This paper describes the management of self-inflicted trauma by a 4-year-old boy who was mentally retarded as a result of encephalitis in infancy. There was severe damage to his lower lip, as well as a moderate degree of injury to the skin and nail of his right thumb. Preliminary intervention was made with a mouthguard using silicone elastomeric positioner material, which soon proved ineffective because of poor retention. In a retrospective review of the radiographs from 100 patients who had undergone orthognathic surgery, the authors found that 10% of radiographs had evidence of surgical complications. These complications were fractures, temporomandibular joint dislocations, transected tooth roots, retained surgical devices, avascular necrosis, malunion or nonunion of osteotomy fragments, and osteomyelitis. This report describes the use of the miniscrew as orthodontic anchorage in maxillary protrusion with a spaced arch and midline deviation in a 16-year-old female patient. In cases with midline deviation, a cant in the maxillary occlusal plane is often observed. Thus, the authors used the miniscrew to control the vertical dimension, thereby flattening the maxillary occlusal plane, and to close and retract the incisors to improve her convex profile. Fused teeth may cause aesthetic, spacing, periodontal, eruption, and caries problems. The present case report describes a 7-year-old boy patient with a chief complaint of unerupted maxillary incisor. Radiographic examination indicated a fused tooth which had two fused roots but two independent root canals. A complex management of a fused tooth is really difficult to standardize.
