Sciatic nerve compression by neurogenic heterotopic ossification: use of CT to determine surgical indications.

OBJECTIVE: To describe the characteristics of neurogenic heterotopic ossification (NHO) based on clinical tests, electroneuromyography (ENMG) and CT in a database of patients with lesions of the central nervous system who required sciatic nerve neurolysis along with posterior hip NHO resection, and to determine the respective roles of ENMG and CT in the management of posterior hip NHOs in patients who are unable to communicate or express pain. METHODS: The consistency of the ENMG results with clinical findings, CT results and macroscopic signs of lesions was retrospectively assessed after sci

Association of vitamin D deficiency, secondary hyperparathyroidism, and heterotopic ossification in spinal cord injury.

Our objective was to explore the relationship between low vitamin D, secondary hyperparathyroidism, and heterotopic ossification (HO) in patients with spinal cord injury (SCI). Ninety-six subjects with acute or chronic motor complete SCI participated. Levels of serum vitamin D25(OH), calcium, and intact parathyroid hormone (PTH) were collected, and information regarding nutritional patterns and fracture history was obtained from subjects. Evidence of current or previous HO was ascertained through chart review. Of the 96 subjects, 12 were found to have developed HO, 11 with serum vitamin D25(OH

Cancer risk estimates from radiation therapy for heterotopic ossification prophylaxis after total hip arthroplasty.

PURPOSE: Heterotopic ossification (HO) is a frequent complication following total hip arthroplasty. This study was conducted to calculate the radiation dose to organs-at-risk and estimate the probability of cancer induction from radiotherapy for HO prophylaxis. METHODS: Hip irradiation for HO with a 6 MV photon beam was simulated with the aid of a Monte Carlo model. A realistic humanoid phantom representing an average adult patient was implemented in Monte Carlo environment for dosimetric calculations. The average out-of-field radiation dose to stomach, liver, lung, prostate, bladder, thyroid

Heterotopic ossification after cervical disc replacement: clinical significance and radiographic analysis. A prospective study.

Limited data is available regarding heterotopic ossification (HO) after cervical disc replacement (CDR). The goal of this study was to determine the incidence of HO after CDR with the Mobi-C artificial disc to identify the risk factors for HO, and to investigate whether HO affects clinical outcome and range of motion (ROM). Seventy one patients were included in this study. The mean follow-up was 21 months. Radiological evaluation included grading of HO and assessment of ROM for each level treated. HO was detectable in 23 treated segments (27.7%). The mean ROM was 8.1 degrees preoperatively and

Multilevel arthroplasty for cervical spondylosis: more heterotopic ossification at 3 years of follow-up.

STUDY DESIGN: Retrospective cohort study. OBJECTIVE: To investigate the differences between single- and multilevel degenerative disc diseases (DDDs) treated with cervical arthroplasty. SUMMARY OF BACKGROUND DATA: The US Food and Drug Administration clinical trials compared arthroplasty with anterior cervical discectomy and fusion for single-level DDD. However, cervical arthroplasty for multilevel DDD is rarely addressed in the literature. METHODS: A total of 102 consecutive patients who underwent Bryan arthroplasty were divided into either a single- or multilevel group. Clinical outcomes we

Periarticular infiltration in total hip replacement: effect on heterotopic ossification, analgesic requirements and outcome.

BACKGROUND: Up to 80% of patients develop heterotopic ossification (HO) following total hip replacement (THR) and high grades may adversely affect outcome. This study investigated the influence of local infiltration of a NSAID (Ketorolac) and local anaesthetic on the incidence and grade of HO following THR, the effect on post-operative opiate analgesic requirement and on patient reported outcome score. METHODS: A retrospective study was performed on 118 THRs performed without periarticular infiltration from 2003 to 2005, and on 211 performed with infiltration from 2005 to 2008. Pre-operative

Heterotopic ossification following surgical treatment of avulsion fracture of the anterior inferior iliac spine.

Avulsion fractures of the anterior inferior iliac spine are rare injuries of the pelvic ring and occur during sports activities. Hereby is presented a case of a 22-year-old professional football player who was diagnosed to have an avulsion fracture of the anterior inferior iliac spine on the right side four months after the initial injury and he was treated surgically with the excision of the avulsed fragment. The football player recovered completely and returned to his usual sports activities. Two years later, due to the pain in the hip an x-ray and MR image were made, which established the e

Implant design may influence delayed heterotopic ossification after total disk arthroplasty in lumbar spine.

BACKGROUND: As total disk arthroplasty (TDA) gains increasing acceptance as an alternative to fusion for degenerative disk disease of the lumbar spine, new complications are encountered by the physicians during and after the procedure. We hereby report a complication after TDA in the lumbar spine that is in variance from previously proposed theories and suggests the possibility of implant design as one of the etiologic factors. The purpose of the present submission is to report a case of delayed heterotopic ossification (HO) after TDA that suggests that the keel-based design of the implant mig

Heterotopic ossification masquerading as deep venous thrombosis in head-injured adult: complications of anticoagulation.

A 26-year-old man manifested clinical signs of a left iliofemoral thrombosis 12 weeks after closed head injury in a motor vehicle accident. The deep vein thrombosis was initially diagnosed by venography and appropriate anticoagulation therapy was instituted. After four days of treatment, there was no significant resolution of signs or symptoms and the circumference of the left thigh had increased with an associated decrease in hemoglobin. A CT scan of the involved thigh revealed hemorrhage and calcification within the quadriceps muscle. In retrospect, it was evident that the hemorrhage and het

A Case Report of Mechanical Intestinal Obstruction Caused by Delayed Heterotopic Ossification of Postoperative Incision.

Heterotopic ossification (HO) refers to the abnormal formation of mature lamellar bone within soft tissues where bone is not typically present, such as muscles, tendons, ligaments, and joint capsules. While commonly associated with trauma, spinal cord injury, or orthopedic procedures, abdominal incisional heterotopic ossification (AIHO) is exceedingly rare. Its pathogenesis may be closely related to local trauma, inflammatory responses, and genetic predisposition. AIHO typically occurs between 3 months and 5 years postoperatively, with an estimated incidence of 0.3%-1.2% among all HO cases. Os

When ectopic bone forms in the abdomen: The diagnostic and management challenges of heterotopic mesenteric ossification.

Heterotopic mesenteric ossification (HMO) is a rare medical condition, with < 100 cases reported globally by 2024. This disorder is characterized by abnormal bone tissue formation within the mesentery, often following abdominal trauma, ischemia, or infection. This editorial reviews the case presented by Zhang et al, involving a 34-year-old male who developed persistent left lower abdominal pain after sustaining blunt trauma to the abdomen. Diagnostic challenges arose due to the rarity and nonspecific presentation of HMO, which shares histopathological features with conditions such as myositis

Healing an ischial tuberosity pressure ulcer in a patient with neurogenic heterotopic ossification: a case report.

Neurogenic heterotopic ossification (NHO) is widely recognised as an aberrant bone formation in soft tissue following central nervous system injury. It is most frequently associated with pain and limited movement, especially in the hip. However, it may be neglected in patients with paraplegia with a pressure ulcer (PU). We report the case of an 18-year-old male patient who presented with a hard-to-heal ischial tuberosity PU and who had undergone three operations at other hospitals during the previous six months, which had failed to repair the PU. There was a history of paraplegia as a conseque

The Use of Tranexamic Acid for Primary Prophylaxis of Heterotopic Ossification Following Total Hip Arthroplasty.

BACKGROUND: Heterotopic ossification (HO) is a relatively common complication after total hip arthroplasty (THA) and can range from a radiographic observation only to severely disabling and requiring revision surgery. Prophylaxis is recommended for high-risk patients, though the ideal method and targeted population are open to debate. Tranexamic acid (TXA) is a medication increasingly being used to reduce blood loss associated with orthopaedic surgeries, including THA. METHODS: A retrospective review of 357 patients undergoing THA from November 2020 through December 2023 was conducted. The pa

The influence of nerve surgical techniques at time of amputation on the prevalence of heterotopic ossification in transtibial amputees.

BACKGROUND: Heterotopic ossification (HO) can form after amputation and may cause pain and functional impairment. We aimed to describe the prevalence of HO in a civilian population of transtibial amputees. We hypothesized that the decreased rate of symptomatic neuroma following active nerve surgery (Targeted Muscle Reinnervation (TMR) or Regenerative Peripheral Nerve Interface (RPNI)) may subsequently lead to a lower prevalence of HO compared to passive nerve surgery (i.e. traction neurectomy) performed at the time of amputation. METHODS: Adult patients undergoing transtibial amputation at a

Anterior Elbow Heterotopic Ossification in Patient With Pemphigus Vulgaris: An Unheard Complication of a Rare Condition.

Pemphigus vulgaris is a potentially fatal disease within the epidermis with rare noncutaneous manifestations. Heterotopic ossification has not been previously described as one of the inherent complications of this skin pathology. A 44-year-old man presented with severe pemphigus vulgaris involving 80 to 90% of his body surface area. He had an extended time to wound closure of 5 months, as well as two additional months bed bound due to related infectious and respiratory complications. He progressively developed a worsening range of motion at his bilateral elbows. X-ray demonstrated bilateral an

Successful Total Knee Arthroplasty in a Patient With Contralateral Ankylosis Due to Severe Heterotopic Ossification.

A 63-year-old woman with ankylosis of the left knee due to severe heterotopic ossification (HO) following total knee arthroplasty (TKA) underwent right TKA with preoperative radiation and postoperative chemical prophylaxis for HO. At the one-year follow-up, the patient had no evidence of HO in the right knee. To our knowledge, there are no reports of successful arthroplasty in patients with a history of ankylosis due to severe HO. We present the first case of successful TKA in a patient with ankylosis of the contralateral knee. TKA can be safely performed in patients at high risk for developin

Heterotopic Ossifications after Fractures of the Anterior Pelvic Segment: Overview of Morphology and Clinical Correlations.

PURPOSE OF THE STUDY In certain patients after treated pelvic fractures, heterotopic ossifications can be observed in the area of the pubic bone, which protrude ventrally, and often also laterally and distally into surrounding soft tissues of the groin or even medially into the proximal thigh. These ossifications are shaped like sharp spikes of various lengths, which is why the authors refer to them as "spicules". In some patients, these ossifications are also associated with pain. The aim of this study was to provide an overview of the morphology of heterotopic ossifications of the anterior s

Activin A promotes the development of acquired heterotopic ossification and is an effective target for disease attenuation in mice.

Heterotopic ossification (HO) is a common, potentially debilitating pathology that is instigated by inflammation caused by tissue damage or other insults, which is followed by chondrogenesis, osteogenesis, and extraskeletal bone accumulation. Current remedies are not very effective and have side effects, including the risk of triggering additional HO. The TGF-β family member activin A is produced by activated macrophages and other inflammatory cells and stimulates the intracellular effectors SMAD2 and SMAD3 (SMAD2/3). Because HO starts with inflammation and because SMAD2/3 activation is chondr

Extensive Atraumatic Heterotopic Ossification of the Achilles Tendon in an Adolescent with Metabolic Syndrome: A Case Report.

CASE: A 15-year-old boy with type 1 diabetes mellitus, hypertension, and obesity presented with atraumatic posterior ankle pain and stiffness due to extensive heterotopic ossification (HO) of the Achilles tendon. The ossification was successfully surgically resected and tendon primarily repaired. Wound dehiscence was noted at the first preoperative visit, managed conservatively by local wound care, and healed uneventfully by secondary intention. One-year follow-up showed no recurrence of HO, return to baseline activities, yet low Oxford scores. CONCLUSION: HO of the Achilles tendon is a rare
