THE VOICE OF THE PATIENT IN OROFACIAL PAIN MANAGEMENT.

As pain cannot be measured objectively, the use of patient reported outcomes (PROs), and specifically dental PROs (dPROs), is essential for adequate assessment and management of the patient with orofacial pain. For orofacial pain conditions, some of the suitable PROs are specific to dentistry and hence can be labelled dPROs, whereas others are not. There is also a need to understand which outcomes and domains are most relevant to the patient with pain complaints within the context of the biopsychosocial model. Acute pain in the orofacial area is most often related to toothache, whereas the mos

Workplace Violence in the Setting of Pain Management.

In the context of the opioid crisis, increased attention has been placed on the risk of violence in outpatient pain medicine clinics. The primary objective of this study was to determine the prevalence and characteristics of workplace violence in a mixed group of clinicians (ie, practicing physicians, resident and fellow physicians in training, nurse practitioners, physician assistants, psychologists) participating in a workplace violence education session at a national pain conference held March 6 through March 10, 2019. A published survey instrument developed to assess workplace violence amo

Managing tooth pain in general practice.

Tooth pain is a common presentation in primary care, with 32.4% of Singaporeans experiencing pain from dental caries in their lifetime. Some systemic conditions can have oral presentations, and oral conditions may be associated with chronic disease. A good history and examination is key in delineating odontogenic from non-odontogenic causes of tooth pain. Primary care physicians should accurately diagnose and assess common dental conditions and make appropriate referrals to the dentist. Common non-odontogenic causes of orofacial pain can be mostly managed in primary care, but important diagnos

Methadone for Pain Management in Children with Cancer.

Methadone is a synthetic opioid with unique pharmacodynamic and pharmacokinetic properties. It is effective in treating both nociceptive and neuropathic pain, which commonly co-exist in children with cancer. Upon reviewing the literature describing the use of methadone in pediatric oncology patients, publications are limited in number and low in quality of evidence; nevertheless, there is support for the safety and efficacy of methadone in treating pain in children with cancer, particularly when pain is refractory to conventional treatment. Although the risk of life-threatening arrhythmia is c

Fentanyl Formulations in the Management of Pain: An Update.

Fentanyl is a synthetic, highly selective opioid with many desirable physicochemical properties, including a high lipophilicity and predictable pharmacokinetics. These properties have an established record in the management of pain in a variety of settings, particularly acute pain and breakthrough cancer pain. Fentanyl was initially developed for parenteral use; however, this is invasive and impractical in the outpatient setting. Unfortunately, the high first-pass metabolism of fentanyl makes oral formulations unfeasible. However, its high lipophilicity allows fentanyl to be absorbed via a num

Orofacial pain - an update on diagnosis and management.

The diagnosis and management of orofacial pain may be challenging due to complex histories, pathophysiology and associated psychosocial co-morbidities such as depression and anxiety. Neuropathic facial pain conditions such as burning mouth syndrome (BMS), persistent idiopathic facial pain (PIFP), atypical odontalgia (AO) and trigeminal neuralgia (TN) require early recognition by primary care clinicians and referral to secondary care. Acute pain-related temporomandibular disorder (TMD) may be managed in the primary care setting, with identification of those at risk of developing chronic TMD rec

AN AUDIT OF THE MANAGEMENT OF ACUTE PAIN IN CHILDREN.

AIM: To evaluate clinical management of acute pain with respect to pain assessment, scoring and timing of analgesia and whether appropriate supportive medicines were prescribed alongside strong opiates. A previous pain audit found dosing of analgesia was appropriate but did not assess clinical management against pain scores. Our paediatric guideline does not currently stipulate guidance on appropriate time frames to administer analgesia and re-assess pain. Standards were developed with a multidisciplinary team to audit against. METHODS: Data were collected over two weeks on paediatric wards e

The approach to the management of the patient with neuropathic pain.

Neuropathic pain occurs in about 6-7% of the general population and in 15-20% of people with diabetes. It is defined as a disease or disorder of the sensorimotor system and must be distinguished from nociceptive pain, which is a consequence of trauma, injury, or inflammation. A host of other conditions can masquerade as neuropathy including entrapments, fasciitis, and claudication. Pain can derive from damage to unmyelinated C-fibers, Aδ fibers in the periphery, or from mechanisms within the spinal cord, brainstem, and cerebral cortex. A variety of excitatory and inhibitory neurotransmitters a

Management strategies for painful bladder syndrome.

Painful bladder syndrome/interstitial cystitis (PBS/IC) is a condition of chronic pelvic pain associated with irritative voiding symptoms. Management of PBS/IC has been a challenge for generations of physicians, owing to a lack of consensus on its definition, an incompletely understood pathophysiology, and numerous available therapies without high-quality evidence to guide their usage. This article reviews the most current conception of PBS/IC and data on effective treatments to recommend a management strategy.

[Physiopathology and management of pain in multiple myeloma].

Pain is a troublesome problem in multiple myeloma (MM) patients which may be affected by several pain syndromes sustained by different underlying causes and mechanisms- In this setting may be due to the disease itself, to its complications, to diagnostic procedures and therapeutic interventions. Most pain syndromes origin from the skeleton and are related to several aspects of the myeloma bone disease (osteolysis and osteopenia). Other remarkable pains frequently observed in MM are due to disease-related and iatrogenic neuropathies. For most MM patients the World Health Organization's three-st

Review of oral oxymorphone in the management of pain.

Chronic cancer and nonmalignant pain (CNMP) is a common and major health problem afflicting approximately 40 million persons in the US. Most cancer patients, and many patients with CNMP, require opioid analgesics to obtain adequate pain relief. Oral oxymorphone is a new formulation of an existing parenteral opioid that has become available for the treatment of significant pain: acute postoperative, chronic arthritis, chronic low back, and chronic cancer pain. Oxymorphone is a typical mu-opioid agonist that is effective in both immediate- and extended-release (IR and ER) formulations. Oxymorpho

Lumiracoxib in the management of osteoarthritis and acute pain.

Lumiracoxib is a highly selective COX-2 inhibitor with a novel chemical structure and a relatively short plasma half-life. It has been approved in > 40 countries for the symptomatic treatment of osteoarthritis and/or acute pain related to primary dysmenorrhoea and dental or orthopaedic surgery. In these conditions, lumiracoxib has proved to be as effective as standard doses of conventional NSAIDs and other COX-2 selective inhibitors (coxibs). According to the Therapeutic Arthritis Research Gastrointestinal Trial, which enrolled 18,325 patients with osteoarthritis, lumiracoxib 400 mg/day (four

Update on managing chronic pain in the elderly.

People aged 65 years and over make up the fastest growing demographic in the United States. By the year 2040 they will comprise approximately one fourth of the US population. The elderly patient in need of chronic pain therapy presents challenges best met with an enlightened and effective treatment strategy. Practice standards must include a thorough pain assessment and formation of a multimodal care plan, which applies knowledge of pain management in an objective and scientific manner. In this article, a patient case study illustrates how the appropriate management of chronic pain in an elder

Managing pain in patients with dementia in hospital.

This is the fourth article in a series of five on dementia. Pain experienced by people with dementia is often poorly managed. Chronic pain can lead to further cognitive impairment, accidents and behavioural problems which can be minimised if the pain is properly assessed and managed. Patients with dementia receive fewer analgesics than any other patient group, but the likelihood of pain in this group is as high as in other older people. This article discusses the role of nurses in identifying and treating pain in patients with dementia. Approaches to assessment for improved detection and treat

[Role of paracetamol in the acute pain management].

Paracetamol (acetaminophen) has been shown to be an effective analgesic for the treatment of moderate pain where it is chiefly indicated, as shown in placebo-controlled studies in the perioperative setting and other acute pain states. In addition, an opioid-sparing effect has been demonstrated. No clinically relevant adverse effects are usually apparent with recommended doses. Paracetamol is an effective component in 'multimodal analgesia' in combination with morphine, weak opioids and non-steroidal anti-inflammatory drugs. Although most studies involve the perioperative setting, similar resul
