Quality Improvement Initiative to Prepare Medical Students With Harm Reduction Principles for Substance Use Care.

BACKGROUND: Undergraduate medical education is increasingly incorporating harm reduction (HR). Assessments of HR curricula have focused on skills like overdose reversal. The literature lacks evaluations of curricula that more broadly teach HR-based physician-patient communication. METHODS: From academic years 2022 to 2024, a Harvard Medical School student working group collaborated with faculty to take a quality improvement approach to incorporating HR into the substance use interview curriculum. We implemented two Plan-Do-Study-Act cycles. We measured the effectiveness of HR education in the

Cost-effectiveness of brief cognitive behaviour therapy versus treatment as usual in recurrent deliberate self-harm: a decision-making approach.

BACKGROUND: Deliberate self-harm can be costly, in terms of treatment and subsequent suicide. Any intervention that reduces episodes of self-harm might therefore have a major impact on the costs incurred by service providers and the productivity losses due to illness or premature death. METHOD: Four hundred and eighty patients with a history of recurrent deliberate self-harm were randomized to manual-assisted cognitive behaviour therapy (MACT) or treatment as usual. Economic data were collected from patients at baseline, 6 and 12 months, and these data were complete for 397 patients. Incremen

Misdiagnosis-related harm quantification through mixture models and harm measures.

Investigating and monitoring misdiagnosis-related harm is crucial for improving health care. However, this effort has traditionally focused on the chart review process, which is labor intensive, potentially unstable, and does not scale well. To monitor medical institutes' diagnostic performance and identify areas for improvement in a timely fashion, researchers proposed to leverage the relationship between symptoms and diseases based on electronic health records or claim data. Specifically, the elevated disease risk following a false-negative diagnosis can be used to signal potential harm. How

Immigrant experiences of harm in sexual and reproductive healthcare in Spain.

The medical encounter represents a site where patients may be harmed, with intersecting vulnerabilities shaping the risk and nature of this harm. Sexual and reproductive healthcare is an important site for exploring this dynamic. Questions concerning how immigrant women experience sexual and reproductive healthcare abound, with researchers and practitioners calling for greater attention to a population whose experiences are underrepresented in existing literature. As a case site, Spain is a high migration country with a stressed healthcare system where 'good' biomedical outcomes are complicate

Feasibility of needle and syringe programs in Tajikistan distributing low dead space needles.

BACKGROUND: In 2012, the World Health Organization recommended that needle and syringe programs offer their clients low dead space insulin syringes with permanently attached needles. However, in many countries, these syringes are not acceptable to a majority of people who inject drugs. This study assessed the feasibility of working with needle and syringe programs to implement the WHO recommendation using low dead space detachable needles. The study also assessed the acceptability of the needles. METHODS: Two needle and syringe programs in Tajikistan-one in Kulob and one in Khudjand-received

Understanding factors that could influence feasibility and acceptability of brief CAT for adults who self-harm: A qualitative exploration using the theoretical framework of acceptability.

INTRODUCTION: Self-harm in adults is a complex and highly prevalent problem. The Relational Approach to Treating Self-Harm (RELATE, ISRCTN code: ISRCTN75661422) trial investigated the feasibility, acceptability and safety of 8-session cognitive analytic therapy (CAT) for self-harm, compared with treatment as usual. This qualitative evaluation explored participants' experiences of study processes and of brief CAT. METHOD: Semi-structured one-to-one interviews were carried out with N = 24 participants and reflexive thematic analysis explored participants' experiences. RESULTS: Five themes were

Comprehensive evidence-based program teaching self-management of auditory hallucinations on inpatient psychiatric units.

Patients hearing command hallucinations to harm whose only self-management strategies are to obey these commands, can represent serious safety concerns on inpatient psychiatric units. A comprehensive evidence-based program teaching self-management of auditory hallucinations on inpatient psychiatric units is described that includes five components: suggestions for staff education; patient self-assessment tools; an interview guide and safety protocol; a course to teach strategies for managing distressing voices and commands to harm; suggestions to improve staff communication; and a plan to exten

Community Water Fluoridation a Cost-Benefit-Risk Consideration.

For over 70 years, the addition of fluoride to public water as community water fluoridation (CWF or fluoridation) with intent to prevent dental caries continues to be controversial, and risks are seldom included in monetary evaluations. Published operational costs and benefits of fluoridation are used, whereas published and clinical experience treating dental fluorosis are utilized to estimate treatment costs of patient-perceived dental fluorosis and lost wages from lower IQ (intelligence quotient). Published estimated caries averted, less operational costs at $8 PPPY (per person per year) wer

A Case Report of Harm-Related Obsessions in Pediatric Obsessive-Compulsive Disorder.

Harm-related, or "aggressive," obsessions are a symptom subtype in obsessive-compulsive disorder (OCD). Given the violent nature of these thoughts, the stark contrast between the child's character and the content of these obsessions often results in high levels of distress for not only the patient but also the family. This case report illustrates the application of family-based exposure and response prevention (ERP) for a 15-year-old male presenting with harm-related obsessions and mild depressive symptoms secondary to the OCD. The obsessions primarily revolved around the fear of acting on unw

'Tranq': perceptions of xylazine and harm reduction practices among people receiving treatment for substance use disorders.

BACKGROUND: Xylazine, commonly known as 'Tranq,' is a veterinary tranquilizer that is increasingly found in the recreational opioid supply, complicating the user experience. Xylazine-adulterated fentanyl is associated with a withdrawal syndrome that may not respond to usual treatment, and the opioid overdose reversal agent naloxone does not reverse the effect of xylazine.1 While it remains unclear whether xylazine directly increases overdose deaths, its presence in the drug supply likely elevates overall harm and morbidity. Moreover, due to unintended contact, xylazine poses an often unaccount

"The community's memory is timeless": exploring cultural and community context to inform the public health response to the overdose crisis in black communities.

BACKGROUND: Substance use treatment and harm reduction strategies are vital tools in addressing the overdose crisis, however, effectiveness depends on access and uptake. Little is known about perceptions of harm reduction and substance use treatment efforts among people who use drugs (PWUD) in minoritized communities and how to enhance acceptability and uptake of evidence-based care. Our aim was to explore perceptions of drug use, PWUD and approaches to harm reduction and treatment in an urban, predominantly Black neighborhood heavily impacted by overdose. METHODS: We conducted one-on-one, se

Barriers to calling emergency services amongst people who use substances in the event of overdose: A scoping review.

BACKGROUND: North America is grappling with an ongoing drug overdose crisis. While harm reduction measures like take-home naloxone kits, and supervised consumption sites, have helped reduce mortality, other strategies to address this public health emergency are required. Good Samaritan Laws (GSLs) offer legal protection for individuals who report overdoses, yet people who use substances (PWUS) may still hesitate to seek help due to concerns about existing legislation. This scoping review explores barriers preventing PWUS from calling emergency services for overdoses, along with potential solut

"It's a delicate balance": clinicians' experiences of providing heroin-assisted treatment.

BACKGROUND: Little attention has been paid to the experiences of clinicians and health personnel who provide heroin-assisted treatment (HAT). This study provides the first empirical findings about the clinicians' experiences of providing HAT in the Norwegian context. METHODS: 23 qualitative interviews were conducted with 31 clinicians shortly after HAT clinics opened in Norway's two largest cities: Oslo and Bergen. By inductive thematic analysis of interview transcripts, we identified what research participants experienced and viewed as the chief rewards and challenges of providing HAT. The s

Adverse Events During Dental Care for Children: Implications for Practitioner Health and Wellness.

Adverse events during dental care for children inevitably happen, despite precautionary procedures. Remedial measures have primarily focused upon affected children and their parents/caregivers. The purpose of this paper was to summarize the effects of adverse events upon practitioners involved in the incident who have been termed second victims. Affected practitioners may suffer negative emotions impinging upon their professional performance as well as deleterious personal health consequences, including substance abuse and depression/suicidal ideation. Peer support has been identified as an im
