AIM: To ascertain the role of platelets in pathogenesis of clinical symptoms in patients with Q-fever.
MATERIAL AND METHODS: We studied hemostasis with estimation of functional platelet activity in 49 patients with Q-fever.
RESULTS: Hemorrhagic syndrome (HS) occurred in 34.4% patients with Q-fever (QF) during seasonal rise of morbidity. HS manifested with petechiae (12%), hematomas (32%), nasal bleeding (17%), stomatorrhagia (9%), melena (12%). Platelet-activating factor (PAF) may influence neuronal migration, and gyral anomaly in hemimegalencephaly is believed to result from a neuronal disorder. A 7-month-old girl with hemimegalencephaly presented with intractable seizures, for which graded hemispherectomy was performed. In the resected specimen, we could not detect PAF, and related enzyme activities were low. These results suggest a role of PAF in neuronal migration.

The distribution of PAI-1 in the plasma and platelets of normal individuals and of patients with platelet abnormalities was studied. An ELISA, capable of measuring PAI-1 in plasma at 1.5 ng/ml, and a functional assay of t-PA inhibition were used to assay platelet-free plasma (PFP), platelet-rich plasma in which the platelets were lysed (PRP) and serum. Platelet releasing activity was found in sera fractionated by 30% ammonium sulphate in seven of 11 patients with systemic rheumatoid vasculitis, two of 16 patients with cutaneous rheumatoid vasculitis, and three of 18 patients with clinically uncomplicated rheumatoid arthritis. Serial studies in three patients suggest a key role of platelet activation by serum factors in the pathogenesis of systemic rheumatoid vasculitis. IgG rheumatoid factor or IgG containing immune complexes, or both, may be responsible for this platelet releasing activity.
