BACKGROUND: Neonatal deaths contribute significantly to slower progress in under-5 mortality reduction. Lack of sufficiently trained birth assistants partly contributes to early neonatal deaths. Resuscitation training equips frontline health-care workers (HCWs) with requisite knowledge and skills to prevent birth asphyxia.
OBJECTIVE: The objective of this study was to evaluate the immediate impact of newborn resuscitation training on cognitive ability of HCWs.
MATERIALS AND METHODS: This is a descriptive observational study using pre- and posttraining scores obtained by HCWs who were partici

Two models of maternal-infant attachment, one related to full-term newborns (Goulet, Bell, St-Cyr Tribble, Paul, & Lang, 1998), and one including premature infants (Bialoskurski, Cox, & Hayes, 1999), are examined. The elements of both models are integrated into a more comprehensive model. Implicit organizational components are made explicit, and a discussion regarding the possibility of fit for the extremely low-birth-weight (ELBW) group is offered. This integrated model can serve as a basis for neonatal nurses to improve care for newly emerging families and especially those of the premature/l

Retinopathy of prematurity (ROP) is the cessation of normal eye development and subsequent abnormal vessel growth that occurs exclusively in premature infants. ROP was first discovered in the 1940s and was for two decades the leading cause of blindness in children. Currently, the disease causes about 500 new cases of blindness per year. The severity of the disease increases with decreasing gestational age. The pathogenesis of ROP involves disruption of normal retinal vascularization. Vessel endothelial growth factor, insulin-like growth factor, and oxygen play important roles in its developmen

BACKGROUND: Ostomy formation is often a necessary surgical intervention in neonates with congenital gastrointestinal or genitourinary conditions or related to a disease process; however, it poses significant challenges in Neonatal Intensive Care Unit (NICU) patients, particularly with stoma care and associated skin breakdown.
PURPOSE: The aim of this project was to achieve a 60% reduction in peristomal skin injuries within 6 months of project implementation.
METHODS: Evidence-based Practice for Improving Quality methodology and Plan-Do-Study-Act cycles were used in developing the project in

The purpose of this study was to learn how neonatal intensive care unit (NICU) nurses describe the problem of skin breakdown in the extremely low birth weight (ELBW) infant, examine interventions currently used to prevent and treat ELBW infant skin breakdown, and to learn how nurses describe and measure skin breakdown. Questionnaires were sent to 482 NICUs in the United States, and a response was requested from a registered nurse with at least 2 years NICU experience currently employed in the NICU who regularly managed ELBW infants. Questionnaires were returned from 215 NICUs (45%). Analysis r

Traditional nursing care strategies may require modification to meet the unique needs of conjoined twins. Here we discuss the strategies found to be useful in planning for and responding to distinctive circumstances encountered throughout hospitalization, as well as lessons learned. Areas of focus include ensuring privacy, designing adequate unit accommodations to meet space and equipment needs, staffing considerations and adaptations to typical neonatal intensive care nursing interventions. The utility of a team-based approach to interdisciplinary care coordination is also discussed. With ade

AimThis study examined the use of telemedicine as a means to follow up infants discharged from a Swedish neonatal intensive care unit to home health care.MethodsFamilies were randomised to either a control group receiving standard home health care (n = 42 families) or a telemedicine group receiving home health care with telemedicine support (n = 47 families) after discharge from the hospital. Both groups had follow-up hospital appointments with the neonatal nurse. In the telemedicine group, appointments were supplemented by the use of a specially designed web page and video calls.ResultsThe us

Developmental dysplasia of the hip (DDH) is a term used to describe a group of disorders of the neonatal hip in which the head of the femur is unstable or incongruous in relation to the acetabulum. Early detection and treatment of the condition generally results in normal development, whereas late diagnosis has poor success. In Northern Ireland, despite all neonates being manually examined for hip disorders by a medical officer before hospital discharge, there is an unacceptable late diagnostic rate of almost 50% of all cases. To help reduce the number of late presentations, vibration arthrome

Recent advancements in medicine have improved the survival of extremely low gestational age neonates, or small babies (22-27 weeks' gestation). Once inconceivable that an infant born as early as 22 weeks' gestation could survive, infants born at periviable gestational ages are now increasingly surviving to discharge from the NICU. Subsequently, clinical focus is pivoting toward practices that decrease morbidity in this extremely vulnerable population. This article aims to discuss obstetrical and neonatal practices during delivery to improve outcomes of the small baby and emphasize the importan

Gastroschisis, one of the more common congenital abdominal wall defects, results in herniation of fetal abdominal viscera into the amniotic cavity. This article discusses theories about gastroschisis etiology, in utero diagnostic tools, delivery options, and postdelivery care. Included are detailed considerations regarding immediate interventions after delivery to support the infant's thermal and fluid management needs and to protect the exposed bowel. Surgical options and postoperative care issues and complications are reviewed, as are respiratory distress and vena cava compression from incre

Prior to the 1970s and the advent of Rho (D) immune globulin (RIG) for Rh negative women, hemolytic disease of the newborn led to morbidity, long-term disabilities, and mortality. Antepartum RIG administration has been a standard of practice since 1983. Yet, Rh isoimmunization (sensitization) and its sequelae have not been completely eradicated. Rh-related issues remain clinical challenges facing perinatal and neonatal nurses. Evidence for the administration of RIG prenatally and during the postpartum period is presented including controversies and challenges. Current information about fetal a

BACKGROUND: The primary purpose of developmental care, the core requirement of premature infant care, is to provide a demand-central care model that reduces pressure in the environment to assist preterm infants in neonatal intensive care units (NICUs). While developmental care is the care aim of NICUs, no validated measures are currently available in Taiwan to evaluate the developmental care ability of nurses. To improve the quality of care provided to preterm infants and strengthen the on-the-job education received by nurses, the current status of relevant competencies in NICU nurses must be

Congenital diaphragmatic hernia (CDH) affects 1 in every 2,000 to 4,000 live births. Many infants with this condition are diagnosed antenatally through routine ultrasound screening. Nearly 90 percent present at delivery with severe respiratory distress requiring intubation. Many of these infants develop persistent pulmonary hypertension of the newborn due to hypoplasia of the affected lung. The survival of infants with CDH is limited by the degree of pulmonary hypoplasia and requires sophisticated medical technology such as high-frequency ventilation and inhaled nitric oxide. Some infants also

Neonatal jaundice is a common illness that affects around 80% of preterm and 50-60% of full-term newborn infants. It is one of the most common causes of neonatal death. Neonatal jaundice may be physiological or pathological. Physiologic jaundice is far more common than pathologic jaundice and accounts for most hyperbilirubinemia. Physiologic jaundice in neonates is due to greater hemoglobin breakdown compared to bilirubin clearance. While pathological jaundice occurs due to various infections, drug toxicity, inborn enzyme deficiencies, Rhesus fetal-maternal incompatibility, hypothyroidism, and

Delayed cord clamping is a collaboration between the obstetric team and the neonatal team. Although the optimal timing for clamping the umbilical cord after birth continues to be subject to study, delaying clamping for at least 30-60 seconds, with the newborn held at or below the level of the uterus/placenta supports normal physiology and has proven benefits. Practical issues such as thermoregulation and equipment can be overcome with team collaboration and prebriefing planning. Neonatal nurses who attend deliveries are perfectly poised to influence this practice and improve short- and long-te

There is little empirical evidence that guides management of infants with neonatal abstinence syndrome. The standard of care first described in the 1970s is still prevalent today, although it has never been tested in this population. Standard of care interventions include decreasing external stimulation, holding, nonnutritive sucking, swaddling, pressure/rubbing, and rocking. These interventions meet the goals of nonpharmacologic interventions, which are to facilitate parental attachment and decrease external stimuli. Many nursing interventions used in infants with neonatal abstinence syndrome

AIMS: The aims were as follows: (1) to adapt the Sophia Observation withdrawal Symptoms-Paediatric Delirium (SOS-PD) screening tool for use in neonates (SOS-ND), (2) to estimate the accuracy of SOS-ND (used by nurses) compared to assessment by a child psychiatrist using an adapted version of Vanderbilt Assessment of Delirium in Infants and Children and (3) to compare the outcome of assessments of the child psychiatrist and neonatologists.
METHODS: A descriptive and comparative design was used. An interdisciplinary group adapted the SOS-PD to develop the SOS-ND. We compared three assessments d
