Scar and short-term outcomes after transverse versus modified Henry incisions for volar plating of selected distal radius fractures: a retrospective comparative study.

BACKGROUND: Volar locking plate fixation is widely used for distal radius fractures (DRFs), but visible scarring and incision-related morbidity remain important patient-centred concerns. This exploratory retrospective study compared scar outcomes and selected short-term clinical outcomes between a single palmar lateral transverse incision and the modified Henry approach in surgically treated DRFs selected for limited volar exposure. METHODS: This retrospective comparative study included 77 of 86 potentially eligible patients with DRFs who underwent volar locking plate fixation between January

Minimally invasive internal fixation for extra-articular distal radius fracture: Comparison between volar plate and intramedullary nail.

INTRODUCTION: There are numerous internal fixation techniques for distal radius fracture, using pins, plates or nails. Some authors have developed minimally invasive procedures. The aim of the present study was to compare two minimally invasive internal fixation techniques for unstable extra-articular distal radius fracture in women over 50 years of age: volar plate (minimally invasive plate osteosynthesis: MIPO), and intramedullary nail. HYPOTHESES: The main study hypothesis was that the incision scar left by minimally invasive internal fixation is smaller using MIPO than an intramedullary n

Intramedullary nail versus volar plate fixation of extra-articular distal radius fractures. Two year results of a prospective randomized trial.

BACKGROUND: Intramedullary techniques for stabilization of displaced distal radius fractures are now available. Purported benefits include limited soft tissue dissection while affording sufficient stability to allow early wrist motion. The primary null hypothesis of this randomized trial is that there is no significant difference with respect to functional outcome, pain and disability between patients treated with either 2.4-mm volar locking plate fixation or intramedullary nail fixation of unstable dorsally displaced extra-articular fractures of the distal radius. METHODS: We conducted a sin

Predictors of distal radioulnar joint instability in distal radius fractures.

PURPOSE: A tear of the triangular fibrocartilage complex (TFCC) is the most frequent soft tissue injury associated with fractures of the distal radius, and repair of the deep ligamentous portion of the TFCC is considered when the tear contributes to instability of the distal radioulnar joint (DRUJ). The purpose of this prospective cohort study was to identify predictors of DRUJ instability accompanying unstable distal radius fractures. METHODS: Between 2002 and 2007, we prospectively treated 163 consecutive patients with unstable distal radius fractures with the volar locking plating system.

Arthritis predicting factors in distal intraarticular radius fractures.

INTRODUCTION: Intra-articular malunion with step off and gap formation is claimed to be crucial for radiocarpal degeneration. In addition to well-defined intraarticular malunion, the shape of the distal radius is important for unaffected wrist function. In typical intra-articular fracture patterns with a dorsoulnar and palmar ulnar fragment, alterations of the shape of the articular surface, in a kind of a cavity, without obvious step off can be observed. The aim of the present study is to determine the residual articular deformity following intra-articular radius fractures and to analyze thei

Investigating the minimum distance between the finger flexor tendons and distal radius during wrist and finger positions in healthy people.

PURPOSE: Rupture of the flexor pollicis longus (FPL) and index flexor digitorum profundus (FDP2) tendons often occurs after locking plate fixation for distal radius fracture. This study aimed to determine the shortest tendon-radius distances of different hand positions. METHODS: Fifty-nine hands of 30 healthy volunteers were studied. Distances between the FPL or FDP2 and distal radius were calculated in six wrist positions: 30° palmar flexion, neutral, 30° dorsiflexion, 60° dorsiflexion, maximum dorsiflexion, and 40° ulnar deviation with three finger positions (full extension and flexion of f

Incidence of clinically symptomatic pneumothorax in ultrasound-guided infraclavicular and supraclavicular brachial plexus block.

The use of periclavicular brachial plexus block as regional anaesthesia for surgical procedures on the upper extremity is common. However, the proximity of the pleura results in a risk of pneumothorax. Without ultrasound monitoring, the pneumothorax risk has been reported to be as high as 6.1%. We conducted a prospective, observational study to examine the risk of pneumothorax in 6366 ultrasound-guided periclavicular plexus blocks. All patients with a clinically manifest and radiologically confirmed pneumothorax were analysed. Clinically symptomatic pneumothorax occurred in four patients (0.06

Volar fixed-angle plate osteosynthesis of unstable distal radius fractures: 12 months results.

BACKGROUND: With an incidence of about 2-4 per 1,000 residents per year, the distal radial fracture is the most common fracture in the human skeleton. The introduction of fixed-angle plate systems for extension fractures at the radius was evaluated in a prospective study performed at our hospital after selection and acquisition of a new system. The focus of our interest was whether a secondary loss of reduction can be avoided by this plating system. METHODS: We reviewed 80 patients treated for unstable distal radius fractures using a volar fixed-angle plate. Postoperative management included

The climber's finger: imaging of non-pulley finger injuries.

Climbing is a rapidly growing sport that subjects the fingers to intense mechanical stress, predisposing athletes to a wide spectrum of injuries beyond the well-recognized flexor pulley lesions. Non-pulley-related finger injuries, although often underrecognized, can impair performance, cause chronic pain, and lead to long-term complications such as osteoarthritis, cyst formation, or tendon rupture. These injuries involve flexor tendons, collateral ligaments, volar plates, joint capsules, extensor mechanisms, lumbrical muscles, epiphyseal growth plates in adolescents, digital nerves, and palmar

Distal radial fracture arthroscopic intraarticular gap and step-off measurement after open reduction and internal fixation with a volar locked plate.

PURPOSE: A persistent articular gap and a step-off of ≥1 mm after a distal radial fracture (DRF) may lead to post-traumatic arthritis of the radiocarpal joint. This study aims to arthroscopically assess the reduction in the articular surface in patients requiring volar locked-plate fixation for DRF via fluoroscopy-guided open reduction and internal fixation (ORIF). METHODS: Seventy consecutive patients with DRF were prospectively enrolled. Posteroanterior and lateral radiographs and axial, coronal, and sagittal computed tomography (CT) scans were obtained before ORIF for DRF. The widest artic
