Assessment of mood, affect, and personality.

Changes in mood, affect, and personality may herald the onset of neurologic disturbances and may precede alterations in intellect and other cognitive operations. Reliance on traditional methods of assessment, especially in the elderly, will often produce misleading results and fail to measure critical constructs such as inappropriateness, indifference, and pragnosia. Evaluation of mood in elderly and cognitively impaired subjects is complicated by the high frequency of physical symptoms in these patients that also occur in depression and may inflate rating scale scores. Newer instruments tend …

Childhood trauma in borderline personality disorder.

Subjects with borderline personality disorder (N = 21) or borderline traits (N = 11) and nonborderline subjects with closely related diagnoses (N = 23) were interviewed in depth regarding experiences of major childhood trauma. Significantly more borderline subjects (81%) gave histories of such trauma, including physical abuse (71%), sexual abuse (68%), and witnessing serious domestic violence (62%); abuse histories were less common in those with borderline traits and least common in the subjects with no borderline diagnosis. These results demonstrate a strong association between a diagnosis of…

Personality disorder: the patients psychiatrists dislike.

A sample of psychiatrists was asked to read a case vignette and indicate likely management and attitudes to the patient on a number of semantic-differential scales. Patients given a previous diagnosis of personality disorder (PD) were seen as more difficult and less deserving of care compared with control subjects who were not. The PD cases were regarded as manipulative, attention-seeking, annoying, and in control of their suicidal urges and debts. PD therefore appears to be an enduring pejorative judgement rather than a clinical diagnosis. It is proposed that the concept be abandoned.

Personality in torticollis: a controlled study.

The Eysenck Personality Questionnaire, the trait scales of the Leyton Obsessional Inventory, the Spielberger State Trait Anxiety Inventory, and a questionnaire assessing marital status and harmony were completed by 100 torticollis patients and a control group of 49 cervical spondylosis sufferers. Information regarding employment status, and events preceding onset of their complaint was also obtained. The two groups did not differ in terms of any of the personality dimensions evaluated, or in their self-reports of events prior to onset of their illness. The groups differed significantly with re…

An update on multiple personality disorder.

Within the last decade, multiple personality disorder has been diagnosed, treated, and studied with increasing frequency. A growing body of literature suggests that this condition is not a rarity but that its manifestations, often coexisting with or obscured by other phenomena, frequently go unrecognized for years within the mental health care delivery system. The author reviews recent advances in the understanding of the etiology, diagnosis, and treatment of multiple personality disorder. The thrust of the contemporary literature is that it is a difficult-to-diagnose condition that usually fo…

[Personality of patients with dermatologic artefacts].

35 female patients with dermatological artefacts being treated in the University Skin Clinic Basel have been investigated by a dermatologist … ulses) and autoaggressive tendencies. These results point to a depressive and schizoid-narcissistic personality-structure. The manipulations at the skin have different functions: by causing pain to themselves the patients try to feel their body-boundaries and to avoid fragmentations in the body-self; on the tactile level emotional experiences of early childho …

Multiple personality and primary affective disorder.

This report describes a young woman who met research criteria for both primary depression and multiple personality. Abnormal dexamethasone suppression test results supported the former diagnosis and returned to normal during electroconvulsive therapy in anticipation of depressive symptom resolution. This suggests that multiple personality may occur as an epiphenomenon of affective disorder or of other illnesses.

Diagnostic interactions. Alcoholism and antisocial personality.

The association of alcoholism with other psychiatric disorders is important from both a research and a therapeutic point of view. In a medically hospitalized inpatient sample, we found a strong relationship between alcoholism and antisocial personality. Controlling for the overlap of diagnostic symptomatology, antisocial subjects still had a significantly higher prevalence of alcoholism than nonantisocial. The antisocial individual was more likely to be exposed to problem drinking, and once exposed, he tended to be more susceptible to developing the full alcoholism syndrome. Gender and a famil…

Nightmares: clinical characteristics and personality patterns.

The authors evaluated the development and clinical course of nightmares and the personality patterns of people with this dosorder by studying 30 adults who had a current complaint of nightmares. Nightmares usually began in childhood or adolescence and were chronic. Psychological factors played a major role in the development and persistence of nightmares, major life events were often associated with the onset of the disorder, mental stress usually increased the frequency of events, and psychological testing indicated relatively high levels of psychopathology. Nightmares sufferers in general we…

Personality and symptom pattern in depression.

This paper reports relationships between symptoms and premorbid personality in a varied sample of depressed patients. Symptoms were rated by a psychiatrist at clinical interview; personality was rated by patients on a self-report after clinical improvement, using the Maudsley Personality Inventory and an inventory of obsessive, hysterical and oral personality. Additional ratings on the latter were obtained at interview with a relative. The most prominent finding was that patients with premorbid neuroticism also showed a neurotic rather than an endogenous symptom pattern. Additional relationshi…

Personality characteristics in patients with vertigo.

The role of emotional factors in patients with vertigo has been widely debated. Patients with vertigo, as well as patients in two separate control groups, were tested with the Eysenck Personality Inventory (EPI), the Cornell Medical Index (CMI), and a questionarie designed to evaluate the severity of the patients symptoms in order to further investigate the role of personality and its relationship to the symptom of vertigo. In patients with Meniere disease, a strong positive correlation was found between the severity of vertigo and the EPI and CMI scores. The concept of the psychological vulne…
