[Anesthetic management of a morbidly obese patient undergoing laparoscopic gastric bypass surgery].

We describe a successful anesthetic management of a morbidly obese patient, weighing 170 kg, height of 170 cm and body mass index of 58.8 kg.m-2, who received gastric bypass surgery performed using laparoscopic assist. After arriving in the operating room, an epidural catheter was inserted into the epidural space at the T 7-8 intervertebral space. The trachea was intubated nasally under bronchofiberscopic assist, after which anesthesia was induced with propofol and maintained with nitrous oxide and oxygen (FIO2 = 0.5), i.v. propofol, fentanyl, and epidural anesthesia. Propofol infusion rate wa

Arm edema in conservatively managed breast cancer: obesity is a major predictive factor.

To identify risk factors in the development of arm edema (AE) after conservative management of breast cancer, the authors prospectively measured differences in upper and lower arm circumference in 282 patients with stage I or II breast cancer who received radiation. AE was defined as a difference of 2.5 cm or more in either measurement between treated and untreated arms. Median follow-up was 37 months (range, 7-109 months). The crude frequency of AE overall was 19.5% (55 patients). In 21 patients (7.4%) AE was transient; 34 patients (12.1%) had persistent AE, which is the focus of this article

Potential Application of Natural Products in Weight Management and Obesity: Focusing on the Gut Microbiota.

Obesity is considered to have reached epidemic proportions, and its prevalence has increased dramatically in many countries and regions around the world. In addition to affecting the quality of life of patients, obesity leads to a wide variety of adverse consequences, such as type 2 diabetes mellitus (T2DM), cardiovascular diseases, non-alcoholic fatty liver disease (NAFLD), and some cancers. Therefore, obesity has become a serious health problem. The gut microbiota is a vital component of the human body, and its stability and diversity are conducive to the maintenance of a normal physiologica

From pathogenesis to prevention: an update on the management of obesity and its associated comorbidities in cats.

Overweight and obesity represent the most common nutritional disorder in domestic cats and constitute a significant global health issue. In this review, we synthesize current knowledge on the determinants, diagnosis, pathophysiology, complications, and comprehensive management of feline obesity. Feline overweight and obesity have a complex and multifactorial pathogenesis, arising from an interplay of intrinsic factors (e.g., genetic predisposition, age, sex, neutering status) and extrinsic factors (e.g., environment, owner behavior). The condition is characterized by a state of chronic low-gra

Insights Into the Role of Obesity in Heart Failure With Preserved Ejection Fraction: Pathophysiology and Management.

Heart failure (HF) is a significant global health issue, categorized by left ventricular ejection fraction, being either reduced (HFrEF < 0.40) or preserved (HFpEF > 0.50), or in the middle of this range. Although the overall incidence of HF remains stable, HFpEF cases are increasing, representing about 50% of all HF cases. Outcomes for HFpEF are similar to those for HFrEF, leading to substantial health-care resource use. Despite extensive research over the past 2 decades, the prognosis and mortality rates for HFpEF remain high. A key feature of HFpEF is exercise intolerance, characterized by

Managing obesity-related male infertility: insights from weight loss intervention.

Obesity is a global health concern with major implications for male reproductive function. It disrupts endocrine and metabolic homeostasis, impairs semen quality, and is associated with reduced pregnancy and live birth rates. Hormonal imbalances, inflammation, and lipid stress are key contributors to these impairments. This mini-review summarizes current evidence on the impact of therapeutic interventions, including lifestyle modification, bariatric surgery, and pharmacological approaches such as glucagon-like peptide-1 (GLP-1) receptor agonists, on male fertility outcomes. Lifestyle intervent

Challenges in Cardiovascular Evaluation and Management of Obese Patients: JACC State-of-the-Art Review.

Many unique clinical challenges accompany the diagnosis and treatment of cardiovascular disease (CVD) in people living with overweight/obesity. Similarly, physicians encounter numerous complicating factors when managing obesity among people with CVD. Diagnostic accuracy in CVD medicine can be hampered by the presence of obesity, and pharmacological treatments or cardiac procedures require careful adjustment to optimize efficacy. The obesity paradox concept remains a source of confusion within the clinical community that may cause important risk factors to go unaddressed, and body mass index is

Δ9-Tetrahydrocannabivarin (THCV): a commentary on potential therapeutic benefit for the management of obesity and diabetes.

Δ9-Tetrahydrocannabivarin (THCV) is a cannabis-derived compound with unique properties that set it apart from the more common cannabinoids, such as Δ9-tetrahydrocannabinol (THC). The main advantage of THCV over THC is the lack of psychoactive effects. In rodent studies, THCV decreases appetite, increases satiety, and up-regulates energy metabolism, making it a clinically useful remedy for weight loss and management of obesity and type 2 diabetic patients. The distinctions between THCV and THC in terms of glycemic control, glucose metabolism, and energy regulation have been demonstrated in prev

Anesthetic management for cesarean delivery in a patient with severe aortic stenosis and severe obesity.

Valvular heart disease has significant effect on the maternal and fetal outcome of pregnancy. The severity and extent of aortic stenosis is of great value for risk assessment and for the design of a therapeutic plan. The therapeutic plan for such patients is further complicated by severe obesity. We report a case describing the anesthetic management of an extremely obese patient for cesarean delivery with severe aortic valve stenosis and regurgitation. The case was made complex due to the patient's deteriorating condition and not offering consent to emergency surgical procedures. The 34-year-o
