A technique of percutaneous multidrilling osteotomy for limb lengthening and deformity correction.

We have recently developed a technique of per-cutaneous multidrilling osteotomy for limb lengthening and deformity correction. The bone is drilled percutaneously, using a special drill guide, and osteotomy is accomplished by connecting the multiple drill holes with a small chisel. The bone segments are subjected to slow progressive distraction with an external fixation device. We have lengthened 33 limbs in 22 patients with congenital or post-traumatic limb shortening and/or bone deformities. All the patients underwent the proposed lengthening and/or correction of the bone deformities through

The effects of orthopedic pathological conditions and systemic diseases on the prevalence of hip osteoarthritis in Modern African- and European-Americans.

Osteoarthritis (OA) is a leading cause of disability among aging adults. In the U.S., many individuals living with total hip replacements attribute OA as the cause. However, the majority of anthropological OA research excludes pathological individuals (i.e., individuals with systemic disease, traumatic injuries, or orthopedic devices). Thus, little is known about how implants and pathological conditions impact OA beyond a general acceptance that they likely increase OA risk. This study adds to the skeletal research surrounding OA by directly investigating its relationship with age, disease, an

Disc repositioning: does it really work?

Although limited, there is evidence to support the assumption that temporomandibular joint (TMJ) articular disc repositioning indeed works; to date, there is no evidence that TMJ articular disc repositioning does not work. Despite the controversy among professionals in private practice and academia, TMJ articular disc repositioning is a procedure based on (still limited) evidence; the opposition is based solely on clinical preference and influenced by the ability to perform it or not.

Monitoring the use of halo fixation device through an assessment form.

PURPOSE: To compare the use of the halo fixation device in our hospital before and after implementation of a new compliance protocol. METHODS: From 2003 to 2008, 17 (47%) of 36 patients had dislodgement of their halo fixation device. All rings and vests and some of the pins were reused. Documentation of poundage assessment and change of skull pins before dislodgement was lacking. There was no protocol for assessing superstructure throughout the course of the application. To improve the standard of care, knowledge about the application of the halo fixation device in orthopaedic and orthotic de

Luque trolley and convex epiphysiodesis in the management of infantile and juvenile idiopathic scoliosis.

STUDY DESIGN: Retrospective analysis of 5-year follow-up data from patients instrumented with Luque trolley with or without convex epiphysiodesis for management of progressive infantile and juvenile idiopathic scoliosis. OBJECTIVE: To assess results, establish predictors of outcome, and suggest more effective surgical interventions. SUMMARY OF BACKGROUND DATA: Initial results have been reported. There are no long-term follow-up studies. METHODS: Luque trolley instrumentation was used in eight patients with idiopathic scoliosis between 1983 and 1984. Luque trolley with convex epiphysiodesis

Endotracheal tube cuff pressure increases significantly during anterior cervical fusion with the Caspar instrumentation system.

To determine whether endotracheal tube cuff pressure increases significantly with surgical retraction and cervical spine distraction during anterior cervical spine surgery with Caspar instrumentation, we prospectively studied 10 patients undergoing this procedure. The tracheas of all patients were intubated with a Mallinckrodt Hi-Lo endotracheal tube. Tracheal tube cuff pressures measured with a transducer system were 42.4 mm Hg +/- 7.0 mm Hg (SEM) after intubation and cuff inflation. Air was removed from the endotracheal tube cuff until the trachea was just barely sealed at a cuff pressure of

Modified medial column arthrodesis.

This manuscript describes a modified medial column arthrodesis involving the medial and intermediate naviculocuneiform joints, the first metatarsocuneiform joint and the first intercuneiform joint. Medial column stabilizing procedures are usually advocated as a component part of a generalized hypermobile flatfoot repair. This procedure, however, is being recommended for elderly or sedentary individuals who have medial column pain, difficulty ambulating and problems wearing standard foot gear secondary to end-stage medial column degenerative joint disease and medial column collapse.

Static and cyclical biomechanical analysis of pedicle screw spinal constructs.

Biomechanical evaluation of twelve different spinal devices in vitro employing pedicle screws was performed using static (n = 5) and cyclical testing (n = 3) parameters. In general, the rank order of implant failures was similar between static and cyclical tests, performed at 600 N compressive load, 5 Hz, and 1 million cycles. The mean number of cycles to failure was higher for spinal instrumentation systems employing longitudinal rods than those using plates (ANOVA F = 16.94, P < .001). At 600 N, the compact Cotrel-Dubousset, TSRH, and ISOLA rod systems demonstrated mean cycles to failure ran

An assessment of oblique base wedge osteotomy stability in the first metatarsal using different modes of internal fixation.

The modified Juvara osteotomy is a classic oblique base wedge osteotomy of the first metatarsal and is often used to reduce hallux valgus deformity. However, complications attributed to osteotomy instability and currently available modes of osteosynthesis are common. The authors have developed a technique that enables them to quantitatively and objectively evaluate the stability of this osteotomy when various modes of internal fixation are used. Stability testing was performed on fresh, previously frozen human cadaveric specimens using engineering principles. Results indicate that there is no

[Surgical treatment of the sequelae of ischemic postdysplasia femur head necrosis in children. II. Surgical technique].

The author presents its concept concerning the possibilities of the surgical treatment of the sequelae of the postdysplastic necrosis of the femoral head originating from the treatment of congenital dislocation or subluxation of the hip joint. The classification is based on the division according to Bucholz and Ogden into 4 types. Indicated for the surgical treatment are hips with the injury of the type II and III. In Grade II type the joint congruence is not affected and the method of operation is the redirection pelvic osteotomy. In children up to the age of 10 the author applies Salter oste

Biomechanical principles of tension band wiring applied to fractures of the distal fibula and fifth metatarsal base.

Application of the tension band principle to provide dynamic compression across sites of the ankle and foot is discussed. Two cases are presented that demonstrate the basic principles of tension band fixation. A biomechanical explanation of fracture failure in long bones is also provided.

Idiopathic scoliosis: foundation for physiological treatment.

The three-dimensional nature of the idiopathic spinal deformity has been investigated in cadaveric specimens and patients with both idiopathic scoliosis and idiopathic kyphosis (Scheuermann's disease). In both scoliotic and kyphotic deformities the essential lesion lies in the sagittal plane with apical vertebral wedging. In idiopathic scoliosis there is an apical lordosis which being biomechanically unstable rotates to the side to produce a scoliotic deformity as a secondary component. In contradistinction the kyphotic wedging process of Scheuermann's disease is mechanically stable and any as
