A 59-year-old man with rusty-colored sputum was admitted for evaluation of a nodular shadow on his chest radiograph. Chest computed tomography (CT) revealed nodules and nodular opacities with a cavity in the right S3. The preparative crude extract of Paragonimus heterotremus was fractionated by isoelectric focusing. Fractions at pH 5 which contained a specific antigen with a relative molecular weight of 31.5 kDa were pooled and used in an indirect enzyme-linked immunosorbent assay (ELISA) and immunoblot analysis for diagnosis of human paragonimiasis. With a diagnostic kit for paragonimiasis we recently developed, 112 active paragonimiasis cases were examined and 111 (99.1%) of them showed positive reaction. Sera of 100 healthy persons all showed negative reaction, while 2.3% (3/130) of the patients with other parasitic diseases showed a week cross reaction. 40 patients infected with P. westermani were divided into 3 groups and treated with praziquantel (2-cyclohexylcarbonyl-1,2,3,6,7,11b-hexahydro-4H-pyrazino[2,1-a]++ +isoquinolin- 4-one) by 3 different dosages. Nine cases of paragonimiasis have been encountered in Laotian Hmong immigrants from Camp Ban Vinai in Thailand. Symptoms included cough, hemoptysis, and fever. Chest x-ray films showed segmental infiltrates and pleural effusions, often bilateral. The clinical presentation mimics tuberculosis. All Hmong patients with chronic infiltrates and pleural disease in whom tuberculosis has not been proven should have parasitologic and serologic evaluation to exclude paragonimiasis.

The authors recapitulate in short the distribution of Paragonimiasis in West Africa. They establish and show keys for determination of adults and eggs of the agents of the disease (Paragonimus uterobilateralis, P. africanus, Poikilorchis congolensis). The first intermediate host is unknown but two species of snails are suspected (Afropomus balanoides, Potadoma sanctipauli). A very abundant crab (Liberonautes latidactylus) seems to be the second intermediate host.

Paragonimiasis is a disease caused by parasitic infections that mainly involve the lungs. However, it can also produce ectopic infections, such as when the parasites invade the liver, brain and subcutaneous tissue, which then cause different symptoms. This current case report describes a 55-year-old male patient with hepatic paragonimiasis that was misdiagnosed as liver cancer with rupture and haemorrhage.
