Osteogenesis imperfecta is a hereditary connective tissue disorder characterized primarily by fractures with no or small causal antecedent; in most patients this is a consequence of diminished or abnormal production of collagen type I. It is a clinically heterogeneous disorder: it has been proposed recently to classify osteogenesis imperfecta in types I-V on the basis of the clinical picture and radiology. It is also a genetically heterogeneous disorder; 90% of cases are due to autosomal dominant mutations, while the remaining 10% are due to autosomal recessive mutations or of unknown cause. O…

More than 90% of people who have osteogenesis imperfecta (OI) have heterozygous mutations in one of the two type I collagen genes, COL1A1 and COL1A2. The effects of these changes range from death in the perinatal period to barely increased fracture frequency and reflect different types of mutations. Introduction of bisphosphonates during the past 20 years has targeted bone fragility by decreased resorption. The recent recognition of biallelic mutations in genes that affect either collagen assembly and processing or the regulation of osteoblast development has raised hopes for therapies that wo…

A limited number of reports published since 2001 have described an association between increased nuchal translucency (NT) and osteogenesis imperfecta (OI). We report a new case which underlines the frequency of this association as well as the importance of follow-up and genetic evaluation. In the present case, ultrasound scanning at 13 weeks of gestation showed a NT of 3.2 mm and no other pathological findings. At 20 weeks a severe skeletal dysplasia was diagnosed by ultrasound. The pathology report of the aborted foetus indicated OI, and DNA analysis confirmed a COL1A1 mutation.

INTRODUCTION: Shaken baby syndrome (SBS) is a form of physical abuse that includes the presence of a subdural or subarachnoid haematoma or diffuse cerebral oedema, retinal haemorrhages and, in general, absence of other physical signs of traumatic injury. Osteogenesis imperfecta (OI) is a genetic disorder affecting the synthesis of type I collagen that leads to brittle bones with frequently occurring fractures, with presenting clinical symptoms taking a variety of forms. A differential diagnosis allowing it to be distinguished from physical abuse is known, due to the existence of bone fractures…

Osteogenesis imperfecta (OI) is a heterogeneous group of genetic disorders that affect connective tissue integrity. The hallmark of OI is bone fragility, although other manifestations, which include osteoporosis, dentigenesis imperfecta, blue sclera, easy bruising, joint laxity and scoliosis, are also common among OI patients. The severity of OI ranges from prenatal death to mild osteopenia without limb deformity. Most forms of OI result from mutations in the genes that encode either the proalpha1or proalpha2 polypeptide chains that comprise type I collagen molecules, the major structural prot…

A case of a child with the hyperplastic callus formation in association with osteogenesis imperfecta is presented. The soft tissue mass arising at the site of femoral fracture was clinically and radiologically suspected to be a hyperplastic callus, although a possibility of osteosarcoma could not be completely excluded. A drill biopsy of the soft tissue mass was performed which showed osteocartilaginous callus. The soft tissue mass disappeared following biopsy. This occurrence is also reported in previously described cases.

BACKGROUND: In Osteogenesis Imperfecta severity of disease and reduced physical activity have been considered the main factors contributing to hypercalciuria; however, its pathogenesis in Osteogenesis Imperfecta Type I, in which mobility is normal, is still unclear.
PATIENT, METHODS AND RESULTS: We describe a patient with Osteogenesis Imperfecta Type I and hypercalciuria, in whom measurement of calcium intake, plasma 1 - 25(OH) (2) Vitamin D, fasting calciuria and tubular proteinuria led us to exclude an absorptive or renal component in the pathogenesis of hypercalciuria.
CONCLUSIONS: We bel…

Osteogenesis imperfecta is a heritable disorder of bone formation resulting in low bone mass and a propensity to fracture. It exhibits a broad range of clinical severity, ranging from multiple fracturing in utero and perinatal death to normal adult stature and a low fracture incidence. The disorder is currently classified into seven types based on differences in clinical presentation and bone architecture. Mutation in one of the type I collagen genes is commonly associated with osteogenesis imperfecta, but is not a prerequisite for the diagnosis. Indeed, the newer forms of osteogenesis imperfe…

Primary hip disorders, such as congenital hip dislocation, have rarely been reported in children with osteogenesis impefecta (OI). Protrusio acetabuli and coxa vara secondary to severe osteopenia and recurrent fractures are common. Perthes' disease in association with osteogenesis imperfecta has not been reported previously. Epiphyseal and growth plate changes may occur in the more severe forms of OI. There was no evidence of growth plate disruption or severe osteopenia in our case. Approximately two-thirds of OI patients have a tendency to bleed easily but the association of clotting and blee…

The combination of osteogenesis imperfecta (OI) and osteosarcoma is rare, as reflected by the fact that there are only eight proven cases in the published English literature. We present here another case, one that was treated by modern oncological techniques and major limb salvage surgery in the form of distal femur endoprosthesis initially and later a total femur replacement. The patient is the only one of the series to have had major limb salvage and is the only--and longest--surviving patient (more than 6 years), thus disproving the probability that this combination of disorders is particul…

Osteogenesis imperfecta is a heterogeneous group of inherited disorders chiefly affecting type I collagen, resulting in bone fragility responsible for a host of recurrent fractures. Limb and spine deformities, growth failure and restricted mobility are the end-result in severe cases. Cyclical intermittent intravenous infusions of pamidronate, a potent inhibitor of bone resorption, have yielded substantial improvement in chronic pain, bone mineral density, fracture rate and mobility without significant side effects. The role of growth hormone and bone marrow transplantation in the treatment of …

Osteogenesis Imperfecta ordinarily leads to an early death. Survival to age 30 is unusual. In this case the patient achieved some pleasure in life, was reported to be of normal intellectual capacity, and proved capable of great skill in certain crafts. He truly lived his life and his last years were made possible by skills of his home health care team, the devotion of his mother, and his own determination, humor, defiance, and toughness of character.

A 65-year-old woman presented with vertebral fractures of the lumbar spine and a history of pathological fractures following minor trauma, which had occurred before the onset of menopause. Her past medical history was significant for intermittent low back pain since childhood, which was attributed to thoracolumbar scoliosis. A diagnosis of unclassifiable osteoporosis was made until invasive diagnostic procedures suggested a mild form of type I osteogenesis imperfecta (OI). In unclear or atypical perimenopausal osteoporosis and diagnosis of OI should be considered.

Osteogenesis imperfecta (OI), or the Van der Hoeve-de Kleyn syndrome, is a heterogeneous group of connective tissue disorders. The key features in this disease are bone fragility with a tendency to spontaneous fractures and deformations. The classical traid of symptoms involves a conductive and/or sensorineural hearing impairment together with a tendency to spontaneous bone fractures and blue sclerae. Between January 1988 and December 1994, ear surgery was performed on eight ears of six OI patients who presented with mixed hearing loss preoperatively. Pathological changes observed in the middl…

Bone mineral content (BMC) in the radius of 23 osteogenesis imperfecta (OI) patients (1-26 yr) was measured with SPA technique. Comparing the values of OI type I. patients with that of type III. patients, the decrease of BMC was more prominent in the latter group, however, the two group hadn't been distinct from each other at p < 0.05 level. Based on the measurements of 39 relatives (mother, father, brother) the method seems not to be helpful in screening of gene-carrying relatives because of the high variability of the phenotype of the disease. Follow-up SPA studies-avoiding the repositioning…
