Orthostatic hypotension affects roughly 10% of individuals with hypertension and is associated with several adverse health outcomes, including dementia, cardiovascular disease, stroke, and death. Respiratory difficulty is a common presenting complaint in the outpatient primary care setting. Because patients may first seek care by calling their physician's office, telephone triage plays a role in the early management of dyspnea. Once the patient is in the office, the initial goal of assessment is to determine the severity of the dyspnea with respect to the need for oxygenation and intubation. Unstable patients typically present with abnormal vital signs, altered mental status, hypoxia, or unstable arrhythmia, and require supplemental oxygen, intravenous access and, possibly, intubation.

A plethora of static and dynamic surgical techniques have been described for lower eyelid repositioning. Autogolous split thickness punch grafting is being increasingly used as an effective technique in the management of refractory vitiligo by dermatosurgeons as an office procedure. Refinements in the technique of punch grafting has been described with special attention to the junctional areas and graft interspaces. An algorithm for the management of urinary incontinence in the female is presented. The performance of history, physical examination, and simple office bladder testing are reviewed. Patients are categorized into a complex group which require multi-channel urodynamic testing or a non-complex group which may proceed to surgical intervention without such testing.
