Clinical significance of hematic cyst of the orbit.

Hematic cyst is a rarely encountered orbital lesion consisting of encapsulated blood or blood breakdown products. The abnormality may be recognized by identifying the cystic structure by B-scan ultrasonography or computed tomography. Cysts located peripherally in the orbit tend to have histologic features suggesting hematoma formation. More centrally located orbital cysts are sometimes associated with vascular abnormalities. In other cases, the etiology is obscure, as in the case presented here. The visual prognosis is usually good, and associated systemic abnormalities are lacking.

Ocular injuries associated with fractures involving the orbit.

Ocular injuries often accompany periorbital fractures. The incidence reported by various authors varies greatly. The charts of 509 patients operated on because of a fracture involving the orbit were reviewed and the intra- and extraocular injuries were recorded. Subconjunctival haematoma, corneal abrasions and mild retinal oedema were not considered. It appeared that 26% of the injured suffered concomitant lesions of the eye and/or its adnexae. The incidence is discussed and certain hints are given, which can help non-ophthalmologists assess the severity of the trauma in cases where obvious si…

MRI of the normal orbit and optic pathway.

In recent years MRI has made increasingly important contributions to the evaluation of the orbit and optic pathway. The unique ability of MRI to demonstrate soft tissue contrast allows exquisite demonstration of anatomic detail. MRI technique and normal anatomy are reviewed. Today, optimal ophthalmologic work-up is often incomplete without the critical input of the radiologist.

Non-chromaffin paraganglioma of the orbit. Case report.

The non chromaffin paraganglioma of the orbit is a relatively rare tumor. To our knowledge only 25 cases have been reported in the world literature. We report on the case of a 39-year-old woman who was treated surgically for the removal of an orbital paraganglioma 10 years ago. She had complained of proptosis and her right eyeball was slightly displaced upwards and laterally. A transcranial operation was performed and the tumor, located medically and weighing 9'5 grs., was completely removed; it was encapsulated. 10 years after this total excision there was no evidence of recurrence. From a hi…

Frontal sinus approach to the orbit. Technical note.

The authors have previously advocated a supraorbital approach to tumors of the orbit. In this paper, they describe a technique in which they take advantage of a large frontal sinus as a means of entering the orbit without the necessity of intracranial exposure, as required by the more conventional supraorbital approach. This is achieved without frontal burr holes, allowing for a superior cosmetic result. The anterior wall of the frontal sinus is removed, and with it the roof of the orbit as a single bone flap. A case in which this technique was used is described.

Pseudallescheria boydii (Petriellidium boydii) infection of the orbit.

A 10-year-old male developed a chronic orbital infection following penetrating trauma and retention of an orbital foreign body. Diagnosis of infection with Pseudallescheria boydii (Petriellidium boydii) was made by fungal smear and culture. Sensitivity tests indicated the organism was resistant to amphotericin B. Surgical debridement of the inferior orbit and removal of the foreign body was followed by a six-week course of intravenous miconazole. The patient has shown no signs of recurrence in a two-year follow-up period. To our knowledge, this is the first reported case of orbital infection w…

Postradiation leiomyosarcoma of the orbit complicating bilateral retinoblastoma.

A 31-year-old woman had bilateral retinoblastoma diagnosed in early childhood. The right eye was enucleated at the age of 1 year, and the left eye was treated with radiation therapy (a total dose of 16,000 rad). Twenty-three years later, in 1975, a subcutaneous mass was noted in the left periorbital region. A biopsy specimen of the mass was taken and a diagnosis of pleomorphic postradiation sarcoma was made. Electron microscopic studies of the periorbital mass confirmed the diagnosis of leiomyosarcoma. After additional radiation therapy, the residual mass was surgically excised. Five years lat…

Aneurysmal bone cyst of the orbit: a review.

An aneurysmal bone cyst involving the orbital roof in a 12-year-old girl is discussed. Other reported cases of this highly unusual non-neoplastic bony lesion affecting primarily adolescents and young adults are reviewed. Similarities and differences between orbital aneurysmal bone cysts and those affecting the long bones and spine are also discussed.

Reticulum cell sarcoma (histiocytic lymphoma) of the orbit.

The clinical and pathologic features of 13 patients with biopsy-proven reticulum cell sarcoma of the orbit are reviewed. All of the patients were adults, in contrast to many previous reports that have emphasized how common this tumor is in children. Eight patients presented with proptosis and without prior diagnosis of lymphoma, and 5 patients developed orbital deposits in the course of a known systemic lymphoma. Of the 8 patients whose orbital biopsies established the diagnosis, 4 were found concomitantly to have other sites of disease, and 4 patients developed other evidence of lymphoma from…
