoma has been described in various organs, including the breast, urinary bladder, ovary and lung. We here present a case of pulmonary micropapillary carcinoma in a 72-year-old Japanese man who died of respiratory failure and septic shock, following which autopsy was performed. A mass measuring 2.5 x 2.5 x 2.5 cm was observed in the left lower lobe of the lung. The tumor showed moderately differentiated papillary adenocarcinoma with a focal micropapillary component. Carcinomatous lymphangiosis was also observed in the left lung and metastatic lesions were observed in the bil

Clinical and Cytopathological Significance of Intracytoplasmic Lumina and Immunohistochemical Mucin Expression in Non-Muscle-Invasive Urothelial Carcinoma of the Bladder.

BACKGROUND: Intracytoplasmic lumina (ICL) are mucin-containing structures identified in histological and cytological specimens of urothelial carcinoma and are associated with high-grade tumors and poor prognosis. However, their biological significance remains unclear. This study investigated mucin expression in ICL and its association with clinical, cytopathological, and prognostic features in non-muscle-invasive urothelial carcinoma of the bladder. METHODS: A total of 105 cases of non-muscle-invasive urothelial carcinoma of the bladder diagnosed were examined. Immunohistochemical staining fo

Racial differences in carcinoma-in-situ and non-muscle-invasive bladder cancer mortality: Accounting for insurance status, black segregation, and neighborhood poverty.

BACKGROUND: Few Bladder Cancer (BC) studies have examined the role of area-level variables. The purpose of this study was to examine racial differences in BC survival to elucidate if insurance status and contextual covariates could explain Black disadvantage in survival. METHOD: Using the Fine-Gray subdistribution hazard models (sHR), five-year survival time was calculated from the date of diagnosis until the last day of follow-up or the date of death due to BC in Florida 2000-2014 (n = 32,321). Non-BC deaths were considered a competing risk. In all models, individual-level clinical and demog
