Development, validation and clinical utility of a risk prediction model for adverse pregnancy outcomes in women with gestational diabetes: The PeRSonal GDM model.

BACKGROUND: The ability to calculate the absolute risk of adverse pregnancy outcomes for an individual woman with gestational diabetes mellitus (GDM) would allow preventative and therapeutic interventions to be delivered to women at high-risk, sparing women at low-risk from unnecessary care. We aimed to develop, validate and evaluate the clinical utility of a prediction model for adverse pregnancy outcomes in women with GDM. METHODS: A prediction model development and validation study was conducted on data from a observational cohort. Participants included all women with GDM from three metrop

DNA Repair Capacity for Personalizing Risk and Treatment Response - Assay Development and Optimization in Human Peripheral Blood Mononuclear Cells (PBMCs).

DNA repair capacity (DRC) is the ability of a cell to repair DNA damage. Differential DRC plays an important role in human disease, including lung and other cancers. Measuring DRC could aid in translational disease research and in personalizing treatment. We developed and optimized a flow cytometry-based assay to measure individual DRC using GFP-expressing plasmids modified by ultraviolet (UV) light for nucleotide excision repair (NER) and restriction enzyme digestion to induce a blunt double-strand cut between promoter and GFP expression regions for nonhomologous end joining (NHEJ). The neonatal "sepsis work-up": personal reflections on the development of an evidence-based approach toward newborn infections in a managed care organization.

"Rule out sepsis" may be the most common discharge diagnosis among infants admitted to the neonatal intensive care unit. Although the frequency of sepsis, meningitis, and other confirmed bacterial infections has remained constant (between 1 and 5/1000 live births) for many years, the number of infants evaluated and treated is much higher. Each year in the United States, as many as 600 000 infants experience at least one evaluation for suspected bacterial infection during the birth hospitalization. The number treated is estimated at 130 000 to 400 000 per year. Despite massive overtreatment, de

[Kinetics of tissue sodium metabolism and the vascular reactivity to vasopressor agents in healthy persons with different degree of risk of developing arterial hypertension].

A study of 69 normal males, aged 20 to 54 years and running increased risks of arterial hypertension, demonstrated vascular hyper-reactivity to humoral vasoconstrictors, weakened gustatory sensitivity to salt and a tendency to sodium deposition in tissues, which were induced by increased salt uptake. Their plasma aldosterone levels were increased in the presence of salt loading and showed correlation to the degree of tissue sodium metabolism slow-down. No intergroup variation was demonstrated in diurnal sodium excretion, total metabolizing sodium in the body and natriuretic response to acute s

"There Was a Light Bulb Moment": Using Persona Dyads to Empathise, Explore and Develop Person-Centred Solutions in Mental Health Service Co-Design.

INTRODUCTION: Co-design of health services is an increasingly used method to meaningfully enable patient and public involvement, especially when working with priority populations whose perspectives are often overlooked in traditional top-down approaches. However, differences in power and influence among stakeholders can lead to asymmetry and the marginalisation of certain voices. We used persona dyads (patient and provider) to co-design improvements in Emergency Department care for people presenting with mental health concerns. METHODS: Persona dyads were used in two Experience-Based Co-Desig

Development of an MSPE-UHPLC-MS/MS method for bisphenols and alkylphenols in personal care products: a pilot study and preliminary safety assessment in China.

As daily essentials for skin protection and cosmetic enhancement, personal care products (PCPs) are used extensively worldwide. Due to their direct and prolonged contact with the skin, any hazardous ingredients they contain warrant serious attention. Although bisphenols (BPs) and alkylphenols (APs) are of growing concern for their known or potential endocrine-disrupting effects, most research has targeted food, water, and environmental samples, with comparatively few methods available for their analysis in PCPs. To assess the occurrence and potential risks of such substances in PCPs, this stud

Expanded nursing roles for person-centred care for people with cognitive impairment in hospitals: developing a complex intervention using a multi-method-approach.

BACKGROUND: People with cognitive impairment face several challenges during hospitalisation. Insufficient attention to person-centred care often means that individual needs are not recognised, which can lead to complications and adverse outcomes such as falls and a decrease in the nutritional status. To promote person-centred care on hospital wards, nurses acting as change agents possessing both clinical expertise and leadership skills are a key factor. We aimed to develop a complex person-centred care intervention for people with cognitive impairment delivered by Expanded Practice Nurses in a

Development and Preliminary Evaluation of an AI-Assisted Clinical Decision Support System for Personalized Musculo-Skeletal Pain Management in Integrative Nursing Practice.

Background/Objectives: Musculoskeletal pain is a common condition that negatively affects quality of life and functional capacity. Integrative nursing emphasizes patient-centered and non-pharmacological approaches; however, variability in clinical decision-making persists due to reliance on clinician experience. This study aimed to design, retrospectively train, and evaluate the preliminary feasibility of a machine learning-based clinical decision support framework for personalized pain management within an integrative nursing approach in a Traditional and Complementary Medicine setting. Development and external validation of a lightweight, explainable clinical decision support system for personalized perioperative risk stratification using electronic health records.

Late-preoperative risk stratification after final surgical scheduling may support perioperative risk communication, monitoring escalation, and resource coordination, yet many established calculators are difficult to automate within structured EHR workflows. We developed and externally validated LiteSurgFormer, a lightweight explainable MLP-attention risk-stratification model, in a retrospective multicenter cohort of 58,630 adult grade III-IV surgical patients treated at six Chinese sites from 2021 to 2024. Predictions were anchored after final operating-room schedule confirmation and primary s

Pharmaceuticals and personal care product modelling: Unleashing artificial intelligence and machine learning capabilities and impact on one health and sustainable development goals.

The presence of pharmaceutical and personal care products (PPCPs) in the environment poses a significant threat to environmental resources, given their potential risks to ecosystems and human health, even in trace amounts. While mathematical modelling offers a comprehensive approach to understanding the fate and transport of PPCPs in the environment, such studies have garnered less attention compared to field and laboratory investigations. This review examines the current state of modelling PPCPs, focusing on their sources, fate and transport mechanisms, and interactions within the whole ecosy

A prospective controlled, randomized clinical trial of kidney transplant recipients developed personalized tacrolimus dosing using model-based Bayesian Prediction.

For three decades, tacrolimus (Tac) dose adjustment in clinical practice has been calculated empirically according to the manufacturer's labeling based on a patient's body weight. Here, we developed and validated a Population pharmacokinetic (PPK) model including pharmacogenetics (cluster CYP3A4/CYP3A5), age, and hematocrit. Our study aimed to assess the clinical applicability of this PPK model in the achievement of Tac Co (therapeutic trough Tac concentration) compared to the manufacturer's labelling dosage. A prospective two-arm, randomized, clinical trial was conducted to determine Tac star

All together now: aggregating multiple records to develop a person-based dataset to integrate and enhance infectious disease surveillance in Ontario, Canada.

SETTING: Syndemics occur when two or more health conditions interact to increase morbidity and mortality and are exacerbated by social, economic, environmental, and political factors. Routine provincial surveillance in Ontario assesses and reports on the epidemiology of single infectious diseases separately. Therefore, we aimed to develop a method that allows disease overlaps to be examined routinely as a path to better understanding and addressing syndemics in Ontario. INTERVENTION: We extracted data for individuals with a record of chlamydia, gonorrhea, infectious syphilis, hepatitis B and
