Postamputation pain: a multidisciplinary review of epidemiology, mechanisms, prevention, and treatment.

Despite humanity's long experience with amputations, postamputation pain remains a highly prevalent, incompletely understood, and clinically challenging condition. There are two main types of postamputation pain: residual limb pain (including but not limited to the "stump") and phantom limb pain. Despite considerable overlap between the two, they also have distinct clinical features, risk factors, and pathophysiological mechanisms. Central, peripheral, and spinal mechanisms may all contribute to the protean manifestations of persistent postamputation pain; an improved understanding of these me…

[Effectiveness of applying the bioacoustic correction method in therapy of phantom pain syndrome in amputees after mine-blast injuries].

UNLABELLED: Phantom pain syndrome (PPS) has attracted the attention of scientists and physicians for many years. In recent decades, the number of patients with PPS has increased due to the growing number of victims of local conflicts, natural disasters, and industrial accidents, as well as the rise in occlusive vascular diseases of the lower extremities. Given the pathogenesis of PPS, treatment for this condition is aimed at suppressing the pathological activity of peripheral pacemakers and hyperexcitable neuronal aggregates and activating the antinociceptive system. Bioacoustic correction, an

A systematic review of the prevalence of postamputation and chronic neuropathic pain associated with combat injury in military personnel.

Combat trauma can lead to widespread tissue damage and limb loss. This may result in chronic neuropathic and post amputation pain, including phantom limb pain (PLP) and residual limb pain (RLP). The military population is distinct with respect to demographic, injury, and social characteristics compared with other amputation and trauma cohorts. We undertook a systematic review of studies of military personnel, with a history of combat injury, that reported a prevalence of any type of postamputation pain or chronic neuropathic pain, identified from Embase and MEDLINE databases.Using the inverse …

Referred Sensation Areas in Bilateral Upper Limb Amputee.

Phantom limb pain (PLP) following amputation considerably reduces the quality of life, given a difficult to treat pain of highly variate profile. The loss of sensory input induces a complex pattern of neuroplastic changes of the sensory neural pathways and their central projections. Referred sensation areas (RSAs) may occur on the stump as a consequence of amputation, providing a direct path towards the altered central sensory projections. Modulated electrical stimulation of RSAs was investigated in a long-term experiment in the case of a 62 years-old participant with bilateral upper limb ampu…

The case study is presented with long standing complex combination of phantom limb and neuropathic pain. A decrease in perceived levels of pain was reported which amounts to a 50% reduction in pain from baseline and an improved range of motion. Revision Targeted Muscle Reinnervation Improves Secondary Pain Insult in an Upper Extremity Amputee: A Case Report.

Targeted muscle reinnervation (TMR) has been shown to improve phantom and neuropathic pain in both the acute and chronic amputee population. Through rerouting of major peripheral nerves into a newly denervated muscle, TMR harnesses the plasticity of the brain, helping to revert the sensory cortex back toward the preinsult state, effectively reducing pain. We highlight a unique case of an above-elbow amputee for sarcoma who was initially treated with successful transhumeral TMR. Following inadvertent nerve biopsy of a TMR coaptation site, his pain returned, and he was unable to don his prosthet

Sleep paralysis and "the bedroom intruder": the role of the right superior parietal, phantom pain and body image projection.

Sleep paralysis (SP) is a common condition occurring either at sleep onset or sleep offset. During SP the sleeper experiences gross motor paralysis while the sensory system is clear. Hypnogogic and hypnopompic hallucinations are common during SP and may involve seeing, hearing, and sensing the presence of menacing intruders in one's bedroom. This "intruder" is often perceived as a shadowy humanoid figure. Supernatural accounts of this hallucinated intruder are common across cultures. In this paper, we postulate that a functional disturbance of the right parietal cortex explains the shadowy noc

Perioperative Pain Management for Major Limb Amputation - A Systematic-Narrative Hybrid Review.

INTRODUCTION: Post-amputation pain, including residual limb pain (RLP) and phantom limb pain (PLP), can affect the outcome of surgery and have an impact on quality of life. Effective management of acute post-amputation pain requires a multidisciplinary approach with collaboration among the surgical and anesthesia teams to optimize pain management. A systematic-narrative hybrid review was conducted to assess and report the effectiveness of various interventions in perioperative pain management for amputation surgery. METHODS: MEDLINE, Embase, CENTRAL, and ClinicalTrials.gov databases were sear…

Complex pattern of facial remapping in somatosensory cortex following congenital but not acquired hand loss.

Cortical remapping after hand loss in the primary somatosensory cortex (S1) is thought to be predominantly dictated by cortical proximity, w … of both forehead and lips in amputees, with no significant relationship to the chronicity of their phantom limb pain (PLP). In contrast, we found converging evidence for a complex pattern of face remapping in congenital one-handers across multiple facial parts, where relative to controls, the location of the cortical neighbour - the forehead - is shown to shi …

Diagnostic ultrasound of the residual limb: A narrative review.

Globally, 57.7 million people lived with traumatic limb loss in 2017, with the prevalence of amputation in the United States alone expected to reach 3.6 million by 2050. Pain is a common complication after limb loss, with up to 59% of patients experiencing residual limb pain (RLP). Although RLP is often due to a structural etiology, it is difficult to treat because the exact structure involved is frequently not apparent on history and physical examination alone. This narrative review aims to summarize the available literature on diagnostic ultrasound of the residual limb and examine the utilit

Current Understanding of Phantom Pain and its Treatment.

BACKGROUND: Phantom limb pain (PLP), defined as a painful sensation in a portion of the body that has been amputated, occurs in upwards of 80% of limb amputees and can significantly impact a patient's quality of life. First hypothesized in 1551, the disease has been poorly understood for much of this time. Still today, the exact etiology of the condition is yet to be elucidated. In the periphery, PLP resembles the neuronal changes seen in other neuropathic pain conditions. However, in the central nervous system (CNS), imaging studies suggest changes unique to PLP, such as cortical reorganizati…

Referred Sensation Areas in a Bilateral Toes Amputee.

Various mechanisms in generating phantom limb pain (PLP) have been hypothesized in the literature. However, there still is no clear understanding of how PLP develops and why it presents. Amputation leads to permanent anatomical and physiological changes of the neural path previously supplying the brain with sensory input, as well as to formation of referred sensation areas (RSAs) on the stump or its vicinity. Sensations may be evoked in the lost body part upon stimulation of RSAs that may be exploited as artificial sensory input. In this work, we present the analysis of RSA maps from a 45-year…

Virtual movements activate primary sensorimotor areas in amputees: report of three cases.

OBJECTIVE: In our multidisciplinary pain clinic, three patients with amputated limbs and with surgical indications for chronic motor cortex stimulation for phantom limb pain were selected for their ability to voluntarily move the missing limb. The sensation of being able to move a missing limb at will occurs quite frequently among traumatic amputees, but the ability to control it sufficiently to perform a functional magnetic resonance imaging (fMRI) examination is more rarely encountered. We used motor fMRI to study these virtual movements. METHODS: In two patients with upper-limb amputations…

Regenerative peripheral nerve interface following forequarter amputation: a case report.

BACKGROUND: Forequarter amputation is a radical surgical procedure for malignant bone and soft tissue tumors involving the shoulder girdle, typically indicated for cases where limb salvage is not feasible due to extensive local invasion. However, post-amputation neuroma and phantom limb pain are major complications, occurring in 80%-90% of cases and severely impairing patients' quality of life. Traditional nerve management techniques often fail to prevent recurrent neuroma formation. The Regenerative Peripheral Nerve Interface (RPNI) is an emerging technique designed to guide axonal regenerati…
